afe1bb73f8 ocfs2: unlock rw lock if inode lock failed
In ocfs2_file_aio_write(), it does ocfs2_rw_lock() first and then
ocfs2_inode_lock().

But if ocfs2_inode_lock() failed, it goes to out_sems without unlocking
rw lock.  This will cause a bug in ocfs2_lock_res_free() when testing
res->l_ex_holders, which is increased in __ocfs2_cluster_lock() and
decreased in __ocfs2_cluster_unlock().

7ae077802c xfs: remote attribute lookups require the value length
When reading a remote attribute, to correctly calculate the length
of the data buffer for CRC enable filesystems, we need to know the
length of the attribute data. We get this information when we look
up the attribute, but we don't store it in the args structure along
with the other remote attr information we get from the lookup. Add
this information to the args structure so we can use it
appropriately.

cf257abf02 xfs: xfs_attr_shortform_allfit() does not handle attr3 format.
xfstests generic/117 fails with:

XFS: Assertion failed: leaf->hdr.info.magic == cpu_to_be16(XFS_ATTR_LEAF_MAGIC)

indicating a function that does not handle the attr3 format
correctly. Fix it.

b17cb364db xfs: fix missing KM_NOFS tags to keep lockdep happy
There are several places where we use KM_SLEEP allocation contexts
and use the fact that they are called from transaction context to
add KM_NOFS where appropriate. Unfortunately, there are several
places where the code makes this assumption but can be called from
outside transaction context but with filesystem locks held. These
places need explicit KM_NOFS annotations to avoid lockdep
complaining about reclaim contexts.

7031d0e1c4 xfs: fix rounding in xfs_free_file_space
The offset passed into xfs_free_file_space() needs to be rounded
down to a certain size, but the rounding mask is built by a 32 bit
variable. Hence the mask will always mask off the upper 32 bits of
the offset and lead to incorrect writeback and invalidation ranges.

This is not actually exposed as a bug because we writeback and
invalidate from the rounded offset to the end of the file, and hence
the offset we are actually punching a hole out of will always be
covered by the code. This needs fixing, however, if we ever want to
use exact ranges for writeback/invalidation here...

480d7467e4 xfs: fix sub-page blocksize data integrity writes
FSX on 512 byte block size filesystems has been failing for some
time with corrupted data. The fault dates back to the change in
the writeback data integrity algorithm that uses a mark-and-sweep
approach to avoid data writeback livelocks.

Unfortunately, a side effect of this mark-and-sweep approach is that
each page will only be written once for a data integrity sync, and
there is a condition in writeback in XFS where a page may require
two writeback attempts to be fully written. As a result of the high
level change, we now only get a partial page writeback during the
integrity sync because the first pass through writeback clears the
mark left on the page index to tell writeback that the page needs
writeback....

The cause is writing a partial page in the clustering code. This can
happen when a mapping boundary falls in the middle of a page - we
end up writing back the first part of the page that the mapping
covers, but then never revisit the page to have the remainder mapped
and written.

The fix is simple - if the mapping boundary falls inside a page,
then simple abort clustering without touching the page. This means
that the next ->writepage entry that write_cache_pages() will make
is the page we aborted on, and xfs_vm_writepage() will map all
sections of the page correctly. This behaviour is also optimal for
non-data integrity writes, as it results in contiguous sequential
writeback of the file rather than missing small holes and having to
write them a "random" writes in a future pass.

With this fix, all the fsx tests in xfstests now pass on a 512 byte
block size filesystem on a 4k page machine.

af21ca8ed5 GFS2: Use single-block reservations for directories
This patch changes the multi-block allocation code, such that
directory inodes only get a single block reserved in the bitmap.
That way, the bitmaps are more tightly packed together, and there
are fewer spans of free blocks for in-use block reservations.
This means it takes less time to find a free span of blocks in the
bitmap, which speeds things up. This increases the performance of
some workloads by almost 2X. In Nate's mockup.py script (which does
(1) create dir, (2) create dir in dir, (3) create file in that dir)
the test executes in 23 steps rather than 43 steps, a 47%
performance improvement.

