x86, intel: Make MSR_IA32_MISC_ENABLE bit constants systematic

Replace somewhat arbitrary constants for bits in MSR_IA32_MISC_ENABLE
with verbose but systematic ones.  Add _BIT defines for all the rest
of them, too.
