In order to handle the !CONFIG_TRANSPARENT_HUGEPAGES case, we need to
return VM_FAULT_FALLBACK from the inlined dax_pmd_fault(), which is
defined in linux/mm.h. Given that we don't want to include <linux/mm.h>
in <linux/fs.h>, the easiest solution is to move the DAX-related
functions to a new header, <linux/dax.h>. We could also have moved
VM_FAULT_* definitions to a new header, or a different header that isn't
quite such a boil-the-ocean header as <linux/mm.h>, but this felt like
the best option.

FS_CGROUP_WRITEBACK indicates whether a file_system_type supports
cgroup writeback; however, different super_blocks of the same
file_system_type may or may not support cgroup writeback depending on
filesystem options. This patch replaces FS_CGROUP_WRITEBACK with a
per-super_block flag.
super_block->s_flags carries some internal flags in the high bits but
it's exposd to userland through uapi header and running out of space
anyway. This patch adds a new field super_block->s_iflags to carry
kernel-internal flags. It is currently only used by the new
SB_I_CGROUPWB flag whose concatenated and abbreviated name is for
consistency with other super_block flags.
ext2_fill_super() is updated to set SB_I_CGROUPWB.
v2: Added super_block->s_iflags instead of stealing another high bit
from sb->s_flags as suggested by Christoph and Jan.

dax_fault() currently relies on the get_block callback to attach an
io completion callback to the mapping buffer head so that it can
run unwritten extent conversion after zeroing allocated blocks.
Instead of this hack, pass the conversion callback directly into
dax_fault() similar to the get_block callback. When the filesystem
allocates unwritten extents, it will set the buffer_unwritten()
flag, and hence the dax_fault code can call the completion function
in the contexts where it is necessary without overloading the
mapping buffer head.
Note: The changes to ext4 to use this interface are suspect at best.
In fact, the way ext4 did this end_io assignment in the first place
looks suspect because it only set a completion callback when there
wasn't already some other write() call taking place on the same
inode. The ext4 end_io code looks rather intricate and fragile with
all it's reference counting and passing to different contexts for
modification via inode private pointers that aren't protected by
locks...

Writeback now supports cgroup writeback and the generic writeback,
buffer, libfs, and mpage helpers that ext2 uses are all updated to
work with cgroup writeback.
This patch enables cgroup writeback for ext2 by adding
FS_CGROUP_WRITEBACK to its ->fs_flags.

The original dax patchset split the ext2/4_file_operations because of the
two NULL splice_read/splice_write in the dax case.
In the vfs if splice_read/splice_write are NULL we then call
default_splice_read/write.
What we do here is make generic_file_splice_read aware of IS_DAX() so the
original ext2/4_file_operations can be used as is.
For write it appears that iter_file_splice_write is just fine. It uses
the regular f_op->write(file,..) or new_sync_write(file, ...).

The rw parameter to direct_IO is redundant with iov_iter->type, and
treated slightly differently just about everywhere it's used: some users
do rw & WRITE, and others do rw == WRITE where they should be doing a
bitwise check. Simplify this with the new iov_iter_rw() helper, which
always returns either READ or WRITE.

All places outside of core VFS that checked ->read and ->write for being NULL or
called the methods directly are gone now, so NULL {read,write} with non-NULL
{read,write}_iter will do the right thing in all cases.

Jan Kara pointed out that calling ext2_xip_verify_sb() in ext2_remount()
doesn't make sense, since changing the XIP option on remount isn't
allowed. It also doesn't make sense to re-check whether blocksize is
supported since it can't change between mounts.
Replace the call to ext2_xip_verify_sb() in ext2_fill_super() with the
equivalent check and delete the definition.

initialisers of it, documentation of it and the only implementation of it.

Instead of calling aops->get_xip_mem from the fault handler, the
filesystem passes a get_block_t that is used to find the appropriate
blocks.
This requires that all architectures implement copy_user_page(). At the
time of writing, mips and arm do not. Patches exist and are in progress.

This is practically generic code; other filesystems will want to call it
from other places, but there's nothing ext2-specific about it.
Make it a little more generic by allowing it to take a count of the number
of bytes to zero rather than fixing it to a single page. Thanks to Dave
Hansen for suggesting that I need to call cond_resched() if zeroing more
than one page.

Now that we got rid of the bdi abuse on character devices we can always use
sb->s_bdi to get at the backing_dev_info for a file, except for the block
device special case. Export inode_to_bdi and replace uses of
mapping->backing_dev_info with it to prepare for the removal of
mapping->backing_dev_info.

In order to support accesses to larger chunks of memory, pass in a
'size' parameter (counted in bytes), and return the amount available at
that address.
Add a new helper function, bdev_direct_access(), to handle common
functionality including partition handling, checking the length requested
is positive, checking for the sector being page-aligned, and checking
the length of the request does not pass the end of the partition.

Percpu allocator now supports allocation mask. Add @gfp to
percpu_counter_init() so that !GFP_KERNEL allocation masks can be used
with percpu_counters too.
We could have left percpu_counter_init() alone and added
percpu_counter_init_gfp(); however, the number of users isn't that
high and introducing _gfp variants to all percpu data structures would
be quite ugly, so let's just do the conversion. This is the one with
the most users. Other percpu data structures are a lot easier to
convert.
This patch doesn't make any functional difference.

