xhci-hub.c tracks suspended ports in a suspended_port bitfield.
This is checked when responding to a Get_Status(PORT) request to see if a
port in running U0 state was recently resumed, and adds the required
USB_PORT_STAT_C_SUSPEND change bit in those cases.
The suspended_port bit was left uncleared if a device is disconnected
during suspend. The bit remained set even when a new device was connected
and enumerated. The set bit resulted in a incorrect Get_Status(PORT)
response with a bogus USB_PORT_STAT_C_SUSPEND change
bit set once the new device reached U0 link state.
USB_PORT_STAT_C_SUSPEND change bit is only used for USB2 ports, but
xhci-hub keeps track of both USB2 and USB3 suspended ports.

The get port status hub request code in xhci-hub.c will complete usb2
port resume signalling if signalling has been going on for long enough.
The code that completes the resume signalling, and the code that returns
the port status have gotten too intertwined, so separate them a bit.

Initially resume related USB2 variables were cleared once port
successfully resumed to U0. Later code was added to clean up
stale resume variables in case of port failed to resume to U0.
Clear the variables in one place after port is no longer resuming
or in suspended U3 state.

Now that we have a port structure for each port it makes sense to
move per port variables, timestamps and completions there.
Get rid of storing bitfileds and arrays of port specific items per bus.

XHCI host drivers may override the default xhci_hub_control() with
their own device specific function. To allow these host drivers to
call the xhci_hub_control() function from within their own
hub_control() callback and be built as a module, export the symbol
for xhci_hub_control.

some Intel Alder Lake xHC hosts on ChromeOS platforms need special
workarounds touching port registers at xHC pci host hibernate.
Export xhci_port_state_to_neutral() and xhci_find_slot_id_by_port()
so they can be called from xhci-pci.c and thus the xhci-pci module.

Turning off port power in shutdown did cause issues such as a laptop not
proprly powering off, and some specific usb devies failing to enumerate the
subsequent boot after a warm reset.
So revert this.

After xHC controller is started, either in probe or resume, it can take
a while before any of the connected usb devices are visible to the roothub
due to link training.
It's possible xhci driver loads, sees no acivity and suspends the host
before the USB device is visible.
In one testcase with a hotplugged xHC controller the host finally detected
the connected USB device and generated a wake 500ms after host initial
start.
If hosts didn't suspend the device duringe training it probablty wouldn't
take up to 500ms to detect it, but looking at specs reveal USB3 link
training has a couple long timeout values, such as 120ms
RxDetectQuietTimeout, and 360ms PollingLFPSTimeout.
So Add a 500ms grace period that keeps polling the roothub for 500ms after
start, preventing runtime suspend until USB devices are detected.

If ports are not turned off in shutdown then runtime suspended
self-powered USB devices may survive in U3 link state over S5.
During subsequent boot, if firmware sends an IPC command to program
the port in DISCONNECT state, it will time out, causing significant
delay in the boot time.
Turning off roothub port power is also recommended in xhci
specification 4.19.4 "Port Power" in the additional note.

The first U3 wake signal by the host may be lost if the USB 3 connection is
tunneled over USB4, with a runtime suspended USB4 host, and firmware
implemented connection manager.
Specs state the host must wait 100ms (tU3WakeupRetryDelay) before
resending a U3 wake signal if device doesn't respond, leading to U3 -> U0
link transition times around 270ms in the tunneled case.

A race between system resume and device-initiated resume may result in
runtime PM imbalance on USB2 root hub. If a device-initiated resume
starts and system resume xhci_bus_resume() directs U0 before hub driver
sees the resuming device in RESUME state, device-initiated resume will
not be finished in xhci_handle_usb2_port_link_resume(). In this case,
usb_hcd_end_port_resume() call is missing.
This changes calls usb_hcd_end_port_resume() if resuming device reaches
U0 to keep runtime PM balance.

xhci_reset() timeout was increased from 250ms to 10 seconds in order to
give Renesas 720201 xHC enough time to get ready in probe.
xhci_reset() is called with interrupts disabled in other places, and
waiting for 10 seconds there is not acceptable.
Add a timeout parameter to xhci_reset(), and adjust it back to 250ms
when called from xhci_stop() or xhci_shutdown() where interrupts are
disabled, and successful reset isn't that critical.
This solves issues when deactivating host mode on platforms like SM8450.
For now don't change the timeout if xHC is reset in xhci_resume().
No issues are reported for it, and we need the reset to succeed.
Locking around that reset needs to be revisited later.
Additionally change the signed integer timeout parameter in
xhci_handshake() to a u64 to match the timeout value we pass to
readl_poll_timeout_atomic()

