d3694f3073 dma-debug: reorder struct dma_debug_entry fields
Move all fields used during exact match lookups to the first cache line.
This makes debug_dma_mapping_error() and friends about 50% faster.

Since it removes two 32bit holes, force a cacheline alignment on struct
dma_debug_entry.

3acac06550 dma-mapping: merge the generic remapping helpers into dma-direct
Integrate the generic dma remapping implementation into the main flow.
This prepares for architectures like xtensa that use an uncached
segment for pages in the kernel mapping, but can also remap highmem
from CMA.  To simplify that implementation we now always deduct the
page from the physical address via the DMA address instead of the
virtual address.

34dc0ea6bc dma-direct: provide mmap and get_sgtable method overrides
For dma-direct we know that the DMA address is an encoding of the
physical address that we can trivially decode.  Use that fact to
provide implementations that do not need the arch_dma_coherent_to_pfn
architecture hook.  Note that we still can only support mmap of
non-coherent memory only if the architecture provides a way to set an
uncached bit in the page tables.  This must be true for architectures
that use the generic remap helpers, but other architectures can also
manually select it.

a445e940ea dma-mapping: fix handling of dma-ranges for reserved memory (again)
Daniele reported that issue previously fixed in c41f9ea998
("drivers: dma-coherent: Account dma_pfn_offset when used with device
tree") reappear shortly after 43fc509c3e ("dma-coherent: introduce
interface for default DMA pool") where fix was accidentally dropped.

Lets put fix back in place and respect dma-ranges for reserved memory.

2cf2aa6a69 dma-mapping: fix false positivse warnings in dma_common_free_remap()
Commit 5cf4537975 ("dma-mapping: introduce a dma_common_find_pages
helper") changed invalid input check in dma_common_free_remap() from:

    if (!area || !area->flags != VM_DMA_COHERENT)

to

    if (!area || !area->flags != VM_DMA_COHERENT || !area->pages)

which seem to produce false positives for memory obtained via
dma_common_contiguous_remap()

This triggers the following warning message when doing "reboot" on ZII
VF610 Dev Board Rev B:

249baa5479 dma-mapping: provide a better default ->get_required_mask
Most dma_map_ops instances are IOMMUs that work perfectly fine in 32-bits
of IOVA space, and the generic direct mapping code already provides its
own routines that is intelligent based on the amount of memory actually
present.  Wire up the dma-direct routine for the ARM direct mapping code
as well, and otherwise default to the constant 32-bit mask.  This way
we only need to override it for the occasional odd IOMMU that requires
64-bit IOVA support, or IOMMU drivers that are more efficient if they
can fall back to the direct mapping.

62fcee9a3b dma-mapping: remove CONFIG_ARCH_NO_COHERENT_DMA_MMAP
CONFIG_ARCH_NO_COHERENT_DMA_MMAP is now functionally identical to
!CONFIG_MMU, so remove the separate symbol.  The only difference is that
arm did not set it for !CONFIG_MMU, but arm uses a separate dma mapping
implementation including its own mmap method, which is handled by moving
the CONFIG_MMU check in dma_can_mmap so that is only applies to the
dma-direct case, just as the other ifdefs for it.

f9f3232a7d dma-mapping: explicitly wire up ->mmap and ->get_sgtable
While the default ->mmap and ->get_sgtable implementations work for the
majority of our dma_map_ops impementations they are inherently safe
for others that don't use the page allocator or CMA and/or use their
own way of remapping not covered by the common code.  So remove the
defaults if these methods are not wired up, but instead wire up the
default implementations for all safe instances.

arch_dma_mmap_pgprot is used for two things:

 1) to override the "normal" uncached page attributes for mapping
    memory coherent to devices that can't snoop the CPU caches
 2) to provide the special DMA_ATTR_WRITE_COMBINE semantics on older
    arm systems and some mips platforms

Replace one with the pgprot_dmacoherent macro that is already provided
by arm and much simpler to use, and lift the DMA_ATTR_WRITE_COMBINE
handling to common code with an explicit arch opt-in.

