2d53492620 irqdomain changes for Linux v3.6
Round of refactoring and enhancements to irq_domain infrastructure. This
 series starts the process of simplifying irqdomain. The ultimate goal is
 to merge LEGACY, LINEAR and TREE mappings into a single system, but had
 to back off from that after some last minute bugs. Instead it mainly
 reorganizes the code and ensures that the reverse map gets populated
 when the irq is mapped instead of the first time it is looked up.
 
 Merging of the irq_domain types is deferred to v3.7

1fcfd08bd0 x86: OLPC: move s/r-related EC cmds to EC driver
The new EC driver calls platform-specific suspend and resume hooks; run
XO-1-specific EC commands from there, rather than deep in s/r code.  If we
attempt to run EC commands after the new EC driver has suspended, it is
refused by the ec->suspended checks.

6cca83d498 Platform: OLPC: move debugfs support from x86 EC driver
There's nothing about the debugfs interface for the EC driver that is
architecture-specific, so move it into the arch-independent driver.

The code is mostly unchanged with the exception of renamed variables, coding
style changes, and API updates.

This uses the new EC driver framework in drivers/platform/olpc.  The
XO-1 and XO-1.5-specific code is still in arch/x86, but the generic stuff

392a325c43 Platform: OLPC: add a stub to drivers/platform/ for the OLPC EC driver
The OLPC EC driver has outgrown arch/x86/platform/.  It's time to both
share common code amongst different architectures, as well as move it out
of arch/x86/.  The XO-1.75 is ARM-based, and the EC driver shares a lot of
code with the x86 code.

e9d90d472d KVM: Remove internal timer abstraction
kvm_timer_fn(), the sole inhabitant of timer.c, is only used by lapic.c. Move
it there to make it easier to hack on it.

struct kvm_timer is a thin wrapper around hrtimer, and only adds obfuscation.
Move near its two users (with different names) to prepare for simplification.

vm_stat_account() accounts the shared_vm, stack_vm and reserved_vm now.
But we can also account for total_vm in the vm_stat_account() which makes
the code tidy.

Even for mprotect_fixup(), we can get the right result in the end.

d07bdfd322 perf/x86: Fix USER/KERNEL tagging of samples properly
Some PMUs don't provide a full register set for their sample,
specifically 'advanced' PMUs like AMD IBS and Intel PEBS which provide
'better' than regular interrupt accuracy.

In this case we use the interrupt regs as basis and over-write some
fields (typically IP) with different information.

The perf core however uses user_mode() to distinguish user/kernel
samples, user_mode() relies on regs->cs. If the interrupt skid pushed
us over a boundary the new IP might not be in the same domain as the
interrupt.

Commit ce5c1fe9a9e ("perf/x86: Fix USER/KERNEL tagging of samples")
tried to fix this by making the perf core use kernel_ip(). This
however is wrong (TM), as pointed out by Linus, since it doesn't allow
for VM86 and non-zero based segments in IA32 mode.

Therefore, provide a new helper to set the regs->ip field,
set_linear_ip(), which massages the regs into a suitable state
assuming the provided IP is in fact a linear address.

Also modify perf_instruction_pointer() and perf_callchain_user() to
deal with segments base offsets.

removed the private DMA API implementation from the OMAP mmc host to exclusively use the DMA engine API.

Unfortunately OMAP MMC and High Speed MMC host drivers don't support poll mode and only works with DMA.

Since omap2plus_defconfig doesn't enable this feature by default, the
following error is happens on an IGEPv2 Rev.C (and probably on most OMAP boards with MMC support):

b74253f784 ARM: 7479/1: mm: avoid NULL dereference when flushing gate_vma with VIVT caches
The vivt_flush_cache_{range,page} functions check that the mm_struct
of the VMA being flushed has been active on the current CPU before
performing the cache maintenance.

The gate_vma has a NULL mm_struct pointer and, as such, will cause a
kernel fault if we try to flush it with the above operations. This
happens during ELF core dumps, which include the gate_vma as it may be
useful for debugging purposes.

This patch adds checks to the VIVT cache flushing functions so that VMAs
with a NULL mm_struct are flushed unconditionally (the vectors page may
be dirty if we use it to store the current TLS pointer).

15ac49b650 ARM: Fix undefined instruction exception handling
While trying to get a v3.5 kernel booted on the cubox, I noticed that
VFP does not work correctly with VFP bounce handling.  This is because
of the confusion over 16-bit vs 32-bit instructions, and where PC is
supposed to point to.

The rule is that FP handlers are entered with regs->ARM_pc pointing at
the _next_ instruction to be executed.  However, if the exception is
not handled, regs->ARM_pc points at the faulting instruction.

