71a5b3db9f powerpc/lib/code-patching: Set up Strict RWX patching earlier
setup_text_poke_area() is a late init call so it runs before
mark_rodata_ro() and after the init calls. This lets all the init code
patching simply write to their locations. In the future, kprobes is
going to allocate its instruction pages RO which means they will need
setup_text__poke_area() to have been already called for their code
patching. However, init_kprobes() (which allocates and patches some
instruction pages) is an early init call so it happens before
setup_text__poke_area().

start_kernel() calls poking_init() before any of the init calls. On
powerpc, poking_init() is currently a nop. setup_text_poke_area() relies
on kernel virtual memory, cpu hotplug and per_cpu_areas being setup.
setup_per_cpu_areas(), boot_cpu_hotplug_init() and mm_init() are called
before poking_init().

Turn setup_text_poke_area() into poking_init().

69d4d6e5fd powerpc: Don't use 'struct ppc_inst' to reference instruction location
'struct ppc_inst' is an internal representation of an instruction, but
in-memory instructions are and will remain a table of 'u32' forever.

Replace all 'struct ppc_inst *' used for locating an instruction in
memory by 'u32 *'. This removes a lot of undue casts to 'struct
ppc_inst *'.

It also helps locating ab-use of 'struct ppc_inst' dereference.

e90a21ea80 powerpc/lib/code-patching: Don't use struct 'ppc_inst' for runnable code in tests.
'struct ppc_inst' is meant to represent an instruction internally, it
is not meant to dereference code in memory.

For testing code patching, use patch_instruction() to properly
write into memory the code to be tested.

18c85964b1 powerpc: Do not dereference code as 'struct ppc_inst' (uprobe, code-patching, feature-fixups)
'struct ppc_inst' is an internal structure to represent an instruction,
it is not directly the representation of that instruction in text code.
It is not meant to map and dereference code.

Dereferencing code directly through 'struct ppc_inst' has two main issues:
- On powerpc, structs are expected to be 8 bytes aligned while code is
spread every 4 byte.
- Should a non prefixed instruction lie at the end of the page and the
following page not be mapped, it would generate a page fault.

In-memory code must be accessed with ppc_inst_read().

In order to simplify use on PPC32, change ppc_inst_as_u64()
into ppc_inst_as_ulong() that returns the 32 bits instruction
on PPC32.

Will be used when porting OPTPROBES to PPC32.

e31cf2f4ca mm: don't include asm/pgtable.h if linux/mm.h is already included
Patch series "mm: consolidate definitions of page table accessors", v2.

The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once.  For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.

Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.

For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.

These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.

This patch (of 12):

The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g.  pte_alloc() and
pmd_alloc().  So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.

The include statements in such cases are remove with a simple loop:

16ef9767e4 powerpc: Add ppc_inst_as_u64()
The code patching code wants to get the value of a struct ppc_inst as
a u64 when the instruction is prefixed, so we can pass the u64 down to
__put_user_asm() and write it with a single store.

The optprobes code wants to load a struct ppc_inst as an immediate
into a register so it is useful to have it as a u64 to use the
existing helper function.

Currently this is a bit awkward because the value differs based on the
CPU endianness, so add a helper to do the conversion.

This fixes the usage in arch_prepare_optimized_kprobe() which was
previously incorrect on big endian.

instructions can be represented. On powerpc32 the type will remain the

0b582db549 powerpc: Make test_translate_branch() independent of instruction length
test_translate_branch() uses two pointers to instructions within a
buffer, p and q, to test patch_branch(). The pointer arithmetic done on
them assumes a size of 4. This will not work if the instruction length
changes. Instead do the arithmetic relative to the void * to the buffer.

f8faaffaa7 powerpc: Use a function for reading instructions
Prefixed instructions will mean there are instructions of different
length. As a result dereferencing a pointer to an instruction will not
necessarily give the desired result. Introduce a function for reading
instructions from memory into the instruction data type.

94afd069d9 powerpc: Use a datatype for instructions
Currently unsigned ints are used to represent instructions on powerpc.
This has worked well as instructions have always been 4 byte words.