90ae409f9e dma-direct: fix zone selection after an unaddressable CMA allocation
The new dma_alloc_contiguous hides if we allocate CMA or regular
pages, and thus fails to retry a ZONE_NORMAL allocation if the CMA
allocation succeeds but isn't addressable.  That means we either fail
outright or dip into a small zone that might not succeed either.

33dcb37cef dma-mapping: fix page attributes for dma_mmap_*
All the way back to introducing dma_common_mmap we've defaulted to mark
the pages as uncached.  But this is wrong for DMA coherent devices.
Later on DMA_ATTR_WRITE_COMBINE also got incorrect treatment as that
flag is only treated special on the alloc side for non-coherent devices.

Introduce a new dma_pgprot helper that deals with the check for coherent
devices so that only the remapping cases ever reach arch_dma_mmap_pgprot
and we thus ensure no aliasing of page attributes happens, which makes
the powerpc version of arch_dma_mmap_pgprot obsolete and simplifies the
remaining ones.

Note that this means arch_dma_mmap_pgprot is a bit misnamed now, but
we'll phase it out soon.

c6622a425a dma-contiguous: do not overwrite align in dma_alloc_contiguous()
The dma_alloc_contiguous() limits align at CONFIG_CMA_ALIGNMENT for
cma_alloc() however it does not restore it for the fallback routine.
This will result in a size mismatch between the allocation and free
when running into the fallback routines after cma_alloc() fails, if
the align is larger than CONFIG_CMA_ALIGNMENT.

This patch adds a cma_align to take care of cma_alloc() and prevent
the align from being overwritten.

449fa54d68 dma-direct: correct the physical addr in dma_direct_sync_sg_for_cpu/device
dma_map_sg() may use swiotlb buffer when the kernel command line includes
"swiotlb=force" or the dma_addr is out of dev->dma_mask range.  After
DMA complete the memory moving from device to memory, then user call
dma_sync_sg_for_cpu() to sync with DMA buffer, and copy the original
virtual buffer to other space.

So dma_direct_sync_sg_for_cpu() should use swiotlb physical addr, not
the original physical addr from sg_phys(sg).

dma_direct_sync_sg_for_device() also has the same issue, correct it as
well.

9087c37584 dma-direct: Force unencrypted DMA under SME for certain DMA masks
If a device doesn't support DMA to a physical address that includes the
encryption bit (currently bit 47, so 48-bit DMA), then the DMA must
occur to unencrypted memory. SWIOTLB is used to satisfy that requirement
if an IOMMU is not active (enabled or configured in passthrough mode).

However, commit fafadcd165 ("swiotlb: don't dip into swiotlb pool for
coherent allocations") modified the coherent allocation support in
SWIOTLB to use the DMA direct coherent allocation support. When an IOMMU
is not active, this resulted in dma_alloc_coherent() failing for devices
that didn't support DMA addresses that included the encryption bit.

Addressing this requires changes to the force_dma_unencrypted() function
in kernel/dma/direct.c. Since the function is now non-trivial and
SME/SEV specific, update the DMA direct support to add an arch override
for the force_dma_unencrypted() function. The arch override is selected
when CONFIG_AMD_MEM_ENCRYPT is set. The arch override function resides in
the arch/x86/mm/mem_encrypt.c file and forces unencrypted DMA when either
SEV is active or SME is active and the device does not support DMA to
physical addresses that include the encryption bit.

d98849aff8 dma-direct: handle DMA_ATTR_NO_KERNEL_MAPPING in common code
DMA_ATTR_NO_KERNEL_MAPPING is generally implemented by allocating
normal cacheable pages or CMA memory, and then returning the page
pointer as the opaque handle.  Lift that code from the xtensa and
generic dma remapping implementations into the generic dma-direct
code so that we don't even call arch_dma_alloc for these allocations.

allocation flags.  Switch over the uncached segment check to this helper
to deal with architectures that do not support the dma_cache_sync

