Since function drivers will still be active until dwc3_disconnect_gadget()

as the endpoint is in the process of being disabled. Change the log

TI PD controller comes with notification mechanism to inform
the host on activity in the PD controller. In the current
driver, the required masks are not set. This patch enables
the following events in the interrupt mask register:
PowerStatusUpdate - Set whenever contents of the power status reg changes
DataStatusUpdate - Set whenever contents of the data status reg changes
PlugInsertOrRemoval - Set whenever USB plug status has changed
With this change, the interrupt flooding issue is not seen anymore.

REGMAP_I2C is not a user visible kconfig symbol so driver configs
should not "depend on" it. They should depend on I2C and then
select REGMAP_I2C.
If this worked, it was only because some other driver had set/enabled

Struct pci_driver contains a struct device_driver, so for PCI devices, it's
easy to convert a device_driver * to a pci_driver * with to_pci_driver().
The device_driver * is in struct device, so we don't need to also keep
track of the pci_driver * in struct pci_dev.
Replace pdev->driver with to_pci_driver(). This is a step toward removing
pci_dev->driver.

Make the drivers which use single-byte netdev addresses
(netdev->addr_len == 1) use the appropriate address setting
helpers.
arcnet copies from int variables and io reads a lot, so
add a helper for arcnet drivers to use.
Similar helper could be reused for phonet and appletalk
but there isn't any good central location where we could
put it, and netdevice.h is already very crowded.

Commit 7c75bde329 ("usb: musb: musb_dsps: request_irq() after
initializing musb") has inverted the calls to
dsps_setup_optional_vbus_irq() and dsps_create_musb_pdev() without
updating correctly the error path. dsps_create_musb_pdev() allocates and
registers a new platform device which must be unregistered and freed
with platform_device_unregister(), and this is missing upon
dsps_setup_optional_vbus_irq() error.
While on the master branch it seems not to trigger any issue, I observed
a kernel crash because of a NULL pointer dereference with a v5.10.70
stable kernel where the patch mentioned above was backported. With this
kernel version, -EPROBE_DEFER is returned the first time
dsps_setup_optional_vbus_irq() is called which triggers the probe to
error out without unregistering the platform device. Unfortunately, on
the Beagle Bone Black Wireless, the platform device still living in the
system is being used by the USB Ethernet gadget driver, which during the
boot phase triggers the crash.
My limited knowledge of the musb world prevents me to revert this commit
which was sent to silence a robot warning which, as far as I understand,
does not make sense. The goal of this patch was to prevent an IRQ to
fire before the platform device being registered. I think this cannot
ever happen due to the fact that enabling the interrupts is done by the
->enable() callback of the platform musb device, and this platform
device must be already registered in order for the core or any other
user to use this callback.
Hence, I decided to fix the error path, which might prevent future
errors on mainline kernels while also fixing older ones.

EP IN is hard-coded as ASYNC both in f_uac1 and f_uac2 but u_audio sends
steady number of audio frames in each USB packet, without any control.
This patch adds 'Playback Pitch 1000000' ctl analogous to the existing
'Capture Pitch 1000000' ctl. The calculation of playback req->length in
u_audio_iso_complete respects the Playback Pitch ctl value to 1ppm now.
Max. value for Playback Pitch is configured by the existing parameter
uac2_opts->fb_max, used also for the Capture Pitch.
Since the EP IN packet size can be increased by uac2_opts->fb_max now,
maxPacketSize for the playback direction is calculated by the same
algorithm as for the async capture direction in
f_uac2.c:set_ep_max_packet_size.

The command ring pointer is located at [6:63] bits of the command
ring control register (CRCR). All the control bits like command stop,
abort are located at [0:3] bits. While aborting a command, we read the
CRCR and set the abort bit and write to the CRCR. The read will always
give command ring pointer as all zeros. So we essentially write only
the control bits. Since we split the 64 bit write into two 32 bit writes,
there is a possibility of xHC command ring stopped before the upper
dword (all zeros) is written. If that happens, xHC updates the upper
dword of its internal command ring pointer with all zeros. Next time,
when the command ring is restarted, we see xHC memory access failures.
Fix this issue by only writing to the lower dword of CRCR where all
control bits are located.

