[ NOTE: unfortunately this tree had to be freshly rebased today,
         it's a same-content tree of 82891be90f3c (-next published)
         merged with v5.11.
 
         The main reason for the rebase was an authorship misattribution
         problem with a new commit, which we noticed in the last minute,
         and which we didn't want to be merged upstream. The offending
         commit was deep in the tree, and dependent commits had to be
         rebased as well. ]
 
 - Core scheduler updates:
 
   - Add CONFIG_PREEMPT_DYNAMIC: this in its current form adds the
     preempt=none/voluntary/full boot options (default: full),
     to allow distros to build a PREEMPT kernel but fall back to
     close to PREEMPT_VOLUNTARY (or PREEMPT_NONE) runtime scheduling
     behavior via a boot time selection.
 
     There's also the /debug/sched_debug switch to do this runtime.
 
     This feature is implemented via runtime patching (a new variant of static calls).
 
     The scope of the runtime patching can be best reviewed by looking
     at the sched_dynamic_update() function in kernel/sched/core.c.
 
     ( Note that the dynamic none/voluntary mode isn't 100% identical,
       for example preempt-RCU is available in all cases, plus the
       preempt count is maintained in all models, which has runtime
       overhead even with the code patching. )
 
     The PREEMPT_VOLUNTARY/PREEMPT_NONE models, used by the vast majority
     of distributions, are supposed to be unaffected.
 
   - Fix ignored rescheduling after rcu_eqs_enter(). This is a bug that
     was found via rcutorture triggering a hang. The bug is that
     rcu_idle_enter() may wake up a NOCB kthread, but this happens after
     the last generic need_resched() check. Some cpuidle drivers fix it
     by chance but many others don't.
 
     In true 2020 fashion the original bug fix has grown into a 5-patch
     scheduler/RCU fix series plus another 16 RCU patches to address
     the underlying issue of missed preemption events. These are the
     initial fixes that should fix current incarnations of the bug.
 
   - Clean up rbtree usage in the scheduler, by providing & using the following
     consistent set of rbtree APIs:
 
      partial-order; less() based:
        - rb_add(): add a new entry to the rbtree
        - rb_add_cached(): like rb_add(), but for a rb_root_cached
 
      total-order; cmp() based:
        - rb_find(): find an entry in an rbtree
        - rb_find_add(): find an entry, and add if not found
 
        - rb_find_first(): find the first (leftmost) matching entry
        - rb_next_match(): continue from rb_find_first()
        - rb_for_each(): iterate a sub-tree using the previous two
 
   - Improve the SMP/NUMA load-balancer: scan for an idle sibling in a single pass.
     This is a 4-commit series where each commit improves one aspect of the idle
     sibling scan logic.
 
   - Improve the cpufreq cooling driver by getting the effective CPU utilization
     metrics from the scheduler
 
   - Improve the fair scheduler's active load-balancing logic by reducing the number
     of active LB attempts & lengthen the load-balancing interval. This improves
     stress-ng mmapfork performance.
 
   - Fix CFS's estimated utilization (util_est) calculation bug that can result in
     too high utilization values

     The scope of the runtime patching can be best reviewed by looking
     at the sched_dynamic_update() function in kernel/sched/core.c.

     ( Note that the dynamic none/voluntary mode isn't 100% identical,
       for example preempt-RCU is available in all cases, plus the
       preempt count is maintained in all models, which has runtime
       overhead even with the code patching. )

     The PREEMPT_VOLUNTARY/PREEMPT_NONE models, used by the vast
     majority of distributions, are supposed to be unaffected.

   - Fix ignored rescheduling after rcu_eqs_enter(). This is a bug that
     was found via rcutorture triggering a hang. The bug is that
     rcu_idle_enter() may wake up a NOCB kthread, but this happens after
     the last generic need_resched() check. Some cpuidle drivers fix it
     by chance but many others don't.

     In true 2020 fashion the original bug fix has grown into a 5-patch
     scheduler/RCU fix series plus another 16 RCU patches to address the
     underlying issue of missed preemption events. These are the initial
     fixes that should fix current incarnations of the bug.

