Global pages support is removed from VT-d spec 3.0. Since global pages G
flag only affects first-level paging structures and because DMA request
with PASID are only supported by VT-d spec. 3.0 and onward, we can
safely remove global pages support.
For kernel shared virtual address IOTLB invalidation, PASID
granularity and page selective within PASID will be used. There is
no global granularity supported. Without this fix, IOTLB invalidation
will cause invalid descriptor error in the queued invalidation (QI)
interface.

If multiple devices try to bind to the same mm/PASID, we need to
set up first level PASID entries for all the devices. The current
code does not consider this case which results in failed DMA for
devices after the first bind.

Intel IOMMU could be turned off with intel_iommu=off. If Intel
IOMMU is off, the intel_iommu struct will not be initialized.
When device drivers call intel_svm_bind_mm(), the NULL pointer
reference will happen there.
Add dmar_disabled check to avoid NULL pointer reference.

The change_pte() interface is tailored for PFN updates, while the
other notifier invalidate_range() should be enough for Intel IOMMU
cache flushing. Actually we've done similar thing for AMD IOMMU
already in 8301da53fb ("iommu/amd: Remove change_pte mmu_notifier
call-back", 2014-07-30) but the Intel IOMMU driver still have it.

VT-d Rev3.0 has made a few changes to the page request interface,
1. widened PRQ descriptor from 128 bits to 256 bits;
2. removed streaming response type;
3. introduced private data that requires page response even the
request is not last request in group (LPIG).
This is a supplement to commit 1c4f88b7f1 ("iommu/vt-d: Shared
virtual address in scalable mode") and makes the svm code compliant
with VT-d Rev3.0.

Deferred invalidation is an ECS specific feature. It will not be
supported when IOMMU works in scalable mode. As we deprecated the
ECS support, remove deferred invalidation and cleanup the code.

Intel vt-d spec rev3.0 requires software to use 256-bit
descriptors in invalidation queue. As the spec reads in
section 6.5.2:
Remapping hardware supporting Scalable Mode Translations
(ECAP_REG.SMTS=1) allow software to additionally program
the width of the descriptors (128-bits or 256-bits) that
will be written into the Queue. Software should setup the
Invalidation Queue for 256-bit descriptors before progra-
mming remapping hardware for scalable-mode translation as
128-bit descriptors are treated as invalid descriptors
(see Table 21 in Section 6.5.2.10) in scalable-mode.
This patch adds 256-bit invalidation descriptor support
if the hardware presents scalable mode capability.

Revert 5ff7091f5a ("mm, mmu_notifier: annotate mmu notifiers with
blockable invalidate callbacks").
MMU_INVALIDATE_DOES_NOT_BLOCK flags was the only one used and it is no
longer needed since 93065ac753 ("mm, oom: distinguish blockable mode for
mmu notifiers"). We now have a full support for per range !blocking
behavior so we can drop the stop gap workaround which the per notifier
flag was used for.

Use new return type vm_fault_t for fault handler. For now, this is just
documenting that the function returns a VM_FAULT value rather than an
errno. Once all instances are converted, vm_fault_t will become a
distinct type.

This patch adds the interfaces for per PCI device pasid
table management. Currently we allocate one pasid table
for all PCI devices under the scope of an IOMMU. It's
insecure in some cases where multiple devices under one
single IOMMU unit support PASID features. With per PCI
device pasid table, we can achieve finer protection and
isolation granularity.

idr_for_each_entry() is used to iteratte over idr elements
of a given type. It isn't suitable for the globle pasid idr
since the pasid idr consumer could specify different types
of pointers to bind with a pasid.

If caching mode is supported, the hardware will cache
none-present or erroneous translation entries. Hence,
software should explicitly invalidate the PASID cache
after a PASID table entry becomes present. We should
issue such invalidation with the PASID value that we
have changed. PASID 0 is not reserved for this case.