However, ISA v3.1 introduces some changes to instructions that mean
this scheme will no longer work as well. This change is Prefixed
Instructions. A prefixed instruction is made up of a word prefix
followed by a word suffix to make an 8 byte double word instruction.
No matter the endianness of the system the prefix always comes first.
Prefixed instructions are only planned for powerpc64.

Introduce a ppc_inst type to represent both prefixed and word
instructions on powerpc64 while keeping it possible to exclusively
have word instructions on powerpc32.

7534625128 powerpc: Use a macro for creating instructions from u32s
In preparation for instructions having a more complex data type start
using a macro, ppc_inst(), for making an instruction out of a u32.  A
macro is used so that instructions can be used as initializer elements.
Currently this does nothing, but it will allow for creating a data type
that can represent prefixed instructions.

7c95d8893f powerpc: Change calling convention for create_branch() et. al.
create_branch(), create_cond_branch() and translate_branch() return the
instruction that they create, or return 0 to signal an error. Separate
these concerns in preparation for an instruction type that is not just
an unsigned int.  Fill the created instruction to a pointer passed as
the first parameter to the function and use a non-zero return value to
signify an error.

ef296729b7 powerpc/lib: Refactor __patch_instruction() to use __put_user_asm()
__patch_instruction() is called in early boot, and uses
__put_user_size(), which includes the allow/prevent calls to enforce
KUAP, which could either be called too early, or in the Radix case,
forced to use "early_" versions of functions just to safely handle
this one case.

__put_user_asm() does not do this, and thus is safe to use both in
early boot, and later on since in this case it should only ever be
touching kernel memory.

__patch_instruction() was previously refactored to use
__put_user_size() in order to be able to return -EFAULT, which would
allow the kernel to patch instructions in userspace, which should
never happen. This has the functional change of causing faults on
userspace addresses if KUAP is turned on, which should never happen in
practice.

A future enhancement could be to double check the patch address is
definitely allowed to be tampered with by the kernel.

b45ba4a51c powerpc/lib: fix book3s/32 boot failure due to code patching
Commit 51c3c62b58b3 ("powerpc: Avoid code patching freed init
sections") accesses 'init_mem_is_free' flag too early, before the
kernel is relocated. This provokes early boot failure (before the
console is active).

As it is not necessary to do this verification that early, this
patch moves the test into patch_instruction() instead of
__patch_instruction().

This modification also has the advantage of avoiding unnecessary
remappings.

51c3c62b58 powerpc: Avoid code patching freed init sections
This stops us from doing code patching in init sections after they've
been freed.

In this chain:
  kvm_guest_init() ->
    kvm_use_magic_page() ->
      fault_in_pages_readable() ->
	 __get_user() ->
	   __get_user_nocheck() ->
	     barrier_nospec();

We have a code patching location at barrier_nospec() and
kvm_guest_init() is an init function. This whole chain gets inlined,
so when we free the init section (hence kvm_guest_init()), this code
goes away and hence should no longer be patched.

We seen this as userspace memory corruption when using a memory
checker while doing partition migration testing on powervm (this
starts the code patching post migration via
/sys/kernel/mobility/migration). In theory, it could also happen when
using /sys/kernel/debug/powerpc/barrier_nospec.

06d0bbc6d0 powerpc/asm: Add a patch_site macro & helpers for patching instructions
Add a macro and some helper C functions for patching single asm
instructions.

The gas macro means we can do something like:

  1:	nop
  	patch_site 1b, patch__foo

Which is less visually distracting than defining a GLOBAL symbol at 1,
and also doesn't pollute the symbol table which can confuse eg. perf.

These are obviously similar to our existing feature sections, but are
not automatically patched based on CPU/MMU features, rather they are
designed to be manually patched by C code at some arbitrary point.

feature fixups need to use patch_instruction() early in the boot,
even before the code is relocated to its final address, requiring
patch_instruction() to use PTRRELOC() in order to address data.

But feature fixups applies on code before it is set to read only,
even for modules. Therefore, feature fixups can use
raw_patch_instruction() instead.