Root-cause:
There is an issue like endpoint is not recognized as primed, when bus
have more pressure and the add dTD tripwire semaphore (ATDTW bit in
USBCMD register) that can cause the controller to ignore a dTD that is
added to a primed endpoint.
This issue observed with the Windows10 host machine.
Workaround:
The software must implement a periodic cycle, and check for each dTD,
if the endpoint is primed. It can do this by reading the corresponding
bits in the ENDPTPRIME and ENDPTSTAT registers. If these bits are read
at 0, the software needs to re-prime the endpoint by writing 1 to the
corresponding bit in the ENDPTPRIME register.
Added conditional revision check of 2.20[CI_REVISION_22].

This reverts commit ce3e90d5a0.
The commit in question added list of quirky hubs, but the match
implementation clearly hasn't been tested at all.
First, hub_udev->dev.parent does not represent a USB interface so using
to_usb_interface() makes no sense and we'd be passing a random pointer
to usb_match_id().
Second, if hub_udev is a root hub it doesn't even even represent a USB
device.

Commit 38225f2ef2 ("ARM/omap1: switch to use dma_direct_set_offset for

offset handling split into arch/arm/mach-omap1/usb.c and
drivers/usb/host/ohci-omap.c.
This can cause a randconfig build warning that now fails the build

A recent change that started reporting break events to the line
discipline caused the tty-buffer insertions to no longer be serialised
by inserting events also from the completion handler for the interrupt
endpoint.
Completion calls for distinct endpoints are not guaranteed to be
serialised. For example, in case a host-controller driver uses
bottom-half completion, the interrupt and bulk-in completion handlers
can end up running in parallel on two CPUs (high-and low-prio tasklets,
respectively) thereby breaking the tty layer's single producer
assumption.
Fix this by holding the read lock also when inserting characters from
the bulk endpoint.

Async feedback patches broke enumeration on Windows 10 previously fixed
by commit 789ea77310 ("usb: gadget: f_uac2: always increase endpoint
max_packet_size by one audio slot").
While the existing calculation for EP OUT capture for async mode yields
size+1 frame due to uac2_opts->fb_max > 0, playback side lost the +1
feature. Therefore the +1 frame addition must be re-introduced for
playback. Win10 enumerates the device only when both EP IN and EP OUT
max packet sizes are (at least) +1 frame.

Currently, the UVC function is activated when open on the corresponding
v4l2 device is called. On another open the activation of the function
fails since the deactivation counter in `usb_function_activate` equals
0. However the error is not returned to userspace since the open of the
v4l2 device is successful.
On a close the function is deactivated (since deactivation counter still
equals 0) and the video is disabled in `uvc_v4l2_release`, although the
UVC application potentially is streaming.
Move activation of UVC function to subscription on UVC_EVENT_SETUP
because there we can guarantee for a userspace application utilizing
UVC. Block subscription on UVC_EVENT_SETUP while another application
already is subscribed to it, indicated by `bool func_connected` in
`struct uvc_device`. Extend the `struct uvc_file_handle` with member
`bool is_uvc_app_handle` to tag it as the handle used by the userspace
UVC application.
With this a process is able to check capabilities of the v4l2 device
without deactivating the function for the actual UVC application.

UCSI does not tell the driver explicitly when the firmware
(PPM in UCSI lingo) has actually detected the partner
alternate modes, there is no specific change event for that.
That's why they have to be checked with any notification
that informs that PD contract with that partner has been
achieved.
Previously the alternate modes were checked always when the
firmware (PPM) informed that something with the partner had
changed, but on some platforms the EC firmware does not
generate separate events for generic partner changes at all.
On those platforms the EC firmware notifies the driver only
about connections, or separately about the PD contract if it
was not achieved soon enough after the initial connection
event.

The "poll worker" that is introduced here is first used for
checking partner alternate modes, but it can later be used
for any partner task that requires a separate job to be
scheduled to the connector specific workqueues.
The mechanism allows the partner device specific tasks to be
polling tasks and also delayed tasks if necessary.
By polling the partner alternate modes with this mechanism
the long command completion timeout value can be reduced
back to normal. The long command completion timeout was only
used to work around a problem on some platforms where the EC
firmware (PPM) didn't return BUSY even when it should with
the alt mode commands.