Make xhci_disable_slot() synchronous, thus ensuring it, and
xhci_free_dev() calling it return after xHC controller completes
the disable slot command.
Otherwise the roothub and xHC host may runtime suspend, and clear the
command ring while the disable slot command is being processed.
This causes a command completion mismatch as the completion event can't
be mapped to the correct command.
Command ring gets out of sync and commands time out.
Driver finally assumes host is unresponsive and bails out.

Some USB 3.1 enumeration issues were reported after the hub driver removed
the minimum 100ms limit for the power-on-good delay.
Since commit 90d28fb53d ("usb: core: reduce power-on-good delay time of
root hub") the hub driver sets the power-on-delay based on the
bPwrOn2PwrGood value in the hub descriptor.
xhci driver has a 20ms bPwrOn2PwrGood value for both roothubs based
on xhci spec section 5.4.8, but it's clearly not enough for the
USB 3.1 devices, causing enumeration issues.
Tests indicate full 100ms delay is needed.

There's a small window where a USB 2 remote wake may be left unhandled
due to a race between hub thread and xhci port event interrupt handler.
When the resume event is detected in the xhci interrupt handler it kicks
the hub timer, which should move the port from resume to U0 once resume
has been signalled for long enough.
To keep the hub "thread" running we set a bus_state->resuming_ports flag.
This flag makes sure hub timer function kicks itself.
checking this flag was not properly protected by the spinlock. Flag was
copied to a local variable before lock was taken. The local variable was
then checked later with spinlock held.
If interrupt is handled right after copying the flag to the local variable
we end up stopping the hub thread before it can handle the USB 2 resume.

The current xhci_create_usb3_bos_desc() uses a static bos u8 array and
various magic numbers and offsets making it difficult to extend support
for USB 3.2. Let's rewrite this entire function to support dual-lane in
USB 3.2.
The hub driver matches the port speed ID from the extended port status
to the SSID of the sublink speed attributes to detect if the device
supports SuperSpeed Plus. Currently we don't provide the default gen1x2
and gen2x2 sublink speed capability descriptor for USB 3.2 roothub. The
USB stack depends on this to detect and match the correct speed.
In addition, if the xHCI host provides Protocol Speed ID (PSI)
capability, then make sure to convert Protocol Speed ID Mantissa and
Exponent (PSIM & PSIE) to lane speed for gen1x2 and gen2x2.

If a USB2 device wakeup is not enabled/supported the link state may
still be in U0 in xhci_bus_suspend(), where it's then manually put
to suspended U3 state.
Just as with selective suspend the device needs time to enter U3
suspend before continuing with further suspend operations
(e.g. system suspend), otherwise we may enter system suspend with link
state in U0.

Because PLS equals to XDEV_RESUME, xHCI driver reports U3 to usbcore,
despite of CAS bit is flagged.

Current xhci debug message doesn't always output bus number, so it's
hard to figure out it's from USB2 or USB3 root hub.
In addition to that, some port numbers are offset to 0 and others are
offset to 1. Use the latter to match the USB core.
So use "bus number - port index + 1" to make debug message consistent.

For some reason, the TEST_ defines in the usb/ch9.h files did not have
the USB_ prefix on it, making it a bit confusing when reading the file,
as well as not the nicest thing to do in a uapi file.
So fix that up and add the USB_ prefix on to them, and fix up all
in-kernel usages. This included deleting the duplicate copy in the
net2272.h file.

Suspending the bus and host controller while a port is in a over-current

Depending on the current link state the steps to resume the link to U0
varies. The normal case when a port is suspended (U3) we set the link
to U0 and wait for a port event when U3exit completed and port moved to
U0.
If the port is in U1/U2, then no event is issued, just set link to U0
If port is in Resume or Recovery state then the device has already
initiated resume, and this host initiated resume is racing against it.
Port event handler for device initiated resume will set link to U0,
just wait for the port to reach U0 before returning.

