Use vb2_video_unregister_device() to automatically stop streaming
at unregister time.
This avoids the use of vb2_queue_release() which should not be
called by drivers that set vdev->queue.

'GRABBER' is a weird name, all other types map to the /dev
device names. Rename to 'VIDEO' to be consistent with the
other types.

This patch fixes a coccinelle warning and includes ktime.h instead of
timekeeping.h. The first includes the latter, but the latter doesn't
exist before 3.17, causing problems for our compat build. It's easier
to just use ktime.h instead.

This adds raw (unprocessed) BT.656 stream capturing support for the analog
part of Medion 95700.
It can be enabled by setting CXUSB_EXTENDEDMODE_CAPTURE_RAW flag in
parm.capture.extendedmode passed to VIDIOC_S_PARM.

This patch adds support for analog part of Medion 95700 in the cxusb
driver.
What works:
* Video capture at various sizes with sequential fields,
* Input switching (TV Tuner, Composite, S-Video),
* TV and radio tuning,
* Video standard switching and auto detection,
* Radio mode switching (stereo / mono),
* Unplugging while capturing,
* DVB / analog coexistence.
What does not work yet:
* Audio,
* VBI,
* Picture controls.