774d5f14ee NFSv4.1 Fix a pNFS session draining deadlock
On a CB_RECALL the callback service thread flushes the inode using
filemap_flush prior to scheduling the state manager thread to return the
delegation. When pNFS is used and I/O has not yet gone to the data server
servicing the inode, a LAYOUTGET can preceed the I/O. Unlike the async
filemap_flush call, the LAYOUTGET must proceed to completion.

If the state manager starts to recover data while the inode flush is sending
the LAYOUTGET, a deadlock occurs as the callback service thread holds the
single callback session slot until the flushing is done which blocks the state
manager thread, and the state manager thread has set the session draining bit
which puts the inode flush LAYOUTGET RPC to sleep on the forechannel slot
table waitq.

Separate the draining of the back channel from the draining of the fore channel
by moving the NFS4_SESSION_DRAINING bit from session scope into the fore
and back slot tables.  Drain the back channel first allowing the LAYOUTGET
call to proceed (and fail) so the callback service thread frees the callback
slot. Then proceed with draining the forechannel.

  Josef has more error handling fixes and an important fix for the new
  skinny extent format.

  This also has my fix for the tracepoint crash from late in 3.9.  It's
  the first stage in a larger clean up to get rid of btrfs_bio and make
  a proper bioset for all the items we need to tack into the bio.  For
  now the bioset only holds our mirror_num and stripe_index, but for the
  next merge window I'll shuffle more in."

9be3395bcd Btrfs: use a btrfs bioset instead of abusing bio internals
Btrfs has been pointer tagging bi_private and using bi_bdev
to store the stripe index and mirror number of failed IOs.

As bios bubble back up through the call chain, we use these
to decide if and how to retry our IOs.  They are also used
to count IO failures on a per device basis.

Recently a bio tracepoint was added lead to crashes because
we were abusing bi_bdev.

This commit adds a btrfs bioset, and creates explicit fields
for the mirror number and stripe index.  The plan is to
extend this structure for all of the fields currently in
struct btrfs_bio, which will mean one less kmalloc in
our IO path.

655b09fe54 Btrfs: make sure roots are assigned before freeing their nodes
If we fail to load the chunk tree we'll call free_root_pointers, except we may
not have assigned the roots for the dev_root/extent_root/csum_root yet, so we
could NULL pointer deref at this point.  Just add checks to make sure these
roots are set to keep us from panicing.  Thanks,

3a6cad9009 Btrfs: explicitly use global_block_rsv for quota_tree
The quota_tree was set up to use the empty_block_rsv before
which would be problematic when the filesystem is filled up
and ENOSPC happens during internal operations while the quota
tree is updated and COWed (when the btrfs_qgroup_info_item
items) are written. In fact, use_block_rsv() which is used
in btrfs_cow_block() falls back to the global_block_rsv in
this case. But just in order to make it more clear what is
happening, change it to explicitly use the global_block_rsv.

17a5adccf3 btrfs: do away with non-whole_page extent I/O
end_bio_extent_readpage computes whole_page based on bv_offset and
bv_len, without taking into account that blk_update_request may modify
them when some of the blocks to be read into a page produce a read
error.  This would cause the read to unlock only part of the file
range associated with the page, which would in turn leave the entire
page locked, which would not only keep the process blocked instead of
returning -EIO to it, but also prevent any further access to the file.

It turns out that btrfs always issues whole-page reads and writes.
The special handling of non-whole_page appears to be a mistake or a
left-over from a time when this wasn't the case.  Indeed,
end_bio_extent_writepage distinguished between whole_page and
non-whole_page writes but behaved identically in both cases!

I've replaced the whole_page computations with warnings, just to be
sure that we're not issuing partial page reads or writes.  The
warnings should probably just go away some time.

inode_tree_del() will move the tree root into the dead root list, and
then the tree will be destroyed by the cleaner. So if we remove the
delayed node which is cached in the inode after inode_tree_del(),
we may access a freed tree root. Fix it.

d88033dbf4 Btrfs: update the global reserve if it is empty
Before applying this patch, we reserved the space for the global reserve
by the minimum unit if we found it is empty, it was unreasonable and
inefficient, because if the global reserve space was depleted, it implied
that the size of the global reserve was too small. In this case, we shoud
update the global reserve and fill it.

