Commit 9fb6c9b3fea1 ("s390/sthyi: add cache to store hypervisor info")
added cache handling for store hypervisor info. This also changed the
possible return code for sthyi_fill().
Instead of only returning a condition code like the sthyi instruction would
do, it can now also return a negative error value (-ENOMEM). handle_styhi()
was not changed accordingly. In case of an error, the negative error value
would incorrectly injected into the guest PSW.
Add proper error handling to prevent this, and update the comment which
describes the possible return values of sthyi_fill().

vmalloc() does not guarantee any alignment, unless it is explicitly
requested with e.g. __vmalloc_node(). Using diag204() with subcode 7
requires a 4k aligned virtual buffer. Therefore switch to __vmalloc_node().
Note: with the current vmalloc() implementation callers would still get a
4k aligned area, even though this is quite non-obvious looking at the
code. So changing this in sthyi doesn't fix a real bug. It is just to make
sure the code will not suffer from some obscure options, like it happened
in the past with kmalloc() where debug options changed the assumed
alignment of allocated memory areas.

When running as a level 3 guest with no host provided sthyi support
sclp_ocf_cpc_name_copy() will only return zeroes. Zeroes are not a
valid group name, so let's not indicate that the group name field is
valid.
Also the group name is not dependent on stsi, let's not return based
on stsi before setting it.

Now that the SPDX tag is in all arch/s390/kernel/ files, that identifies
the license in a specific and legally-defined manner. So the extra GPL
text wording can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the arch/s390/kernel/sthyi file with the correct SPDX license
identifier based on the license text in the file itself. The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

Add a syscall of s390_sthyi to implement STHYI instruction in LPAR
which reuses the implementation for KVM by Janosch Frank -
commit 95ca2cb57985 ("KVM: s390: Add sthyi emulation").
STHYI(Store Hypervisor Information) is an emulated z/VM instruction that
provides a guest with basic information about the layers it is running
on. This includes information about the cpu configuration of both the
machine and the lpar, as well as their names, machine model and
machine type. This information enables an application to determine the
maximum capacity of CPs and IFLs available to software.
For the arguments of s390_sthyi, code shall be 0 and flags is reserved for
future use, info is the output argument to store the required hypervisor
info.

STHYI requires extensive locking in the higher hypervisors and is
very computational/memory expensive. Therefore we cache the retrieved
hypervisor info whose valid period is 1s with mutex to allow concurrent
access. rw semaphore can't benefit here due to cache line bounce.