37bc3e5fd7 powerpc/lib/code-patching: Use alternate map for patch_instruction()
This patch creates the window using text_poke_area, allocated via
get_vm_area(). text_poke_area is per CPU to avoid locking.
text_poke_area for each cpu is setup using late_initcall, prior to
setup of these alternate mapping areas, we continue to use direct
write to change/modify kernel text. With the ability to use alternate
mappings to write to kernel text, it provides us the freedom to then
turn text read-only and implement CONFIG_STRICT_KERNEL_RWX.

This code is CPU hotplug aware to ensure that the we have mappings for
any new cpus as they come online and tear down mappings for any CPUs
that go offline.

71f6e58e5e powerpc/kprobes: Convert __kprobes to NOKPROBE_SYMBOL()
Along similar lines as commit 9326638cbee2 ("kprobes, x86: Use NOKPROBE_SYMBOL()
instead of __kprobes annotation"), convert __kprobes annotation to either
NOKPROBE_SYMBOL() or nokprobe_inline. The latter forces inlining, in which case
the caller needs to be added to NOKPROBE_SYMBOL().

Also:
 - blacklist arch_deref_entry_point(), and
 - convert a few regular inlines to nokprobe_inline in lib/sstep.c

A key benefit is the ability to detect such symbols as being
blacklisted. Before this patch:

  $ cat /sys/kernel/debug/kprobes/blacklist | grep read_mem
  $ perf probe read_mem
  Failed to write event: Invalid argument
    Error: Failed to add events.
  $ dmesg | tail -1
  [ 3736.112815] Could not insert probe at _text+10014968: -22

After patch:
  $ cat /sys/kernel/debug/kprobes/blacklist | grep read_mem

7d134b2ce6 kprobes: move kprobe declarations to asm-generic/kprobes.h
Often all is needed is these small helpers, instead of compiler.h or a
full kprobes.h.  This is important for asm helpers, in fact even some
asm/kprobes.h make use of these helpers...  instead just keep a generic
asm file with helpers useful for asm code with the least amount of
clutter as possible.

Likewise we need now to also address what to do about this file for both
when architectures have CONFIG_HAVE_KPROBES, and when they do not.  Then
for when architectures have CONFIG_HAVE_KPROBES but have disabled
CONFIG_KPROBES.

Right now most asm/kprobes.h do not have guards against CONFIG_KPROBES,
this means most architecture code cannot include asm/kprobes.h safely.
Correct this and add guards for architectures missing them.
Additionally provide architectures that not have kprobes support with
the default asm-generic solution.  This lets us force asm/kprobes.h on
the header include/linux/kprobes.h always, but most importantly we can
now safely include just asm/kprobes.h on architecture code without
bringing the full kitchen sink of header files.

Two architectures already provided a guard against CONFIG_KPROBES on its
kprobes.h: sh, arch.  The rest of the architectures needed gaurds added.
We avoid including any not-needed headers on asm/kprobes.h unless
kprobes have been enabled.

In a subsequent atomic change we can try now to remove compiler.h from
include/linux/kprobes.h.

During this sweep I've also identified a few architectures defining a
common macro needed for both kprobes and ftrace, that of the definition
of the breakput instruction up.  Some refer to this as
BREAKPOINT_INSTRUCTION.  This must be kept outside of the #ifdef
CONFIG_KPROBES guard.

51c9c08439 powerpc/kprobes: Implement Optprobes
Current infrastructure of kprobe uses the unconditional trap instruction
to probe a running kernel. Optprobe allows kprobe to replace the trap
with a branch instruction to a detour buffer. Detour buffer contains
instructions to create an in memory pt_regs. Detour buffer also has a
call to optimized_callback() which in turn call the pre_handler(). After
the execution of the pre-handler, a call is made for instruction
emulation. The NIP is determined in advanced through dummy instruction
emulation and a branch instruction is created to the NIP at the end of
the trampoline.

To address the limitation of branch instruction in POWER architecture,
detour buffer slot is allocated from a reserved area. For the time
being, 64KB is reserved in memory for this purpose.