This is easy for ARM mode, because we know that the next instruction and
previous instructions are separated by four bytes.  This is not true of
Thumb2 though.

Since all FP instructions are 32-bit in Thumb2, it makes things easy.
We just need to select the appropriate adjustment.  Do this by moving
the adjustment out of do_undefinstr() into the assembly code, as only
the assembly code knows whether it's dealing with a 32-bit or 16-bit
instruction.

24b35521b8 ARM: 7477/1: vfp: Always save VFP state in vfp_pm_suspend on UP
vfp_pm_suspend should save the VFP state in suspend after
any lazy context switch.  If it only saves when the VFP is enabled,
the state can get lost when, on a UP system:
  Thread 1 uses the VFP
  Context switch occurs to thread 2, VFP is disabled but the
     VFP context is not saved
  Thread 2 initiates suspend
  vfp_pm_suspend is called with the VFP disabled, and the unsaved
     VFP context of Thread 1 in the registers

Modify vfp_pm_suspend to save the VFP context whenever
vfp_current_hw_state is not NULL.

Includes a fix from Ido Yariv <ido@wizery.com>, who pointed out that on
SMP systems, the state pointer can be pointing to a freed task struct if
a task exited on another cpu, fixed by using #ifndef CONFIG_SMP in the
new if clause.

a76d7bd96d ARM: 7467/1: mutex: use generic xchg-based implementation for ARMv6+
The open-coded mutex implementation for ARMv6+ cores suffers from a
severe lack of barriers, so in the uncontended case we don't actually
protect any accesses performed during the critical section.

Furthermore, the code is largely a duplication of the ARMv6+ atomic_dec
code but optimised to remove a branch instruction, as the mutex fastpath
was previously inlined. Now that this is executed out-of-line, we can
reuse the atomic access code for the locking (in fact, we use the xchg
code as this produces shorter critical sections).

This patch uses the generic xchg based implementation for mutexes on
ARMv6+, which introduces barriers to the lock/unlock operations and also
has the benefit of removing a fair amount of inline assembly code.

The CPU will endlessly spin at the end of machine_halt and
machine_restart calls.  However, this will lead to a soft lockup
warning after about 20 seconds, if CONFIG_LOCKUP_DETECTOR is enabled,
as system timer is still alive.

Disable interrupt before going to spin endlessly, so that the lockup
warning will never be seen.

079a96ae38 ipc: add COMPAT_SHMLBA support
If the SHMLBA definition for a native task differs from the definition for
a compat task, the do_shmat() function would need to handle both.

This patch introduces COMPAT_SHMLBA, which is used by the compat shmat
syscall when calling the ipc code and allows architectures such as AArch64
(where the native SHMLBA is 64k but the compat (AArch32) definition is
16k) to provide the correct semantics for compat IPC system calls.

4ed940d4c3 firmware_map: make firmware_map_add_early() argument consistent with firmware_map_add_hotplug()
There are two ways to create /sys/firmware/memmap/X sysfs:

  - firmware_map_add_early
    When the system starts, it is calledd from e820_reserve_resources()
  - firmware_map_add_hotplug
    When the memory is hot plugged, it is called from add_memory()

But these functions are called without unifying value of end argument as
below:

  - end argument of firmware_map_add_early()   : start + size - 1
  - end argument of firmware_map_add_hogplug() : start + size

The patch unifies them to "start + size".  Even if applying the patch,
/sys/firmware/memmap/X/end file content does not change.

7eafef6d1d alpha: remove mysterious if zero-ed out includes
There's a small group of odd looking includes in smc37c669.c.  These
includes appear to be if zero-ed out ever since they were added to the
tree (in v2.1.89).  Their purpose is unclear to me.  Perhaps they were
used in someones build system.  Whatever their purpose was, nothing else
uses something comparable.  This entire if zero-ed out block might as well
be removed.

f107701f01 xtensa/mm/fault.c: port OOM changes to do_page_fault
Commits d065bd810b6d ("mm: retry page fault when blocking on disk
transfer") and 37b23e0525d3 ("x86,mm: make pagefault killable")
introduced changes into the x86 pagefault handler for making the page
fault handler retryable as well as killable.

These changes reduce the mmap_sem hold time, which is crucial during OOM
killer invocation.

Port these changes to xtensa.

4a4541a40e KVM: Don't update PPR on any APIC read
The current code will update the PPR on almost any APIC read; however
that's only required if we read the PPR.

kvm_update_ppr() shows up in some profiles, albeit with a low usage (~1%).
This should reduce it further (it will still be called during interrupt
processing).