Commit 4d4bbd8526 ("mm, oom_reaper: skip mm structs with mmu
notifiers") prevented the oom reaper from unmapping private anonymous
memory with the oom reaper when the oom victim mm had mmu notifiers
registered.
The rationale is that doing mmu_notifier_invalidate_range_{start,end}()
around the unmap_page_range(), which is needed, can block and the oom
killer will stall forever waiting for the victim to exit, which may not
be possible without reaping.
That concern is real, but only true for mmu notifiers that have
blockable invalidate_range_{start,end}() callbacks. This patch adds a
"flags" field to mmu notifier ops that can set a bit to indicate that
these callbacks do not block.
The implementation is steered toward an expensive slowpath, such as
after the oom reaper has grabbed mm->mmap_sem of a still alive oom
victim.

supports 5-level paging then enable the 5-level paging mode for first-

checked by the callers. Later when intel_svm_bind_mm() is called,

to pre-cache them in device to avoid overhead of IOMMU lookups.
IOMMU needs to check for canonicallity of the address before performing

A driver would need to know if there are any active references to a
a PASID before cleaning up its resources. This function helps check
if there are any active users of a PASID before it can perform any
recovery on that device.

We are going to split <linux/sched/mm.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/mm.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

We already have the helper, we can convert the rest of the kernel
mechanically using:

Somehow I ended up with an off-by-three error in calculating the size of
the PASID and PASID State tables, which triggers allocations failures as
those tables unfortunately have to be physically contiguous.
In fact, even the *correct* maximum size of 8MiB is problematic and is
wont to lead to allocation failures. Since I have extracted a promise
that this *will* be fixed in hardware, I'm happy to limit it on the
current hardware to a maximum of 0x20000 PASIDs, which gives us 1MiB
tables — still not ideal, but better than before.

only the allocation (and not the free) to the "correct" limit... which

According to the VT-d specification we need to clear the PPR bit in
the Page Request Status register when handling page requests, or the
hardware won't generate any more interrupts.
This wasn't actually necessary on SKL/KBL (which may well be the
subject of a hardware erratum, although it's harmless enough). But
other implementations do appear to get it right, and we only ever get
one interrupt unless we clear the PPR bit.

Holding mm_users works OK for graphics, which was the first user of SVM
with VT-d. However, it works less well for other devices, where we actually
do a mmap() from the file descriptor to which the SVM PASID state is tied.
In this case on process exit we end up with a recursive reference count:
- The MM remains alive until the file is closed and the driver's release()
call ends up unbinding the PASID.
- The VMA corresponding to the mmap() remains intact until the MM is
destroyed.
- Thus the file isn't closed, even when exit_files() runs, because the
VMA is still holding a reference to it. And the MM remains alive…
To address this issue, we *stop* holding mm_users while the PASID is bound.
We already hold mm_count by virtue of the MMU notifier, and that can be
made to be sufficient.
It means that for a period during process exit, the fun part of mmput()
has happened and exit_mmap() has been called so the MM is basically
defunct. But the PGD still exists and the PASID is still bound to it.
During this period, we have to be very careful — exit_mmap() doesn't use
mm->mmap_sem because it doesn't expect anyone else to be touching the MM
(quite reasonably, since mm_users is zero). So we also need to fix the
fault handler to just report failure if mm_users is already zero, and to
temporarily bump mm_users while handling any faults.
Additionally, exit_mmap() calls mmu_notifier_release() *before* it tears
down the page tables, which is too early for us to flush the IOTLB for
this PASID. And __mmu_notifier_release() removes every notifier from the
list, so when exit_mmap() finally *does* tear down the mappings and
clear the page tables, we don't get notified. So we work around this by
clearing the PASID table entry in our MMU notifier release() callback.
That way, the hardware *can't* get any pages back from the page tables
before they get cleared.
Hardware designers have confirmed that the resulting 'PASID not present'
faults should be handled just as gracefully as 'page not present' faults,
the important criterion being that they don't perturb the operation for
any *other* PASID in the system.

Change the 'pages' parameter to 'unsigned long' to avoid overflow.
Fix the device-IOTLB flush parameter calculation — the size of the IOTLB
flush is indicated by the position of the least significant zero bit in
the address field. For example, a value of 0x12345f000 will flush from
0x123440000 to 0x12347ffff (256KiB).
Finally, the cap_pgsel_inv() is not relevant to SVM; the spec says that
*all* implementations must support page-selective invaliation for
"first-level" translations. So don't check for it.