It is very likely that there are lots of subvolumes/snapshots in the filesystem,
so if we use global block reservation to do inode cache truncation, we may hog
all the free space that is reserved in global rsv. So it is better that we do
the free space reservation for inode cache truncation by ourselves.

7cfa9e51d2 Btrfs: don't abort the current transaction if there is no enough space for inode cache
The filesystem with inode cache was forced to be read-only when we umounted it.

Steps to reproduce:
 # mkfs.btrfs -f ${DEV}
 # mount -o inode_cache ${DEV} ${MNT}
 # dd if=/dev/zero of=${MNT}/file1 bs=1M count=8192
 # btrfs fi syn ${MNT}
 # dd if=${MNT}/file1 of=/dev/null bs=1M
 # rm -f ${MNT}/file1
 # btrfs fi syn ${MNT}
 # umount ${MNT}

It is because there was no enough space to do inode cache truncation, and then
we aborted the current transaction.

But no space error is not a serious problem when we write out the inode cache,
and it is safe that we just skip this step if we meet this problem. So we need
not abort the current transaction.

8250dabedb Correct allowed raid levels on balance.
Raid5 with 3 devices is well defined while the old logic allowed
raid5 only with a minimum of 4 devices when converting the block group
profile via btrfs balance. Creating a raid5 with just three devices
using mkfs.btrfs worked always as expected. This is now fixed and the
whole logic is rewritten.

b1c79e0947 Btrfs: handle running extent ops with skinny metadata
Chris hit a bug where we weren't finding extent records when running extent ops.
This is because we use the delayed_ref_head when running the extent op, which
means we can't use the ->type checks to see if we are metadata.  We also lose
the level of the metadata we are working on.  So to fix this we can just check
the ->is_data section of the extent_op, and we can store the level of the buffer
we were modifying in the extent_op.  Thanks,

4dfea4f0d7 eCryptfs: Use the ablkcipher crypto API
Make the switch from the blkcipher kernel crypto interface to the
ablkcipher interface.

encrypt_scatterlist() and decrypt_scatterlist() now use the ablkcipher
interface but, from the eCryptfs standpoint, still treat the crypto
operation as a synchronous operation. They submit the async request and
then wait until the operation is finished before they return. Most of
the changes are contained inside those two functions.

Despite waiting for the completion of the crypto operation, the
ablkcipher interface provides performance increases in most cases when
used on AES-NI capable hardware.

The problem appears to be a regression that was introduced in commit
9a9c6478 "nfsd: make NFSv4 recovery client tracking options per net".
Prior to that commit, it was safe to pass a NULL net pointer to
nfsd4_client_tracking_exit in the legacy recdir case, and
legacy_recdir_name_error did so. After that comit, the net pointer must
be valid.

This patch just fixes legacy_recdir_name_error to pass in a valid net
pointer to that function.

c2b93e0699 cifs: only set ops for inodes in I_NEW state
It's generally not safe to reset the inode ops once they've been set. In
the case where the inode was originally thought to be a directory and
then later found to be a DFS referral, this can lead to an oops when we
try to trigger an inode op on it after changing the ops to the blank
referral operations.

c23266d532 NFS4.1 Fix data server connection race
Unlike meta data server mounts which support multiple mount points to
the same server via struct nfs_server, data servers support a single connection.

Concurrent calls to setup the data server connection can race where the first
call allocates the nfs_client struct, and before the cache struct nfs_client
pointer can be set, a second call also tries to setup the connection, finds the
already allocated nfs_client, bumps the reference count, re-initializes the
session,etc. This results in a hanging data server session after umount.

59bbd474ab f2fs: cover free_nid management with spin_lock
After build_free_nids() searches free nid candidates from nat pages and
current journal blocks, it checks all the candidates if they are allocated
so that the nat cache has its nid with an allocated block address.

In this procedure, previously we used
    list_for_each_entry_safe(fnid, next_fnid, &nm_i->free_nid_list, list).
But, this is not covered by free_nid_list_lock, resulting in null pointer bug.