Instructions which can be emulated using analyse_instr() are the
candidates for optimization. Before optimization ensure that the address
range between the detour buffer allocated and the instruction being
probed is within +/- 32MB.

ebfa50df43 powerpc: Add helper to check if offset is within relative branch range
To permit the use of relative branch instruction in powerpc, the target
address has to be relatively nearby, since the address is specified in an
immediate field (24 bit filed) in the instruction opcode itself. Here
nearby refers to 32MB on either side of the current instruction.

This patch verifies whether the target address is within +/- 32MB
range or not.

636802ef96 powerpc: Don't use __put_user() in patch_instruction
patch_instruction() can be called very early on ppc32, when the kernel
isn't yet running at it's linked address. That can cause the !
is_kernel_addr() test in __put_user() to trip and call might_sleep()
which is very bad at that point during boot.

Use a lower level function instead for now, at least until we get to
rework ppc32 boot process to do the code patching later, like ppc64
does.

For ftrace to use the patch_instruction code, it needs to check for
faults on write. Ftrace updates code all over the kernel, and we need to
know if code is updated or not due to protections that are placed on
some portions of the kernel. If ftrace does not detect a fault, it will
error later on, and it will be much more difficult to find the problem.

By changing patch_instruction() to detect faults, then ftrace will be
able to make use of it too.

27ac792ca0 PAGE_ALIGN(): correctly handle 64-bit values on 32-bit architectures
On 32-bit architectures PAGE_ALIGN() truncates 64-bit values to the 32-bit
boundary. For example:

	u64 val = PAGE_ALIGN(size);

always returns a value < 4GB even if size is greater than 4GB.

The problem resides in PAGE_MASK definition (from include/asm-x86/page.h for
example):

#define PAGE_SHIFT      12
#define PAGE_SIZE       (_AC(1,UL) << PAGE_SHIFT)
#define PAGE_MASK       (~(PAGE_SIZE-1))
...
#define PAGE_ALIGN(addr)       (((addr)+PAGE_SIZE-1)&PAGE_MASK)

The "~" is performed on a 32-bit value, so everything in "and" with
PAGE_MASK greater than 4GB will be truncated to the 32-bit boundary.
Using the ALIGN() macro seems to be the right way, because it uses
typeof(addr) for the mask.

Also move the PAGE_ALIGN() definitions out of include/asm-*/page.h in
include/linux/mm.h.

ae0dc73625 powerpc: Add tests of the code patching routines
Add tests of the existing code patching routines, as well as the new
routines added in the last commit.  The self-tests are run late in boot
when CONFIG_CODE_PATCHING_SELFTEST=y, which depends on DEBUG_KERNEL=y.

053a858efa powerpc: Make create_branch() return errors if the branch target is too large
If you pass a target value to create_branch() which is more than 32MB - 4,
or - 32MB away from the branch site, then it's impossible to create an
immediate branch.  The current code doesn't check, which will lead to us
creating a branch to somewhere else - which is bad.

For code that cares to check we return 0, which is easy to check for, and
for code that doesn't at least we'll be creating an illegal instruction,
rather than a branch to some random address.

e7a57273c6 powerpc: Allow create_branch() to return errors
Currently create_branch() creates a branch instruction for you, and
patches it into the call site.  In some circumstances it would be nice
to be able to create the instruction and patch it later, and also some
code might want to check for errors in the branch creation before
doing the patching.  A future commit will change create_branch() to
check for errors.

For callers that don't care, replace create_branch() with
patch_branch(), which just creates the branch and patches it directly.

While we're touching all the callers, change to using unsigned int *,
as this seems to match usage better.  That allows (and requires) us to
remove the volatile in the definition of vector in powermac/smp.c and
mpc86xx_smp.c, that's correct because now that we're passing vector as
an unsigned int * the compiler knows that it's value might change
across the patch_branch() call.

aaddd3eaca powerpc: Move code patching code into arch/powerpc/lib/code-patching.c
We currently have a few routines for patching code in asm/system.h, because
they didn't fit anywhere else. I'd like to clean them up a little and add
some more, so first move them into a dedicated C file - they don't need to
be inlined.

While we're moving the code, drop create_function_call(), it's intended
caller never got merged and will be replaced in future with something
different.
