Hypercalls can modify arbitrary regions of memory. Make sure to indicate this
in the clobber list. This fixes a hang when using KVM_GUEST kernel built with
GCC 4.3.0.
This was originally spotted and analyzed by Marcelo.

The kvm_* structs are obsoleted by the pvclock_* ones.
Now all users have been switched over and the old structs
can be dropped.

Hypercall based pte updates are faster than faults, and also allow use
of the lazy MMU mode to batch operations.
Don't report the feature if two dimensional paging is enabled.
[avi:
- one mmu_op hypercall instead of one per op
- allow 64-bit gpa on hypercall
- don't pass host errors (-ENOMEM) to guest]

This is the host part of kvm clocksource implementation. As it does
not include clockevents, it is a fairly simple implementation. We
only have to register a per-vcpu area, and start writing to it periodically.
The area is binary compatible with xen, as we use the same shadow_info
structure.

Currently kvm provides hypercalls only for x86* architectures. To
provide hypercall infrastructure for other kvm architectures I split
kvm_para.h into a generic header file and architecture specific
definitions.