The USB2.0 spec chapter 11.24.2.13 says that the USB port which is going
under test needs to be put in suspend state before sending the test
command. Many hubs, don't enforce this precondition and they work fine
without this step. But there are some "special" hubs, which requires to
disable the port power before sending the test command.
Because the USB spec mention that the port should be suspended, also
do this step before sending the test command. This could rise the
problem with other hubs which are not compliant with the spec and the
test command will not work if the port is suspend. If such hubs are
found, a similar workaround like the disable part could be implemented
to skip the suspend port command.

Some CP2102 do not support event-insertion mode but return no error when
attempting to enable it.
This means that any event escape characters in the input stream will not
be escaped by the device and consequently regular data may be
interpreted as escape sequences and be removed from the stream by the
driver.
The reporter's device has batch number DCL00X etched into it and as
discovered by the SHA2017 Badge team, counterfeit devices with that
marking can be detected by sending malformed vendor requests. [1][2]
Tests confirm that the possibly counterfeit CP2102 returns a single byte
in response to a malformed two-byte part-number request, while an
original CP2102 returns two bytes. Assume that every CP2102 that behaves
this way also does not support event-insertion mode (e.g. cannot report
parity errors).

There is a race present where the DWC3 runtime resume runs in parallel
to the UDC unbind sequence. This will eventually lead to a possible
scenario where we are enabling the run/stop bit, without a valid
composition defined.
Thread#1 (handling UDC unbind):
usb_gadget_remove_driver()
-->usb_gadget_disconnect()
-->dwc3_gadget_pullup(0)
--> continue UDC unbind sequence
-->Thread#2 is running in parallel here
Thread#2 (handing next cable connect)
__dwc3_set_mode()
-->pm_runtime_get_sync()
-->dwc3_gadget_resume()
-->dwc->gadget_driver is NOT NULL yet
-->dwc3_gadget_run_stop(1)
--> _dwc3gadget_start()
...
Fix this by tracking the pullup disable routine, and avoiding resuming
of the DWC3 gadget. Once the UDC is re-binded, that will trigger the
pullup enable routine, which would handle enabling the DWC3 gadget.

The new wrapper functions usb_control_msg_send/recv accept stack
variables for USB message buffers and eliminate the need of manually
allocating temporary DMA buffers. The read wrapper also treats short
reads as errors. Hence use the wrappers instead of using
usb_control_msg() directly.
Note that the conversion of f81534a_ctrl_set_register() adds an extra an
extra allocation and memcpy for every retry. Since this function is
called rarely and retries are hopefully rare, the overhead should be
acceptable.

According USB spec each ISOC transaction should be performed in a
designated for that transaction interval. On bus errors or delays
in operating system scheduling of client software can result in no
packet being transferred for a (micro)frame. An error indication
should be returned as status to the client software in such a case.
Current implementation in case of missed/dropped interval send same
data in next possible interval instead of reporting missed isoc.
This fix complete requests with -ENODATA if interval elapsed.
HSOTG core in BDMA and Slave modes haven't HW support for
(micro)frames tracking, this is why SW should care about tracking
of (micro)frames. Because of that method and consider operating
system scheduling delays, added few additional checking's of elapsed
target (micro)frame:
1. Immediately before enabling EP to start transfer.
2. With any transfer completion interrupt.
3. With incomplete isoc in/out interrupt.
4. With EP disabled interrupt because of incomplete transfer.
5. With OUT token received while EP disabled interrupt (for OUT
transfers).
6. With NAK replied to IN token interrupt (for IN transfers).
As part of ISOC flow, additionally fixed 'current' and 'target' frame
calculation functions. In HS mode SOF limits provided by DSTS register
is 0x3fff, but in non HS mode this limit is 0x7ff.

That commit broke USB on all routers that have USB always powered on and
don't require toggling any GPIO. It's a majority of devices actually.
The original code worked and seemed safe: vcc GPIO is optional and
bcma_hci_platform_power_gpio() takes care of checking the pointer before
using it.
This revert fixes:

If the driver runs out of minor numbers it would release minor 0 and
allow another device to claim the minor while still in use.
Fortunately, registering the tty class device of the second device would
fail (with a stack dump) due to the sysfs name collision so no memory is
leaked.