Like U3 case, xHCI spec doesn't specify the upper bound of U0 transition
time. The 20ms is not enough for some devices.
Intead of polling PLS or PLC, we can facilitate the port change event to
know that the link transits to U0 is completed.
While at it, also separate U0 and U3 case to make the code cleaner.

The xHCI spec doesn't specify the upper bound of U3 transition time. For
some devices 20ms is not enough, so we need to make sure the link state
is in U3 before further actions.
I've tried to use U3 Entry Capability by setting U3 Entry Enable in
config register, however the port change event for U3 transition
interrupts the system suspend process.
For now let's use the less ideal method by polling PLS.
[use usleep_range(), and shorten the delay time while polling -Mathias]

xhci driver assumed that xHC controllers have at most one custom
supported speed table (PSI) for all usb 3.x ports.
Memory was allocated for one PSI table under the xhci hub structure.
Turns out this is not the case, some controllers have a separate
"supported protocol capability" entry with a PSI table for each port.
This means each usb3 roothub port can in theory support different custom
speeds.
To solve this, cache all supported protocol capabilities with their PSI
tables in an array, and add pointers to the xhci port structure so that
every port points to its capability entry in the array.
When creating the SuperSpeedPlus USB Device Capability BOS descriptor
for the xhci USB 3.1 roothub we for now will use only data from the
first USB 3.1 capable protocol capability entry in the array.
This could be improved later, this patch focuses resolving
the memory leak.

spin_unlock_irqrestore() might be called with stale flags after
reading port status, possibly restoring interrupts to a incorrect
state.
If a usb2 port just finished resuming while the port status is read
the spin lock will be temporary released and re-acquired in a separate
function. The flags parameter is passed as value instead of a pointer,
not updating flags properly before the final spin_unlock_irqrestore()
is called.

A race in xhci USB3 remote wake handling may force device back to suspend
after it initiated resume siganaling, causing a missed resume event or warm
reset of device.
When a USB3 link completes resume signaling and goes to enabled (UO)
state a interrupt is issued and the interrupt handler will clear the
bus_state->port_remote_wakeup resume flag, allowing bus suspend.
If the USB3 roothub thread just finished reading port status before
the interrupt, finding ports still in suspended (U3) state, but hasn't
yet started suspending the hub, then the xhci interrupt handler will clear
the flag that prevented roothub suspend and allow bus to suspend, forcing
all port links back to suspended (U3) state.
Example case:

Fix this by increasing roothub usage count during port resume to prevent
roothub autosuspend, and by making sure bus_state->port_remote_wakeup
flag is only cleared after resume completion is visible, i.e.
after xhci roothub returned U0 or other non-U3 link state link on a
get port status request.

Improve port related dynamic debugging by printing out the bus number,
port number and port status register content each time there is a port
related debug messages.
Use the same port numbering method as usbcore to simplify debugging.
i.e. starting with port number 1.

Commit 2f31a67f01 ("usb: xhci: Prevent bus suspend if a port connect
change or polling state is detected") was intended to prevent ports that
were still link training from being forced to U3 suspend state mid
enumeration.
This solved enumeration issues for devices with slow link training.
Turns out some devices are stuck in the link training/polling state,
and thus that patch will prevent suspend completely for these devices.
This is seen with USB3 card readers in some MacBooks.
Instead of preventing suspend, give some time to complete the link
training. On successful training the port will end up as connected
and enabled.
If port instead is stuck in link training the bus suspend will continue
suspending after 360ms (10 * 36ms) timeout (tPollingLFPSTimeout).
Original patch was sent to stable, this one should go there as well

The code to prevent a bus suspend if a USB3 port was still in link training
also reacted to USB2 port polling state.
This caused bus suspend to busyloop in some cases.
USB2 polling state is different from USB3, and should not prevent bus
suspend.
Limit the USB3 link training state check to USB3 root hub ports only.
The origial commit went to stable so this need to be applied there as well

Refactoring, no functional changes.
But worth mentioning that checking for port link resume state is now behind
a additional port power check.
This is fine as ports can't be in resume state if port power bit is not
set.
xhci spec section 4.19.1.1.6 figure 34 shows that port power bit must be
set for all 'Enable' substates, including U0,U1,U2,U3 (suspended), Resume,
and RExit states.
