This patch refactors the LS_ACC paths to use SLI-4 as the primary
interface:
- Conversion away from using SLI-3 iocb structures to set/access fields in
common routines. Use the new generic get/set routines that were added.
This move changes code from indirect structure references to using local
variables with the generic routines.
- Refactor routines when setting non-generic fields, to have both SLI3 and
SLI4 specific sections. This replaces the set-as-SLI3 then translate to
SLI4 behavior of the past.

The patch refactors the general ELS handling paths to migrate to SLI-4
structures or common element abstractions. The fabric login paths are
revised as part of this patch:
- New generic lpfc_sli_prep_els_req_rsp jump table routine
- Introduce ls_rjt_error_be and ulp_bde64_le unions to correct legacy
endianness assignments
- Conversion away from using SLI-3 iocb structures to set/access fields in
common routines. Use the new generic get/set routines that were added.

- Refactor routines when setting non-generic fields, to have both SLI3 and

SLI4 behavior of the past.
- Clean up poor indentation on some of the ELS paths

Introduce lpfc_prep_wqe routine.
The lpfc_prep_wqe() routine is used with lpfc_sli_issue_iocb() and
lpfc_sli_issue_iocb_wait(). The routine performs additional SLI-4 wqe field
setting that the generic routines did not perform as they kept their
actions compatible with both SLI3 and SLI4.

Convert the SLI4 fast and slow paths to use native SLI4 wqe constructs
instead of iocb SLI3-isms.
Includes the following:
- Create simple get_xxx and set_xxx routines to wrapper access to common
elements in both SLI3 and SLI4 commands - allowing calling routines to
avoid sli-rev-specific structures to access the elements.
- using the wqe in the job structure as the primary element
- use defines from SLI-4, not SLI-3
- Removal of iocb to wqe conversion from fast and slow path
- Add below routines to handle fast path
lpfc_prep_embed_io - prepares the wqe for fast path
lpfc_wqe_bpl2sgl - manages bpl to sgl conversion
lpfc_sli_wqe2iocb - converts a WQE to IOCB for SLI-3 path
- Add lpfc_sli3_iocb2wcqecmpl in completion path to convert an SLI-3
iocb completion to wcqe completion
- Refactor some of the code that works on both revs for clarity

Currently, SLI3 and SLI4 data paths use the same lpfc_iocbq structure.
This is a "common" structure but many of the components refer to sli-rev
specific entities which can lead the developer astray as to what they
actually mean, should be set to, or when they should be used.
This first patch prepares the lpfc_iocbq structure so that elements common
to both SLI3 and SLI4 data paths are more appropriately named, making it
clear they apply generically.
Fieldnames based on 'iocb' (sli3) or 'wqe' (sli4) which are actually
generic to the paths are renamed to 'cmd':
- iocb_flag is renamed to cmd_flag
- lpfc_vmid_iocb_tag is renamed to lpfc_vmid_tag
- fabric_iocb_cmpl is renamed to fabric_cmd_cmpl
- wait_iocb_cmpl is renamed to wait_cmd_cmpl
- iocb_cmpl and wqe_cmpl are combined and renamed to cmd_cmpl
- rsvd2 member is renamed to num_bdes due to pre-existing usage
The structure name itself will retain the iocb reference as changing to a
more relevant "job" or "cmd" title induces many hundreds of line changes
for only a name change.
lpfc_post_buffer is also renamed to lpfc_sli3_post_buffer to indicate use
in the SLI3 path only.

The soft_wwpn/soft_wwn functionality, which allows the driver to modify
service parameters in an attempt to override the adapter-assigned WWN, was
originally attempted to be removed roughly 6 yrs ago as new fabric features
were being introduced that clashed with the implementation. In the end,
the feature was left in with the user being responsible if things went
south.
We've reached a point where soft_wwn is no longer functional and is failing
in almost all production use cases. Use of Fabric features such as Fabric
Assigned WWPN and Automatic DPORT is now prevalent and the features require
coordination between the adapter and driver that can't be solved by the
simplistic update of the service parameters. As it is no longer functional,
the feature is to be removed.
There are still ways to override the adapter-assigned WWN but they require
the admin to invoke bios/efi level menus.