For DEV_VER_V3 version there exist race condition between clearing
ep_sts.EP_STS_TRBERR and setting ep_cmd.EP_CMD_DRDY bit.
Setting EP_CMD_DRDY will be ignored by controller when
EP_STS_TRBERR is set. So, between these two instructions we have
a small time gap in which the EP_STSS_TRBERR can be set. In such case
the transfer will not start after setting doorbell.

This loop is supposed to loop until if reads something other than
CS_IDST or until it times out after 30,000 attempts. But because of
the || vs && bug, it will never time out and instead it will loop a

This bug is quite old but the code is only used in USB_DEVICE_TEST_MODE

The patch increases the bitshift in feedback frequency
calculation with EP-OUT bInterval value.
Tests have revealed that Win10 and OSX UAC2 drivers require
the feedback frequency to be based on the actual packet
interval instead of on the USB2 microframe. Otherwise they
ignore the feedback value. Linux snd-usb-audio driver
detects the applied bitshift automatically.

The variable master is being masked with ~MST_R_BITS however this
masked value is never used, the following updates to master are
assignments. I suspect the original intention was to mask out the
MST_R_BITS and then bit-wise or in the appropriate read bits rather
than perform an assignment. Fix this by using the |= operator rather
than a straight assignment.

Set reference clock period when it differs from dwc3 default hardware
set.
We could calculate clock period based on reference clock frequency. But
this information is not always available. This is the case of PCI bus
attached USB host. For that reason we use a custom property.
Tested (USB2 only) on IPQ6010 SoC based board with 24 MHz reference
clock while hardware default is 19.2 MHz.

As noted in the "Deprecated Interfaces, Language Features, Attributes,
and Conventions" documentation [1], size calculations (especially
multiplication) should not be performed in memory allocator (or similar)
function arguments due to the risk of them overflowing. This could lead
to values wrapping around and a smaller allocation being made than the
caller was expecting. Using those allocations could lead to linear
overflows of heap memory and other misbehaviors.
So, use the struct_size() helper to do the arithmetic instead of the
argument "size + count * size" in the kzalloc() function.

the final removal of scsi_ioctl, which has been deprecated for over a

The set/clear of the otg control values is done writing to
two different 16bit registers, however we setup the regmap
width for isp1760/61 to 32bit value bits.
So, just access the clear register address (0x376)as the high
part of the otg control register set (0x374), and write the
values in one go to make sure they get clear/set.

The issue happens only when the gadget is using the sync type "async", not

The problem is that request is freed regardless of usb_ep_dequeue(), which

If the port is going to send Discover_Identity Message, vdm_sm_running
flag was intentionally set before entering Ready States in order to
avoid the conflict because the port and the port partner might start
AMS at almost the same time after entering Ready States.
However, the original design has a problem. When the port is doing
DR_SWAP from Device to Host, it raises the flag. Later in the
tcpm_send_discover_work, the flag blocks the procedure of sending the
Discover_Identity and it might never be cleared until disconnection.
Since there exists another flag send_discover representing that the port
is going to send Discover_Identity or not, it is enough to use that flag
to prevent the conflict. Also change the timing of the set/clear of
vdm_sm_running to indicate whether the VDM SM is actually running or
not.

The recent attempt to handle an unknown ROM state in the commit
d143825baf ("usb: renesas-xhci: Fix handling of unknown ROM state")
resulted in a regression and reverted later by the commit 44cf53602f
("Revert "usb: renesas-xhci: Fix handling of unknown ROM state"").
The problem of the former fix was that it treated the failure of
firmware loading as a fatal error. Since the firmware files aren't
included in the standard linux-firmware tree, most users don't have
them, hence they got the non-working system after that. The revert
fixed the regression, but also it didn't make the firmware loading
triggered even on the devices that do need it. So we need still a fix
for them.
This is another attempt to handle the unknown ROM state. Like the
previous fix, this also tries to load the firmware when ROM shows
unknown state. In this patch, however, the failure of a firmware
loading (such as a missing firmware file) isn't handled as a fatal
error any longer when ROM has been already detected, but it falls back
to the ROM mode like before. The error is returned only when no ROM
is detected and the firmware loading failed.
Along with it, for simplifying the code flow, the detection and the
check of ROM is factored out from renesas_fw_check_running() and done
in the caller side, renesas_xhci_check_request_fw(). It avoids the
redundant ROM checks.
The patch was tested on Lenovo Thinkpad T14 gen (BIOS 1.34). Also it
was confirmed that no regression is seen on another Thinkpad T14
machine that has worked without the patch, too.

