01abdcc528 phy: qcom-usb-hs: Mark expected switch fall-through 
In preparation to enabling -Wimplicit-fallthrough, mark switch cases

f0b5c2c963 phy: qcom-usb-hs: Replace the extcon API 
This patch uses the resource-managed extcon API for extcon_register_notifier()
and replaces the deprecated extcon API as following:
- (deprecated) extcon_get_cable_state_() -> extcon_get_state()

0b56e9a7e8 phy: Group vendor specific phy drivers 
Adding vendor specific directories in phy to group
phy drivers under their respective vendor umbrella.

Also updated the MAINTAINERS file to reflect the correct
directory structure for phy drivers.