4a54d16f61 dma-mapping: truncate dma masks to what dma_addr_t can hold
The dma masks in struct device are always 64-bits wide.  But for builds
using a 32-bit dma_addr_t we need to ensure we don't store an
unsupportable value.  Before Linux 5.0 this was handled at least by
the ARM dma mapping code by never allowing to set a larger dma_mask,
but these days we allow the driver to just set the largest supported
value and never fall back to a smaller one.  Ensure this always works
by truncating the value.

the swiotlb_tbl_map_single() function truncates its return code
in the failure path, making it impossible to identify the error
later, as we compare to the original value:

kernel/dma/swiotlb.c:551:9: error: implicit conversion from 'dma_addr_t' (aka 'unsigned long long') to 'phys_addr_t' (aka 'unsigned int') changes value from 18446744073709551615 to 4294967295 [-Werror,-Wconstant-conversion]
        return DMA_MAPPING_ERROR;

Use an explicit typecast here to convert it to the narrower type,
and use the same expression in the error handling later.

With architectures allowing the kernel to be placed almost arbitrarily
in memory (e.g.: ARM64), it is possible to have the kernel resides at
physical addresses above 4GB, resulting in neither the default CMA area,
nor the atomic pool from successfully allocating. This does not prevent
specific peripherals from working though, one example is XHCI, which
still operates correctly.

4aa095ea32 swiotlb: Return consistent SWIOTLB segments/nr_tbl
With a specifically contrived memory layout where there is no physical
memory available to the kernel below the 4GB boundary, we will fail to
perform the initial swiotlb_init() call and set no_iotlb_memory to true.

There are drivers out there that call into swiotlb_nr_tbl() to determine
whether they can use the SWIOTLB. With the right DMA_BIT_MASK() value
for these drivers (say 64-bit), they won't ever need to hit
swiotlb_tbl_map_single() so this can go unoticed and we would be
possibly lying about those drivers.

bd2e75633c dma-contiguous: use fallback alloc_pages for single pages
The addresses within a single page are always contiguous, so it's
not so necessary to always allocate one single page from CMA area.
Since the CMA area has a limited predefined size of space, it may
run out of space in heavy use cases, where there might be quite a
lot CMA pages being allocated for single pages.

However, there is also a concern that a device might care where a
page comes from -- it might expect the page from CMA area and act
differently if the page doesn't.

This patch tries to use the fallback alloc_pages path, instead of
one-page size allocations from the global CMA area in case that a
device does not have its own CMA area. This'd save resources from
the CMA global area for more CMA allocations, and also reduce CMA
fragmentations resulted from trivial allocations.

Both dma_alloc_from_contiguous() and dma_release_from_contiguous() are
very simply implemented, but requiring callers to pass certain
parameters like count and align, and taking a boolean parameter to check
__GFP_NOWARN in the allocation flags. So every function call duplicates
similar work:

Additionally, as CMA can be used only in the context which permits
sleeping, most of callers do a gfpflags_allow_blocking() check and a
corresponding fallback allocation of normal pages upon any false result:

83ca259489 swiotlb: dump used and total slots when swiotlb buffer is full
So far the kernel only prints the requested size if swiotlb buffer if full.
It is not possible to know whether it is simply an out of buffer, or it is
because swiotlb cannot allocate buffer with the requested size due to
fragmentation.

As 'io_tlb_used' is available since commit 71602fe6d4 ("swiotlb: add
debugfs to track swiotlb buffer usage"), both 'io_tlb_used' and
'io_tlb_nslabs' are printed when swiotlb buffer is full.

a0bf842e89 swiotlb: add checks for the return value of memblock_alloc*()
Add panic() calls if memblock_alloc() returns NULL.

The panic() format duplicates the one used by memblock itself and in
order to avoid explosion with long parameters list replace open coded
allocation size calculations with a local variable.

133d624b1c dma: Introduce dma_max_mapping_size()
The function returns the maximum size that can be mapped
using DMA-API functions. The patch also adds the
implementation for direct DMA and a new dma_map_ops pointer

ffe3dfd4e3 powerpc/dma: stop overriding dma_get_required_mask
The ppc_md and pci_controller_ops methods are unused now and can be
removed.  The dma_nommu implementation is generic to the generic one
except for using max_pfn instead of calling into the memblock API,
and all other dma_map_ops instances implement a method of their own.