We can't depend on the TRB's HWO bit to determine if the TRB ring is
"full". A TRB is only available when the driver had processed it, not
when the controller consumed and relinquished the TRB's ownership to the
driver. Otherwise, the driver may overwrite unprocessed TRBs. This can
happen when many transfer events accumulate and the system is slow to
process them and/or when there are too many small requests.
If a request is in the started_list, that means there is one or more
unprocessed TRBs remained. Check this instead of the TRB's HWO bit

xhci-mtk depends on xhci's internal virt_dev when it retrieves its
internal data from usb_host_endpoint both in add_endpoint and
drop_endpoint callbacks. But when setup packet was retired by
transaction errors in xhci_setup_device() path, a virt_dev for the slot
is newly created with real_port 0. This leads to xhci-mtks's NULL pointer
dereference from drop_endpoint callback as xhci-mtk assumes that virt_dev's
real_port is always started from one. The similar problems were addressed
by [1] but that can't cover the failure cases from setup_device.
This patch drops the usages of xhci's virt_dev in xhci-mtk's drop_endpoint
callback by adopting hashtable for searching mtk's schedule entity
from a given usb_host_endpoint pointer instead of searching a linked list.
So mtk's drop_endpoint callback doesn't have to rely on virt_dev at all.

the pattern in the bus get changed to make sure we are testing
correctly the scratch pattern.
Clang-analyzer complains about the never read scratch variable:

just do a small format change in the error message bellow.

When a remote usb device is attached to the local Virtual USB
Host Controller Root Hub port, the bound device driver may send
a port reset command.
vhci_hcd accepts port resets only when the device doesn't have
port address assigned to it. When reset happens device is in
assigned/used state and vhci_hcd rejects it leaving the port in
a stuck state.
This problem was found when a blue-tooth or xbox wireless dongle
was passed through using usbip.
A few drivers reset the port during probe including mt76 driver
specific to this bug report. Fix the problem with a change to
honor reset requests when device is in used state (VDEV_ST_USED).

This reverts commit 5d5323a6f3.
That commit effectively disabled Intel host initiated U1/U2 lpm for devices
with periodic endpoints.
Before that commit we disabled host initiated U1/U2 lpm if the exit latency
was larger than any periodic endpoint service interval, this is according
to xhci spec xhci 1.1 specification section 4.23.5.2
After that commit we incorrectly checked that service interval was smaller
than U1/U2 inactivity timeout. This is not relevant, and can't happen for
Intel hosts as previously set U1/U2 timeout = 105% * service interval.
Patch claimed it solved cases where devices can't be enumerated because of
bandwidth issues. This might be true but it's a side effect of accidentally
turning off lpm.

Only TDs with status TD_CLEARING_CACHE will be given back after
cache is cleared with a set TR deq command.
xhci_invalidate_cached_td() failed to set the TD_CLEARING_CACHE status
for some cancelled TDs as it assumed an endpoint only needs to clear the
TD it stopped on.
This isn't always true. For example with streams enabled an endpoint may
have several stream rings, each stopping on a different TDs.
Note that if an endpoint has several stream rings, the current code
will still only clear the cache of the stream pointed to by the last
cancelled TD in the cancel list.
This patch only focus on making sure all canceled TDs are given back,
avoiding hung task after device removal.
Another fix to solve clearing the caches of all stream rings with
cancelled TDs is needed, but not as urgent.
This issue was simultanously discovered and debugged by
by Tao Wang, with a slightly different fix proposal.

