Use control path to send EAPOL frames to make sure they are
sent with higher priority with aggregation disabled.

This patch streamlines firmware state tracking. In particular, state
QTNF_FW_STATE_FW_DNLD_DONE is removed, states QTNF_FW_STATE_RESET and
QTNF_FW_STATE_DETACHED are merged into a single state. Besides, new
state QTNF_FW_STATE_RUNNING is introduced to distinguish between
the following two cases:
- firmware load succeeded, firmware init process is ongoing
- firmware init succeeded, firmware is fully functional

Waiting for "completion" to be set in FW load thread can not be used
in case PCIe remove is called before FW load work was scheduled.
Just wait for work completion instead to avoid problems.

Control path will not be operational after firmware failure. Change bus
state to QTNF_FW_STATE_EP_DEAD after the control path timeout.
Don't wait for timeout if control path is already dead.

This patch adds support for new FullMAC WiFi driver for Quantenna
QSR10G chipsets.
QSR10G (aka Pearl) is Quantenna's 8x8, 160M, 11ac offering.
QSR10G supports 2 simultaneous WMACs - one 5G and one 2G.
5G WMAC supports 160M, 8x8 configuration. FW supports
up to 8 concurrent virtual interfaces on each WMAC.
Patch introduces 2 new drivers:
- qtnfmac.ko for interfacing with kernel wireless core
- qtnfmac_pearl_pcie.ko for interfacing with hardware over PCIe interface
