Having received another series of whitespace patches I decided to do this
once and for all rather than dealing with this kind of patches trickling
in forever.

Cause is a misplaced bracket.
The code
strlen(buf+1)
will be two bytes less than
strlen(buf)+1
The +1 is in this code to reserve space for an additional space character.
[ralf@linux-mips.org: Thomas' original patch fixed the issue only for
Yosemite but the same bug exists also in Emma.]

Mark interrupts with no_action handler, cascade interrupts, low level
interrupts (bus error, halt ..) with IRQF_NO_THREAD to exclude them
from forced threading.

Although all EMMAxxx SoCs can support IP2 and IP3 hardware interrupts,
current EMMA2RH plat_irq_dispatch() supports IP2 only. We can make it
configurable in the future, but for the time being, would like to make
things explicitly allcated to IP2 in accordance with plat_irq_dispatch().

For historical reasons, we used to put MIPS CPU IRQs behind SoC-specific
IRQs in the queue, and have been using CPU_IRQ_BASE as MIPS_CPU_IRQ_BASE.
In recent years, however, we've brought it back to normal order, and now
CPU_IRQ_BASE just redefines the generic MIPS_CPU_IRQ_BASE.
At the same time, NUM_CPU_IRQ is also removed as useless.

They tend to get not updated when files are moved around or copied and
lack any obvious use. While at it zap some only too obvious comments and
as per Shinya's suggestion, add a copyright header to extable.c.

Fix two remaining set_irq_chip_and_handler() users which are encourated
to migrate to set_irq_chip_and_handler_name().

EMMA's GPIO interrupts are latched by GPIO interrupt status register.
In this case, we're encouraged to use handle_edge_irq() handler.
The following changes are made along with replacing set_irq_chip() with
set_irq_chip_and_handler_name(,,handle_edge_irq,"edge"):
* Fix emma2rh_gpio_irq_ack not to disable interrupts
With handle_edge_irq(), we're not expected to disable interrupts
when chip->ack is served, so fix it accordingly. We also add a new
emma2rh_gpio_irq_mask_ack() for chip->mask_ack operation, instead.
* Remove emma2rh_gpio_irq_end(), as chip->end is no longer served.

It's unused, since about 1995. So remove all initialization of it in
preparation for actually removing the field.

Remove runtime db_* macros as we don't need them any more. In general,
such helpers are useful for initial porting, but once approved, they are
not indispensable.

Let's use immediate value, instead. This also saves memory footprint,
and probably a little bit faster.