   - Clean up rbtree usage in the scheduler, by providing & using the
     following consistent set of rbtree APIs:

       partial-order; less() based:
         - rb_add(): add a new entry to the rbtree
         - rb_add_cached(): like rb_add(), but for a rb_root_cached

       total-order; cmp() based:
         - rb_find(): find an entry in an rbtree
         - rb_find_add(): find an entry, and add if not found

         - rb_find_first(): find the first (leftmost) matching entry
         - rb_next_match(): continue from rb_find_first()
         - rb_for_each(): iterate a sub-tree using the previous two

   - Improve the SMP/NUMA load-balancer: scan for an idle sibling in a
     single pass. This is a 4-commit series where each commit improves
     one aspect of the idle sibling scan logic.

   - Improve the cpufreq cooling driver by getting the effective CPU
     utilization metrics from the scheduler

   - Improve the fair scheduler's active load-balancing logic by
     reducing the number of active LB attempts & lengthen the
     load-balancing interval. This improves stress-ng mmapfork
     performance.

   - Fix CFS's estimated utilization (util_est) calculation bug that can
     result in too high utilization values

  Misc updates & fixes:

   - Fix the HRTICK reprogramming & optimization feature

   - Fix SCHED_SOFTIRQ raising race & warning in the CPU offlining code

   - Reduce dl_add_task_root_domain() overhead

   - Fix uprobes refcount bug

   - Process pending softirqs in flush_smp_call_function_from_idle()

   - Clean up task priority related defines, remove *USER_*PRIO and
     USER_PRIO()

9d061ba6bc sched: Remove USER_PRIO, TASK_USER_PRIO and MAX_USER_PRIO
The only remaining use of MAX_USER_PRIO (and USER_PRIO) is the
SCALE_PRIO() definition in the PowerPC Cell architecture's Synergistic
Processor Unit (SPU) scheduler. TASK_USER_PRIO isn't used anymore.

Commit fe443ef2ac ("[POWERPC] spusched: Dynamic timeslicing for
SCHED_OTHER") copied SCALE_PRIO() from the task scheduler in v2.6.23.

Commit a4ec24b48d ("sched: tidy up SCHED_RR") removed it from the task
scheduler in v2.6.24.

Commit 3ee237dddc ("sched/prio: Add 3 macros of MAX_NICE, MIN_NICE and
NICE_WIDTH in prio.h") introduced NICE_WIDTH much later.

With:

  MAX_USER_PRIO = USER_PRIO(MAX_PRIO)

                = MAX_PRIO - MAX_RT_PRIO

       MAX_PRIO = MAX_RT_PRIO + NICE_WIDTH

  MAX_USER_PRIO = MAX_RT_PRIO + NICE_WIDTH - MAX_RT_PRIO

  MAX_USER_PRIO = NICE_WIDTH

MAX_USER_PRIO can be replaced by NICE_WIDTH to be able to remove all the
{*_}USER_PRIO defines.

ab950e1acd KVM: PPC: Book3S HV: Ensure radix guest has no SLB entries
Commit 68ad28a4cd ("KVM: PPC: Book3S HV: Fix radix guest SLB side
channel") changed the older guest entry path, with the side effect
that vcpu->arch.slb_max no longer gets cleared for a radix guest.
This means that a HPT guest which loads some SLB entries, switches to
radix mode, runs the guest using the old guest entry path (e.g.,
because the indep_threads_mode module parameter has been set to
false), and then switches back to HPT mode would now see the old SLB
entries being present, whereas previously it would have seen no SLB
entries.

To avoid changing guest-visible behaviour, this adds a store
instruction to clear vcpu->arch.slb_max for a radix guest using the
old guest entry path.

25edcc50d7 KVM: PPC: Book3S HV: Save and restore FSCR in the P9 path
The Facility Status and Control Register is a privileged SPR that
defines the availability of some features in problem state. Since it
can be written by the guest, we must restore it to the previous host
value after guest exit.