This patch moves this checking routine inside add_free_nid() in order not to use
the spin_lock.

Since there is NOT nmi->free_nid_list_lock spinlock protection between

threads may already add new free_nid to the free_nid_list during this
period.

We need to make sure nmi->fcnt is never > 2 * MAX_FREE_NIDS.

Thanks to Zach Brown's work to rip out the retry infrastructure, we don't
need this anymore - ki_retry was only called right after the kiocb was
initialized.

This also refactors and trims some duplicated code, as well as cleaning up
the refcounting/error handling a bit.

In older kernels, creating a file with open(..., O_CREAT) created

Richard bisected to determine that the problems started with commit
bfcec708, but the log messages have changed with some later
audit-related patches.

The problem is that this audit_inode call is passing in the parent of
the dentry being opened, but audit_inode is being called with the parent
flag false. This causes later audit_inode and audit_inode_child calls to
match the wrong entry in the audit_names list.

This patch simply sets the flag to properly indicate that this inode
represents the parent. With this, the audit_names entries are back to
looking like they did before.

58c85dc20a aio: kill struct aio_ring_info
struct aio_ring_info was kind of odd, the only place it's used is where
it's embedded in struct kioctx - there's no real need for it.

The next patch rearranges struct kioctx and puts various things on their
own cachelines - getting rid of struct aio_ring_info now makes that
reordering a bit clearer.

a1c8eae75e aio: kill batch allocation
Previously, allocating a kiocb required touching quite a few global
(well, per kioctx) cachelines...  so batching up allocation to amortize
those was worthwhile.  But we've gotten rid of some of those, and in
another couple of patches kiocb allocation won't require writing to any
shared cachelines, so that means we can just rip this code out.

3e845ce01a aio: change reqs_active to include unreaped completions
The aio code tries really hard to avoid having to deal with the
completion ringbuffer overflowing.  To do that, it has to keep track of
the number of outstanding kiocbs, and the number of completions
currently in the ringbuffer - and it's got to check that every time we
allocate a kiocb.  Ouch.

But - we can improve this quite a bit if we just change reqs_active to
mean "number of outstanding requests and unreaped completions" - that
means kiocb allocation doesn't have to look at the ringbuffer, which is
a fairly significant win.

0460fef2a9 aio: use cancellation list lazily
Cancelling kiocbs requires adding them to a per kioctx linked list,
which is one of the few things we need to take the kioctx lock for in
the fast path.  But most kiocbs can't be cancelled - so if we just do
this lazily, we can avoid quite a bit of locking overhead.

While we're at it, instead of using a flag bit switch to using ki_cancel
itself to indicate that a kiocb has been cancelled/completed.  This lets
us get rid of ki_flags entirely.

a31ad380be aio: make aio_read_evt() more efficient, convert to hrtimers
Previously, aio_read_event() pulled a single completion off the
ringbuffer at a time, locking and unlocking each time.  Change it to
pull off as many events as it can at a time, and copy them directly to
userspace.

This also fixes a bug where if copying the event to userspace failed,
we'd lose the event.

Also convert it to wait_event_interruptible_hrtimeout(), which
simplifies it quite a bit.

36f5588905 aio: refcounting cleanup
The usage of ctx->dead was fubar - it makes no sense to explicitly check
it all over the place, especially when we're already using RCU.

Now, ctx->dead only indicates whether we've dropped the initial
refcount. The new teardown sequence is:

  set ctx->dead
  hlist_del_rcu();
  synchronize_rcu();

Now we know no system calls can take a new ref, and it's safe to drop
the initial ref:

  put_ioctx();

We also need to ensure there are no more outstanding kiocbs.  This was
done incorrectly - it was being done in kill_ctx(), and before dropping
the initial refcount.  At this point, other syscalls may still be
submitting kiocbs!

Now, we cancel and wait for outstanding kiocbs in free_ioctx(), after
kioctx->users has dropped to 0 and we know no more iocbs could be
submitted.

11599ebac4 aio: make aio_put_req() lockless
Freeing a kiocb needed to touch the kioctx for three things:

 * Pull it off the reqs_active list
 * Decrementing reqs_active
 * Issuing a wakeup, if the kioctx was in the process of being freed.