71602fe6d4 swiotlb: add debugfs to track swiotlb buffer usage
The device driver will not be able to do dma operations once swiotlb buffer
is full, either because the driver is using so many IO TLB blocks inflight,
or because there is memory leak issue in device driver. To export the
swiotlb buffer usage via debugfs would help the user estimate the size of
swiotlb buffer to pre-allocate or analyze device driver memory leak issue.

2e05ea5cdc dma-mapping: implement dma_map_single_attrs using dma_map_page_attrs
And also switch the way we implement the unmap side around to stay
consistent.  This ensures dma-debug works again because it records which
function we used for mapping to ensure it is also used for unmapping,
and also reduces further code duplication.  Last but not least this
also officially allows calling dma_sync_single_* for mappings created
using dma_map_page, which is perfectly fine given that the sync calls
only take a dma_addr_t, but not a virtual address or struct page.

If we want to map memory from the DMA allocator to userspace it must be
zeroed at allocation time to prevent stale data leaks.   We already do
this on most common architectures, but some architectures don't do this
yet, fix them up, either by passing GFP_ZERO when we use the normal page
allocator or doing a manual memset otherwise.

c92a54cfa0 dma-direct: do not include SME mask in the DMA supported check
The dma_direct_supported() function intends to check the DMA mask against
specific values. However, the phys_to_dma() function includes the SME
encryption mask, which defeats the intended purpose of the check. This
results in drivers that support less than 48-bit DMA (SME encryption mask
is bit 47) from being able to set the DMA mask successfully when SME is
active, which results in the driver failing to initialize.

Change the function used to check the mask from phys_to_dma() to
__phys_to_dma() so that the SME encryption mask is not part of the check.

While the dma-direct code is (relatively) clean and simple we actually
have to use the swiotlb ops for the mapping on many architectures due
to devices with addressing limits.  Instead of keeping two
implementations around this commit allows the dma-direct
implementation to call the swiotlb bounce buffering functions and
thus share the guts of the mapping implementation.  This also
simplified the dma-mapping setup on a few architectures where we
don't have to differenciate which implementation to use.

Only report report a DMA addressability report once to avoid spewing the
kernel log with repeated message.  Also provide a stack trace to make it
easy to find the actual caller that caused the problem.

Instead of providing a special dma_mark_clean hook just for ia64, switch
ia64 to use the normal arch_sync_dma_for_cpu hooks instead.

This means that we now also set the PG_arch_1 bit for pages in the
swiotlb buffer, which isn't stricly needed as we will never execute code
out of the swiotlb buffer, but otherwise harmless.

90ac706e98 dma-mapping: factor out dummy DMA ops
The dummy DMA ops are currently used by arm64 for any device which has
an invalid ACPI description and is thus barred from using DMA due to not
knowing whether is is cache-coherent or not. Factor these out into
general dma-mapping code so that they can be referenced from other
common code paths. In the process, we can prune all the optional
callbacks which just do the same thing as the default behaviour, and
fill in .map_resource for completeness.

3731c3d477 dma-mapping: always build the direct mapping code
All architectures except for sparc64 use the dma-direct code in some
form, and even for sparc64 we had the discussion of a direct mapping
mode a while ago.  In preparation for directly calling the direct
mapping code don't bother having it optionally but always build the
code in.  This is a minor hardship for some powerpc and arm configs
that don't pull it in yet (although they should in a relase ot two),
and sparc64 which currently doesn't need it at all, but it will
reduce the ifdef mess we'd otherwise need significantly.

ad78dee0b6 dma-debug: Batch dma_debug_entry allocation
DMA debug entries are one of those things which aren't that useful
individually - we will always want some larger quantity of them - and
which we don't really need to manage the exact number of - we only care
about having 'enough'. In that regard, the current behaviour of creating
them one-by-one leads to a lot of unwarranted function call overhead and
memory wasted on alignment padding.