This restoration is currently done by taking the value from
current->thread.fscr, which in the P9 path is not enough anymore
because the guest could context switch the QEMU thread, causing the
guest-current value to be saved into the thread struct.

The above situation manifested when running a QEMU linked against a
libc with System Call Vectored support, which causes scv
instructions to be run by QEMU early during the guest boot (during
SLOF), at which point the FSCR is 0 due to guest entry. After a few
scv calls (1 to a couple hundred), the context switching happens and
the QEMU thread runs with the guest value, resulting in a Facility
Unavailable interrupt.

This patch saves and restores the host value of FSCR in the inner
guest entry loop in a way independent of current->thread.fscr. The old
way of doing it is still kept in place because it works for the old
entry path.

68ad28a4cd KVM: PPC: Book3S HV: Fix radix guest SLB side channel
The slbmte instruction is legal in radix mode, including radix guest
mode. This means radix guests can load the SLB with arbitrary data.

KVM host does not clear the SLB when exiting a guest if it was a
radix guest, which would allow a rogue radix guest to use the SLB as
a side channel to communicate with other guests.

Fix this by ensuring the SLB is cleared when coming out of a radix
guest. Only the first 4 entries are a concern, because radix guests
always run with LPCR[UPRT]=1, which limits the reach of slbmte. slbia
is not used (except in a non-performance-critical path) because it
can clear cached translations.

b1b1697ae0 KVM: PPC: Book3S HV: Remove support for running HPT guest on RPT host without mixed mode support
This reverts much of commit c01015091a ("KVM: PPC: Book3S HV: Run HPT
guests on POWER9 radix hosts"), which was required to run HPT guests on
RPT hosts on early POWER9 CPUs without support for "mixed mode", which
meant the host could not run with MMU on while guests were running.

This code has some corner case bugs, e.g., when the guest hits a machine
check or HMI the primary locks up waiting for secondaries to switch LPCR
to host, which they never do. This could all be fixed in software, but
most CPUs in production have mixed mode support, and those that don't
are believed to be all in installations that don't use this capability.
So simplify things and remove support.

Also, KVM will support 2nd DAWR only if CPU_FTR_DAWR1 is set.

On powerpc, L1 hypervisor takes help of L0 using H_ENTER_NESTED
hcall to load L2 guest state in cpu. L1 hypervisor prepares the
L2 state in struct hv_guest_state and passes a pointer to it via
hcall. Using that pointer, L0 reads/writes that state directly
from/to L1 memory. Thus L0 must be aware of hv_guest_state layout
of L1. Currently it uses version field to achieve this. i.e. If
L0 hv_guest_state.version != L1 hv_guest_state.version, L0 won't
allow nested kvm guest.

This restriction can be loosened up a bit. L0 can be taught to
understand older layout of hv_guest_state, if we restrict the
new members to be added only at the end, i.e. we can allow
nested guest even when L0 hv_guest_state.version > L1
hv_guest_state.version. Though, the other way around is not
possible.

4fc096a99e KVM: Raise the maximum number of user memslots
Current KVM_USER_MEM_SLOTS limits are arch specific (512 on Power, 509 on x86,
32 on s390, 16 on MIPS) but they don't really need to be. Memory slots are
allocated dynamically in KVM when added so the only real limitation is
'id_to_index' array which is 'short'. We don't have any other
KVM_MEM_SLOTS_NUM/KVM_USER_MEM_SLOTS-sized statically defined structures.

Low KVM_USER_MEM_SLOTS can be a limiting factor for some configurations.
In particular, when QEMU tries to start a Windows guest with Hyper-V SynIC
enabled and e.g. 256 vCPUs the limit is hit as SynIC requires two pages per
vCPU and the guest is free to pick any GFN for each of them, this fragments
memslots as QEMU wants to have a separate memslot for each of these pages
(which are supposed to act as 'overlay' pages).

The kernel consider thread AMR value for kernel thread to be
AMR_KUAP_BLOCKED. Hence access to userspace is denied. This
of course not correct and we should allow userspace access after
kthread_use_mm(). To be precise, kthread_use_mm() should inherit the
AMR value of the operating address space. But, the AMR value is
thread-specific and we inherit the address space and not thread
access restrictions. Because of this ignore AMR value when accessing
userspace via kernel thread.

