Add support for Altera 8250/16550 compatible serial port.

The loopback test mode is not implemented in all
NS16550 compatible UARTs. The 8250 driver uses the
UPF_SKIP_TEST flag to indicate this, however it is
not possible to set this flag via device-tree.
Add a new 'no-loopback-test' property, and modify
the of_serial driver to set the UPF_SKIP_TEST flag
if the property is present.

This patch adds a "compatible" string for the new 8250 UART type PORT_LPC3220.
This is necessary for initializing LPC32xx UARTs via DT.

Some platforms e.g. TI Davinci require 32-bit accesses to the UARTs.
The of_serial driver currently registers all UARTs as UPIO_MEM. Add a
new attribute "reg-io-width" to allow the port to be registered with
different IO width requirements.

The of_serial bindings can be used to register a number of serial
devices. Document this binding with all of the others.