Now that we don't have to worry about freeing anything via
dma_debug_resize_entries(), we can optimise the allocation behaviour by
grabbing whole pages at once, which will save considerably on the
aforementioned overheads, and probably offer a little more cache/TLB
locality benefit for traversing the lists under normal operation. This
should also give even less reason for an architecture-level override of
the preallocation size, so make the definition unconditional - if there
is still any desire to change the compile-time value for some platforms
it would be better off as a Kconfig option anyway.

Since freeing a whole page of entries at once becomes enough of a
challenge that it's not really worth complicating dma_debug_init(), we
may as well tweak the preallocation behaviour such that as long as we
manage to allocate *some* pages, we can leave debugging enabled on a
best-effort basis rather than otherwise wasting them.

ceb51173b2 dma-debug: Make leak-like behaviour apparent
Now that we can dynamically allocate DMA debug entries to cope with
drivers maintaining excessively large numbers of live mappings, a driver
which *does* actually have a bug leaking mappings (and is not unloaded)
will no longer trigger the "DMA-API: debugging out of memory - disabling"
message until it gets to actual kernel OOM conditions, which means it
could go unnoticed for a while. To that end, let's inform the user each
time the pool has grown to a multiple of its initial size, which should
make it apparent that they either have a leak or might want to increase
the preallocation size.

2b9d9ac02b dma-debug: Dynamically expand the dma_debug_entry pool
Certain drivers such as large multi-queue network adapters can use pools
of mapped DMA buffers larger than the default dma_debug_entry pool of
65536 entries, with the result that merely probing such a device can
cause DMA debug to disable itself during boot unless explicitly given an
appropriate "dma_debug_entries=..." option.

Developers trying to debug some other driver on such a system may not be
immediately aware of this, and at worst it can hide bugs if they fail to
realise that dma-debug has already disabled itself unexpectedly by the
time their code of interest gets to run. Even once they do realise, it
can be a bit of a pain to emprirically determine a suitable number of
preallocated entries to configure, short of massively over-allocating.

There's really no need for such a static limit, though, since we can
quite easily expand the pool at runtime in those rare cases that the
preallocated entries are insufficient, which is arguably the least
surprising and most useful behaviour. To that end, refactor the
prealloc_memory() logic a little bit to generalise it for runtime
reallocations as well.

0c3b3171ce dma-mapping: move the arm64 noncoherent alloc/free support to common code
The arm64 codebase to implement coherent dma allocation for architectures
with non-coherent DMA is a good start for a generic implementation, given
that is uses the generic remap helpers, provides the atomic pool for
allocations that can't sleep and still is realtively simple and well
tested.  Move it to kernel/dma and allow architectures to opt into it
using a config symbol.  Architectures just need to provide a new
arch_dma_prep_coherent helper to writeback an invalidate the caches
for any memory that gets remapped for uncached access.

f0edfea8ef dma-mapping: move the remap helpers to a separate file
The dma remap code only makes sense for not cache coherent architectures
(or possibly the corner case of highmem CMA allocations) and currently
is only used by arm, arm64, csky and xtensa.  Split it out into a
separate file with a separate Kconfig symbol, which gets the right
copyright notice given that this code was written by Laura Abbott
working for Code Aurora at that point.

a4a4330db4 swiotlb: add support for non-coherent DMA
Handle architectures that are not cache coherent directly in the main
swiotlb code by calling arch_sync_dma_for_{device,cpu} in all the right
places from the various dma_map/unmap/sync methods when the device is
non-coherent.

Because swiotlb now uses dma_direct_alloc for the coherent allocation
that side is already taken care of by the dma-direct code calling into
arch_dma_{alloc,free} for devices that are non-coherent.

fafadcd165 swiotlb: don't dip into swiotlb pool for coherent allocations
All architectures that support swiotlb also have a zone that backs up
these less than full addressing allocations (usually ZONE_DMA32).