24321ac668 powerpc/64/signal: Fix regression in __kernel_sigtramp_rt64() semantics
Commit 0138ba5783 ("powerpc/64/signal: Balance return predictor
stack in signal trampoline") changed __kernel_sigtramp_rt64() VDSO and
trampoline code, and introduced a regression in the way glibc's
backtrace()[1] detects the signal-handler stack frame. Apart from the
practical implications, __kernel_sigtramp_rt64() was a VDSO function
with the semantics that it is a function you can call from userspace
to end a signal handling. Now this semantics are no longer valid.

I believe the aforementioned change affects all releases since 5.9.

This patch tries to fix both the semantics and practical aspect of
__kernel_sigtramp_rt64() returning it to the previous code, whilst
keeping the intended behaviour of 0138ba5783 by adding a new symbol
to serve as the jump target from the kernel to the trampoline. Now the
trampoline has two parts, a new entry point and the old return point.

2a6c6b7d7a perf/core: Add PERF_SAMPLE_WEIGHT_STRUCT
Current PERF_SAMPLE_WEIGHT sample type is very useful to expresses the
cost of an action represented by the sample. This allows the profiler
to scale the samples to be more informative to the programmer. It could
also help to locate a hotspot, e.g., when profiling by memory latencies,
the expensive load appear higher up in the histograms. But current
PERF_SAMPLE_WEIGHT sample type is solely determined by one factor. This
could be a problem, if users want two or more factors to contribute to
the weight. For example, Golden Cove core PMU can provide both the
instruction latency and the cache Latency information as factors for the
memory profiling.

For current X86 platforms, although meminfo::latency is defined as a
u64, only the lower 32 bits include the valid data in practice (No
memory access could last than 4G cycles). The higher 32 bits can be used
to store new factors.

Add a new sample type, PERF_SAMPLE_WEIGHT_STRUCT, to indicate the new
sample weight structure. It shares the same space as the
PERF_SAMPLE_WEIGHT sample type.

Users can apply either the PERF_SAMPLE_WEIGHT sample type or the
PERF_SAMPLE_WEIGHT_STRUCT sample type to retrieve the sample weight, but
they cannot apply both sample types simultaneously.

Currently, only X86 and PowerPC use the PERF_SAMPLE_WEIGHT sample type.
- For PowerPC, there is nothing changed for the PERF_SAMPLE_WEIGHT
  sample type. There is no effect for the new PERF_SAMPLE_WEIGHT_STRUCT
  sample type. PowerPC can re-struct the weight field similarly later.
- For X86, the same value will be dumped for the PERF_SAMPLE_WEIGHT
  sample type or the PERF_SAMPLE_WEIGHT_STRUCT sample type for now.
  The following patches will apply the new factors for the
  PERF_SAMPLE_WEIGHT_STRUCT sample type.

The field in the union perf_sample_weight should be shared among
different architectures. A generic name is required, but it's hard to
abstract a name that applies to all architectures. For example, on X86,
the fields are to store all kinds of latency. While on PowerPC, it
stores MMCRA[TECX/TECM], which should not be latency. So a general name
prefix 'var$NUM' is used here.

bce74491c3 powerpc/vdso: fix unnecessary rebuilds of vgettimeofday.o
vgettimeofday.o is unnecessarily rebuilt. Adding it to 'targets' is not
enough to fix the issue. Kbuild is correctly rebuilding it because the
command line is changed.

PowerPC builds each vdso directory twice; first in vdso_prepare to
generate vdso{32,64}-offsets.h, second as part of the ordinary build
process to embed vdso{32,64}.so.dbg into the kernel.

The problem shows up when CONFIG_PPC_WERROR=y due to the following line
in arch/powerpc/Kbuild:

  subdir-ccflags-$(CONFIG_PPC_WERROR) := -Werror

In the preparation stage, Kbuild directly visits the vdso directories,
hence it does not inherit subdir-ccflags-y. In the second descend,
Kbuild adds -Werror, which results in the command line flipping
with/without -Werror.