f_hid provides the OUT Endpoint as only way for receiving reports
from the host. SETUP/SET_REPORT method is not supported, and this causes
a number of compatibility problems with various host drivers, especially
in the case of keyboard emulation using f_hid.
- Some hosts do not support the OUT Endpoint and ignore it,
so it becomes impossible for the gadget to receive a report
from the host. In the case of a keyboard, the gadget loses
the ability to receive the status of the LEDs.
- Some BIOSes/UEFIs can't work with HID devices with the OUT Endpoint
at all. This may be due to their bugs or incomplete implementation
of the HID standard.
For example, absolutely all Apple UEFIs can't handle the OUT Endpoint
if it goes after IN Endpoint in the descriptor and require the reverse
order (OUT, IN) which is a violation of the standard.
Other hosts either do not initialize gadgets with a descriptor
containing the OUT Endpoint completely (like some HP and DELL BIOSes
and embedded firmwares like on KVM switches), or initialize them,
but will not poll the IN Endpoint.
This patch adds configfs option no_out_endpoint=1 to disable
the OUT Endpoint and allows f_hid to receive reports from the host
via SETUP/SET_REPORT.
Previously, there was such a feature in f_hid, but it was replaced
by the OUT Endpoint [1] in the commit 99c5150058 ("usb: gadget: hidg:
register OUT INT endpoint for SET_REPORT"). So this patch actually
returns the removed functionality while making it optional.
For backward compatibility reasons, the OUT Endpoint mode remains
the default behaviour.
- The OUT Endpoint mode provides the report queue and reduces
USB overhead (eliminating SETUP routine) on transmitting a report
from the host.
- If the SETUP/SET_REPORT mode is used, there is no report queue,
so the userspace will only read last report. For classic HID devices
like keyboards this is not a problem, since it's intended to transmit
the status of the LEDs and only the last report is important.
This mode provides better compatibility with strange and buggy
host drivers.
Both modes passed USBCV tests. Checking with the USB protocol analyzer
also confirmed that everything is working as it should and the new mode
ensures operability in all of the described cases.

Commit a20dcf53ea ("usb: typec: tcpm: Respond Not_Supported if no
snk_vdo"), stops tcpm_pd_data_request() calling tcpm_handle_vdm_request()
when port->nr_snk_vdo is not set. But the VDM might be intended for an
altmode-driver, in which case nr_snk_vdo does not matter.
This change breaks the forwarding of connector hotplug (HPD) events
for displayport altmode on devices which don't set nr_snk_vdo.
tcpm_pd_data_request() is the only caller of tcpm_handle_vdm_request(),
so we can move the nr_snk_vdo check to inside it, at which point we
have already looked up the altmode device so we can check for this too.
Doing this check here also ensures that vdm_state gets set to
VDM_STATE_DONE if it was VDM_STATE_BUSY, even if we end up with
responding with PD_MSG_CTRL_NOT_SUPP later.
Note that tcpm_handle_vdm_request() was already sending
PD_MSG_CTRL_NOT_SUPP in some circumstances, after moving the nr_snk_vdo
check the same error-path is now taken when that check fails. So that
we have only one error-path for this and not two. Replace the
tcpm_queue_message(PD_MSG_CTRL_NOT_SUPP) used by the existing error-path
with the more robust tcpm_pd_handle_msg() from the (now removed) second
error-path.

On DWC_usb3 revisions 3.00a and newer (including DWC_usb31 and
DWC_usb32) the GUCTL1 register gained the DEV_DECOUPLE_L1L2_EVT
field (bit 31) which when enabled allows the controller in device
mode to treat USB 2.0 L1 LPM & L2 events separately.
After commit d1d90dd272 ("usb: dwc3: gadget: Enable suspend
events") the controller will now receive events (and therefore
interrupts) for every state change when entering/exiting either
L1 or L2 states. Since L1 is handled entirely by the hardware
and requires no software intervention, there is no need to even
enable these events and unnecessarily notify the gadget driver.
Enable the aforementioned bit to help reduce the overall interrupt
count for these L1 events that don't need to be handled while
retaining the events for full L2 suspend/wakeup.

populated from the device tree to decide the port PD capability. Adding
"pd-disable" property in device tree indicates that the port does not
support PD. If PD is not supported, the device tree property
"typec-power-opmode" shall be added to specify the advertised Rp value
if the port supports SRC role.