The driver uses irq_set_affinity_hint to set the affinity for the lpfc
interrupts to a mask corresponding to the local NUMA node to avoid
performance overhead on AMD architectures.
However, irq_set_affinity_hint() setting the affinity is an undocumented
side effect that this function also sets the affinity under the hood.
To remove this side effect irq_set_affinity_hint() has been marked as
deprecated and new interfaces have been introduced.
Also, as per the commit dcaa21367938 ("scsi: lpfc: Change default IRQ model
on AMD architectures"):
"On AMD architecture, revert the irq allocation to the normal style
(non-managed) and then use irq_set_affinity_hint() to set the cpu affinity
and disable user-space rebalancing."
we don't really need to set the affinity_hint as user-space rebalancing for
the lpfc interrupts is not desired.
Hence, replace the irq_set_affinity_hint() with irq_set_affinity() which
only applies the affinity for the interrupts.

A prior patch reworked the get/put handling by ensuring nlp_get was done
before WQE submission and a put was done in the completion path.
Unfortunately, the issue_els_rscn path had a piece of legacy code that did
a nlp_put, causing an imbalance on the ref counts.
Fixed by removing the unnecessary legacy code snippet.

During heavy I/O testing with issue_lip to bounce the link, occasionally
I/O is terminated with status 3 result 9, which means the RPI is suspended.
The I/O is completed and this type of error will result in immediate retry
by the SCSI layer. The retry count expires and the I/O fails and returns
error to the application.
To avoid these quick retry/retries exhausted scenarios change the return
code given to the midlayer to DID_REQUEUE rather than DID_ERROR. This gets
them retried, and eventually succeed when the link recovers.

During rmmod testing, messages appeared indicating lpfc_mbuf_pool entries
were still busy. This situation was only seen doing rmmod after at least 1
vport (NPIV) instance was created and destroyed. The number of messages
scaled with the number of vports created.
When a vport is created, it can receive a PLOGI from another initiator
Nport. When this happens, the driver prepares to ack the PLOGI and
prepares an RPI for registration (via mbx cmd) which includes an mbuf
allocation. During the unsolicited PLOGI processing and after the RPI
preparation, the driver recognizes it is one of the vport instances and
decides to reject the PLOGI. During the LS_RJT preparation for the PLOGI,
the mailbox struct allocated for RPI registration is freed, but the mbuf
that was also allocated is not released.
Fix by freeing the mbuf with the mailbox struct in the LS_RJT path.
As part of the code review to figure the issue out a couple of other areas
where found that also would not have released the mbuf. Those are cleaned
up as well.

A commit introduced formal regstration of all Fabric nodes to the SCSI
transport as well as REG/UNREG RPI mailbox requests. The commit introduced
the NLP_RELEASE_RPI flag for rports set in the lpfc_cmpl_els_logo_acc()
routine to help clean up the RPIs. This new code caused the driver to
release the RPI value used for the remote port and marked the RPI invalid.
When the driver later attempted to re-login, it would use the invalid RPI
and the adapter rejected the PLOGI request. As no login occurred, the
devloss timer on the rport expired and connectivity was lost.
This patch corrects the code by removing the snippet that requests the rpi
to be unregistered. This change only occurs on a node that is already
marked to be rediscovered. This puts the code back to its original
behavior, preserving the already-assigned rpi value (registered or not)
which can be used on the re-login attempts.

This series consists of the usual driver updates (ufs, smartpqi, lpfc,
target, megaraid_sas, hisi_sas, qla2xxx) and minor updates and bug
fixes. Notable core changes are the removal of scsi->tag which caused
some churn in obsolete drivers and a sweep through all drivers to call
scsi_done() directly instead of scsi->done() which removes a pointer
indirection from the hot path and a move to register core sysfs files
earlier, which means they're available to KOBJ_ADD processing, which
necessitates switching all drivers to using attribute groups.