It implies a potential danger; if a more critical flag that would impact
the resulted vdso, the offsets recorded in the headers might be different
from real offsets in the embedded vdso images.

Removing the unneeded second descend solves the problem.

7a3c90df20 arch: powerpc: Stop building and using oprofile
The "oprofile" user-space tools don't use the kernel OPROFILE support
any more, and haven't in a long time. User-space has been converted to
the perf interfaces.

This commits stops building oprofile for powerpc and removes any
reference to it from directories in arch/powerpc/ apart from
arch/powerpc/oprofile, which will be removed in the next commit (this is
broken into two commits as the size of the commit became very big, ~5k
lines).

Note that the member "oprofile_cpu_type" in "struct cpu_spec" isn't
removed as it was also used by other parts of the code.

that may have been created, and runs them. Running softirqs enables
local irqs, which can replay pending interrupts causing recursion in

This can not be prevented easily, because softirqs must not block hard
irqs, so it has to be dealt with.

The recursion is bounded by design in the softirq code because softirq
replay disables softirqs and loops around again to check for new
softirqs created while it ran, so that's not a problem.

However it does mess up interrupt replay state, causing superfluous
interrupts when the second replay_soft_interrupts clears a pending
interrupt, leaving it still set in the first call in the 'happened'
local variable.

Fix this by not caching a copy of irqs_happened across interrupt
handler calls.

08685be776 powerpc/64s: fix scv entry fallback flush vs interrupt
The L1D flush fallback functions are not recoverable vs interrupts,
yet the scv entry flush runs with MSR[EE]=1. This can result in a
timer (soft-NMI) or MCE or SRESET interrupt hitting here and overwriting
the EXRFI save area, which ends up corrupting userspace registers for
scv return.

Fix this by disabling RI and EE for the scv entry fallback flush.

A subsequent patch will add additional atomic operations. These new
operations will use the same opcode field as the existing XADD, with
the immediate discriminating different operations.

In preparation, rename the instruction mode BPF_ATOMIC and start
calling the zero immediate BPF_ADD.

This is possible (doesn't break existing valid BPF progs) because the
immediate field is currently reserved MBZ and BPF_ADD is zero.

All uses are removed from the tree but the BPF_XADD definition is
kept around to avoid breaking builds for people including kernel
headers.

__kernel_timespec, with 64-bit time_t, so use the clock_gettime64

clock_getres_fallback should use the clock_getres_time64 syscall,
though it isn't yet called from the 32-bit VDSO.

2225a8dda2 powerpc: Fix alignment bug within the init sections
This is a bug that causes early crashes in builds with an .exit.text
section smaller than a page and an .init.text section that ends in the
beginning of a physical page (this is kinda random, which might
explain why this wasn't really encountered before).

The init sections are ordered like this:
  .init.text
  .exit.text
  .init.data

Currently, these sections aren't page aligned.

Because the init code might become read-only at runtime and because
the .init.text section can potentially reside on the same physical
page as .init.data, the beginning of .init.data might be mapped
read-only along with .init.text.

Then when the kernel tries to modify a variable in .init.data (like
kthreadd_done, used in kernel_init()) the kernel panics.

To avoid this, make _einittext page aligned and also align .exit.text
to make sure .init.data is always seperated from the text segments.

41026c3435 Kconfig: regularize selection of CONFIG_BINFMT_ELF
with mips converted to use of fs/config_binfmt_elf.c, there's no
need to keep selects of that thing all over arch/* - we can simply
turn into def_bool y if COMPAT && BINFMT_ELF (in fs/Kconfig.binfmt)
and get rid of all selects.

Several architectures got those selects wrong (e.g. you could
end up with sparc64 sans BINFMT_ELF, with select violating
dependencies, etc.)

Randy Dunlap has spotted some of those; IMO this is simpler than
his fix, but it depends upon the stuff that would need to be
backported, so we might end up using his variant for -stable.