Currently xhci-mtk needs software-managed bandwidth allocation for
periodic endpoints, it allocates the microframe index for the first
start-split packet for each endpoint. As this index allocation logic
should avoid the conflicts with other full/low-speed periodic endpoints,
it uses the worst case byte budgets on high-speed bus bandwidth
For example, for an isochronos IN endpoint with 192 bytes budget,
it will consume the whole 4 u-frames(188 * 4) while the actual
full-speed bus budget should be just 192bytes.
This patch changes the low/full-speed bandwidth allocation logic
to use "approximate" best case budget for lower speed bandwidth
management. For the same endpoint from the above example, the
approximate best case budget is now reduced to (188 * 2) bytes.
Without this patch, many usb audio headsets with 3 interfaces
(audio input, audio output, and HID) cannot be configured
on xhci-mtk.

xhci-mtk depends on xhci's internal virt_dev when it retrieves its
internal data from usb_host_endpoint both in add_endpoint and
drop_endpoint callbacks.
This patch drops the usages of xhci's virt_dev in xhci-mtk's drop_endpoint
callback by adopting rhashtable for searching mtk's schedule entity
from a given usb_host_endpoint pointer instead of searching a linked list.
So mtk's drop_endpoint callback doesn't have to rely on virt_dev at all.

If the device is already in the runtime suspended state, any call to
the pullup routine will issue a runtime resume on the DWC3 core
device. If the USB gadget is disabling the pullup, then avoid having
to issue a runtime resume, as DWC3 gadget has already been
halted/stopped.
This fixes an issue where the following condition occurs:
usb_gadget_remove_driver()
-->usb_gadget_disconnect()
-->dwc3_gadget_pullup(0)
-->pm_runtime_get_sync() -> ret = 0
-->pm_runtime_put() [async]
-->usb_gadget_udc_stop()
-->dwc3_gadget_stop()
-->dwc->gadget_driver = NULL
...
dwc3_suspend_common()
-->dwc3_gadget_suspend()
-->DWC3 halt/stop routine skipped, driver_data == NULL
This leads to a situation where the DWC3 gadget is not properly
stopped, as the runtime resume would have re-enabled EP0 and event
interrupts, and since we avoided the DWC3 gadget suspend, these
resources were never disabled.

The list_for_each_entry_safe() macro saves the current item (n) and
the item after (n+1), so that n can be safely removed without
corrupting the list. However, when traversing the list and removing
items using gadget giveback, the DWC3 lock is briefly released,
allowing other routines to execute. There is a situation where, while
items are being removed from the cancelled_list using
dwc3_gadget_ep_cleanup_cancelled_requests(), the pullup disable
routine is running in parallel (due to UDC unbind). As the cleanup
routine removes n, and the pullup disable removes n+1, once the
cleanup retakes the DWC3 lock, it references a request who was already
removed/handled. With list debug enabled, this leads to a panic.
Ensure all instances of the macro are replaced where gadget giveback
is used.
Example call stack:
Thread#1:
__dwc3_gadget_ep_set_halt() - CLEAR HALT
-> dwc3_gadget_ep_cleanup_cancelled_requests()
->list_for_each_entry_safe()
->dwc3_gadget_giveback(n)
->dwc3_gadget_del_and_unmap_request()- n deleted[cancelled_list]
->spin_unlock
->Thread#2 executes
...
->dwc3_gadget_giveback(n+1)
->Already removed!
Thread#2:
dwc3_gadget_pullup()
->waiting for dwc3 spin_lock
...
->Thread#1 released lock
->dwc3_stop_active_transfers()
->dwc3_remove_requests()
->fetches n+1 item from cancelled_list (n removed by Thread#1)
->dwc3_gadget_giveback()
->dwc3_gadget_del_and_unmap_request()- n+1
deleted[cancelled_list]
->spin_unlock
Fix this condition by utilizing list_replace_init(), and traversing
through a local copy of the current elements in the endpoint lists.
This will also set the parent list as empty, so if another thread is
also looping through the list, it will be empty on the next iteration.