Because of that it is rather pointless to fall back to the global swiotlb
buffer if the normal dma direct allocation failed - the only thing this
will do is to eat up bounce buffers that would be more useful to serve
streaming mappings.

dff8d6c1ed swiotlb: remove the overflow buffer
Like all other dma mapping drivers just return an error code instead
of an actual memory buffer.  The reason for the overflow buffer was
that at the time swiotlb was invented there was no way to check for

All properly written drivers now have error handling in the
dma_map_single / dma_map_page callers.  As swiotlb_tbl_map_single already
prints a useful warning when running out of swiotlb pool space we can

99c65fa7c5 dma-debug: Check for drivers mapping invalid addresses in dma_map_single()
I recently debugged a DMA mapping oops where a driver was trying to map
a buffer returned from request_firmware() with dma_map_single(). Memory
returned from request_firmware() is mapped into the vmalloc region and
this isn't a valid region to map with dma_map_single() per the DMA
documentation's "What memory is DMA'able?" section.

Unfortunately, we don't really check that in the DMA debugging code, so
enabling DMA debugging doesn't help catch this problem. Let's add a new
DMA debug function to check for a vmalloc address or an invalid virtual
address and print a warning if this happens. This makes it a little
easier to debug these sorts of problems, instead of seeing odd behavior
or crashes when drivers attempt to map the vmalloc space for DMA.

1fc8e6423e dma-direct: fix return value of dma_direct_supported
It appears that in commit 9d7a224b46 ("dma-direct: always allow dma mask
<= physiscal memory size") the logic of the test was changed from a "<" to
a ">=" however I don't see any reason for that change. I am assuming that
there was some additional change planned, specifically I suspect the logic
was intended to be reversed and possibly used for a return. Since that is
the case I have gone ahead and done that.

This addresses issues I had on my system that prevented me from booting
with the above mentioned commit applied on an x86_64 system w/ Intel IOMMU.

This is somewhat modelled after the powerpc version, and differs from
the legacy fallback in use fls64 instead of pointlessly splitting up the

We can use the arch_dma_coherent_to_pfn hook to provide a ->get_sgtable
implementation.  Note that this isn't an endorsement of this interface
(which is a horrible bad idea), but it is required to move arm64 over
to the generic code without a loss of functionality.

58b0440663 dma-mapping: consolidate the dma mmap implementations
The only functional differences (modulo a few missing fixes in the arch
code) is that architectures without coherent caches need a hook to
convert a virtual or dma address into a pfn, given that we don't have
the kernel linear mapping available for the otherwise easy virt_to_page
call.  As a side effect we can support mmap of the per-device coherent
area even on architectures not providing the callback, and we make
previous dangerous default methods dma_common_mmap actually save for
non-coherent architectures by rejecting it without the right helper.

In addition to that we need a hook so that some architectures can
override the protection bits when mmaping a dma coherent allocations.

d834c5ab83 kernel/dma: remove unsupported gfp_mask parameter from dma_alloc_from_contiguous()
The CMA memory allocator doesn't support standard gfp flags for memory
allocation, so there is no point having it as a parameter for
dma_alloc_from_contiguous() function.  Replace it by a boolean no_warn
argument, which covers all the underlaying cma_alloc() function
supports.

This will help to avoid giving false feeling that this function supports
standard gfp flags and callers can pass __GFP_ZERO to get zeroed buffer,
what has already been an issue: see commit dd65a941f6 ("arm64:
dma-mapping: clear buffers allocated with FORCE_CONTIGUOUS flag").

f07d141fe9 dma-mapping: Generalise dma_32bit_limit flag
Whilst the notion of an upstream DMA restriction is most commonly seen
in PCI host bridges saddled with a 32-bit native interface, a more
general version of the same issue can exist on complex SoCs where a bus
or point-to-point interconnect link from a device's DMA master interface
to another component along the path to memory (often an IOMMU) may carry
fewer address bits than the interfaces at both ends nominally support.
In order to properly deal with this, the first step is to expand the
dma_32bit_limit flag into an arbitrary mask.

To minimise the impact on existing code, we'll make sure to only
consider this new mask valid if set. That makes sense anyway, since a
mask of zero would represent DMA not being wired up at all, and that
would be better handled by not providing valid ops in the first place.

Currently the code is split over various files with dma- prefixes in the
lib/ and drives/base directories, and the number of files keeps growing.
Move them into a single directory to keep the code together and remove
the file name prefixes.  To match the irq infrastructure this directory
is placed under the kernel/ directory.