This patch moves these to aio_complete(), for a couple reasons:

 * aio_complete() already has to issue the wakeup, so if we drop the
   kioctx refcount before aio_complete does its wakeup we don't have to
   do it twice.
 * aio_complete currently has to take the kioctx lock, so it makes sense
   for it to pull the kiocb off the reqs_active list too.
 * A later patch is going to change reqs_active to include unreaped
   completions - this will mean allocating a kiocb doesn't have to look
   at the ringbuffer. So taking the decrement of reqs_active out of
   kiocb_free() is useful prep work for that patch.

This doesn't really affect cancellation, since existing (usb) code that
implements a cancel function still calls aio_complete() - we just have
to make sure that aio_complete does the necessary teardown for cancelled
kiocbs.

It does affect code paths where we free kiocbs that were never
submitted; they need to decrement reqs_active and pull the kiocb off the
reqs_active list.  This occurs in two places: kiocb_batch_free(), which
is going away in a later patch, and the error path in io_submit_one.

41003a7bcf aio: remove retry-based AIO
This removes the retry-based AIO infrastructure now that nothing in tree
is using it.

We want to remove retry-based AIO because it is fundemantally unsafe.
It retries IO submission from a kernel thread that has only assumed the
mm of the submitting task.  All other task_struct references in the IO
submission path will see the kernel thread, not the submitting task.
This design flaw means that nothing of any meaningful complexity can use
retry-based AIO.

This removes all the code and data associated with the retry machinery.
The most significant benefit of this is the removal of the locking
around the unused run list in the submission path.

af73e4d950 hugetlbfs: fix mmap failure in unaligned size request
The current kernel returns -EINVAL unless a given mmap length is
"almost" hugepage aligned.  This is because in sys_mmap_pgoff() the
given length is passed to vm_mmap_pgoff() as it is without being aligned
with hugepage boundary.

This is a regression introduced in commit 40716e2924 ("hugetlbfs: fix
alignment of huge page requests"), where alignment code is pushed into
hugetlb_file_setup() and the variable len in caller side is not changed.

To fix this, this patch partially reverts that commit, and adds
alignment code in caller side.  And it also introduces hstate_sizelog()
in order to get proper hstate to specified hugepage size.

running.

This was done for xfs_attrlist_by_handle but
xfs_compat_attrlist_by_handle (the 32-bit version) needs the same
attention.

742ae1e35b xfs: introduce CONFIG_XFS_WARN
Running a CONFIG_XFS_DEBUG kernel in production environments is not
the best idea as it introduces significant overhead, can change
the behaviour of algorithms (such as allocation) to improve test
coverage, and (most importantly) panic the machine on non-fatal
errors.

There are many cases where all we want to do is run a
kernel with more bounds checking enabled, such as is provided by the
ASSERT() statements throughout the code, but without all the
potential overhead and drawbacks.

This patch converts all the ASSERT statements to evaluate as
WARN_ON(1) statements and hence if they fail dump a warning and a
stack trace to the log. This has minimal overhead and does not
change any algorithms, and will allow us to find strange "out of
bounds" problems more easily on production machines.

There are a few places where assert statements contain debug only
code. These are converted to be debug-or-warn only code so that we
still get all the assert checks in the code.

667e7d94a1 Btrfs: allow superblock mismatch from older mkfs
We've added new checks to make sure the super block crc is correct
during mount.  A fresh filesystem from an older mkfs won't have the
crc set.  This adds a warning when it finds a newly created filesystem
but doesn't fail the mount.

1104a88551 btrfs: enhance superblock checks
The superblock checksum is not verified upon mount. <awkward silence>

Add that check and also reorder existing checks to a more logical
order.

Current mkfs.btrfs does not calculate the correct checksum of
super_block and thus a freshly created filesytem will fail to mount when
this patch is applied.

First transaction commit calculates correct superblock checksum and
saves it to disk.

Reproducer:
$ mfks.btrfs /dev/sda
$ mount /dev/sda /mnt
$ btrfs scrub start /mnt
$ sleep 5
$ btrfs scrub status /mnt
... super:2 ...