A link bounce to a slow fabric may observe FDISC response delays lasting
longer than devloss tmo. Current logic decrements the final fabric node
kref during a devloss tmo event. This results in a NULL ptr dereference
crash if the FDISC completes for that fabric node after devloss tmo.
Fix by adding the NLP_IN_RECOV_POST_DEV_LOSS flag, which is set when
devloss tmo triggers and we've noticed that fabric node recovery has
already started or finished in between the time lpfc_dev_loss_tmo_callbk
queues lpfc_dev_loss_tmo_handler. If fabric node recovery succeeds, then
the driver reverses the devloss tmo marked kref put with a kref get. If
fabric node recovery fails, then the final kref put relies on the ELS
timing out or the REG_LOGIN cmpl routine.

If an FC link down transition while PLOGIs are outstanding to fabric well
known addresses, outstanding ABTS requests may result in a NULL pointer
dereference. Driver unload requests may hang with repeated "2878" log
messages.
The Link down processing results in ABTS requests for outstanding ELS
requests. The Abort WQEs are sent for the ELSs before the driver had set
the link state to down. Thus the driver is sending the Abort with the
expectation that an ABTS will be sent on the wire. The Abort request is
stalled waiting for the link to come up. In some conditions the driver may
auto-complete the ELSs thus if the link does come up, the Abort completions
may reference an invalid structure.
Fix by ensuring that Abort set the flag to avoid link traffic if issued due
to conditions where the link failed.

A remote nport can stop responding to PLOGI beyond the ELS I/O timeout
under some fault conditions. When this happens, the non-response triggers
a dev_loss_tmo event from the transport which causes the driver to abort
the PLOGI and stop any retries. This was due to a policy in the ELS
completion handler whenever an ELS was terminated due to driver request.
Revise the ELS completion path to detect PLOGIs that were aborted and
allow retries.

An error is detected with the following report when unloading the driver:
"KASAN: use-after-free in lpfc_unreg_rpi+0x1b1b"
The NLP_REG_LOGIN_SEND nlp_flag is set in lpfc_reg_fab_ctrl_node(), but the
flag is not cleared upon completion of the login.
This allows a second call to lpfc_unreg_rpi() to proceed with nlp_rpi set
to LPFC_RPI_ALLOW_ERROR. This results in a use after free access when used
as an rpi_ids array index.
Fix by clearing the NLP_REG_LOGIN_SEND nlp_flag in
lpfc_mbx_cmpl_fc_reg_login().

Applications determine loop support in part by querying the 'pls' sysfs
node. Reporting of 'pls' (Private Loop Support) is derived from the
descriptor returned by the COMMON_GET_SLI4_PARAMETERS mailbox command,
which is issued during initialization or after a reset.
The value of this field may change if there is a dynamic SFP change. The
driver currently will not pick up the change as there was no reset
scenario.
Rework to commonize the sending of the COMMON_GET_SLI4_PARAMETERS
command. Add the calling of the routine after receipt of an async event
indicating an SFP change.

A prior patch introduced HBA_NEEDS_CFG_PORT flag logic, but in
lpfc_sli_brdrestart_s3() code path, right after HBA_NEEDS_CFG_PORT is set,
the phba->hba_flag is cleared in lpfc_sli_brdreset().
Fix by calling lpfc_sli_chipset_init() to wait for successful restart of
the HBA in lpfc_host_reset_handler() after lpfc_sli_brdrestart().
lpfc_sli_chipset_init() sets the HBA_NEEDS_CFG_PORT flag so that the
lpfc_sli_hba_setup() routine from lpfc_online() will execute
lpfc_sli_config_port() initialization step when the brdrestart is
successful.

In cases when lpfc_enable_pci_dev() fails, lpfc_printf_log() with
LOG_TRACE_EVENT set will call lpfc_dmp_dbg() which uses the
phba->port_list_lock.
However, phba->port_list_lock does not get initialized until
lpfc_setup_driver_resource_phase1(). Thus, any initialization routine with
LOG_TRACE_EVENT log message prior to lpfc_setup_driver_resource_phase1()
will crash.
Revert LOG_TRACE_EVENT back to LOG_INIT for all log messages in routines
prior to lpfc_setup_driver_resource_phase1().