Reclaim will be leaving shadow entries in the page cache radix tree upon
evicting the real page. As those pages are found from the LRU, an
iput() can lead to the inode being freed concurrently. At this point,
reclaim must no longer install shadow pages because the inode freeing
code needs to ensure the page tree is really empty.
Add an address_space flag, AS_EXITING, that the inode freeing code sets
under the tree lock before doing the final truncate. Reclaim will check
for this flag before installing shadow pages.

Many of the uses of get_random_bytes() do not actually need
cryptographically secure random numbers. Replace those uses with a
call to prandom_u32(), which is faster and which doesn't consume
entropy from the /dev/random driver.
The commit dd1f723bf5 has made that for
ext4, and i did the same for ext2/3.

ext2_quota_write() doesn't properly setup bh it passes to
ext2_get_block() and thus we hit assertion BUG_ON(maxblocks == 0) in
ext2_get_blocks() (or we could actually ask for mapping arbitrary number
of blocks depending on whatever value was on stack).
Fix ext2_quota_write() to properly fill in number of blocks to map.

Commit 8e3dffc651 "Ext2: mark inode dirty after the function
dquot_free_block_nodirty is called" unveiled a bug in __ext2_get_block()
called from ext2_get_xip_mem(). That function called ext2_get_block()
mistakenly asking it to map 0 blocks while 1 was intended. Before the
above mentioned commit things worked out fine by luck but after that commit
we started returning that we allocated 0 blocks while we in fact
allocated 1 block and thus allocation was looping until all blocks in
the filesystem were exhausted.
Fix the problem by properly asking for one block and also add assertion
in ext2_get_blocks() to catch similar problems.

Commit 8e3dffc6 introduced a regression where deleting inode with
large extended attributes leads to triggering
BUG_ON(inode->i_state != (I_FREEING | I_CLEAR))
in fs/inode.c:evict(). That happens because freeing of xattr block
dirtied the inode and it happened after clear_inode() has been called.
Fix the issue by moving removal of xattr block into ext2_evict_inode()
before clear_inode() call close to a place where data blocks are
truncated. That is also more logical place and removes surprising
requirement that ext2_free_blocks() mustn't dirty the inode.

Modify the request_module to prefix the file system type with "fs-"
and add aliases to all of the filesystems that can be built as modules
to match.
A common practice is to build all of the kernel code and leave code
that is not commonly needed as modules, with the result that many
users are exposed to any bug anywhere in the kernel.
Looking for filesystems with a fs- prefix limits the pool of possible
modules that can be loaded by mount to just filesystems trivially
making things safer with no real cost.
Using aliases means user space can control the policy of which
filesystem modules are auto-loaded by editing /etc/modprobe.d/*.conf
with blacklist and alias directives. Allowing simple, safe,
well understood work-arounds to known problematic software.
This also addresses a rare but unfortunate problem where the filesystem
name is not the same as it's module name and module auto-loading
would not work. While writing this patch I saw a handful of such
cases. The most significant being autofs that lives in the module
autofs4.
This is relevant to user namespaces because we can reach the request
module in get_fs_type() without having any special permissions, and
people get uncomfortable when a user specified string (in this case
the filesystem type) goes all of the way to request_module.
After having looked at this issue I don't think there is any
particular reason to perform any filtering or permission checks beyond
making it clear in the module request that we want a filesystem
module. The common pattern in the kernel is to call request_module()
without regards to the users permissions. In general all a filesystem
module does once loaded is call register_filesystem() and go to sleep.
Which means there is not much attack surface exposed by loading a
filesytem module unless the filesystem is mounted. In a user
namespace filesystems are not mounted unless .fs_flags = FS_USERNS_MOUNT,
which most filesystems do not set today.

There's no reason to call rcu_barrier() on every
deactivate_locked_super(). We only need to make sure that all delayed rcu
free inodes are flushed before we destroy related cache.
Removing rcu_barrier() from deactivate_locked_super() affects some fast
paths. E.g. on my machine exit_group() of a last process in IPC
namespace takes 0.07538s. rcu_barrier() takes 0.05188s of that time.

Pull user namespace changes from Eric Biederman:
"This is a mostly modest set of changes to enable basic user namespace
support. This allows the code to code to compile with user namespaces
enabled and removes the assumption there is only the initial user
namespace. Everything is converted except for the most complex of the
filesystems: autofs4, 9p, afs, ceph, cifs, coda, fuse, gfs2, ncpfs,
nfs, ocfs2 and xfs as those patches need a bit more review.
The strategy is to push kuid_t and kgid_t values are far down into
subsystems and filesystems as reasonable. Leaving the make_kuid and
from_kuid operations to happen at the edge of userspace, as the values
come off the disk, and as the values come in from the network.
Letting compile type incompatible compile errors (present when user
namespaces are enabled) guide me to find the issues.
The most tricky areas have been the places where we had an implicit
union of uid and gid values and were storing them in an unsigned int.
Those places were converted into explicit unions. I made certain to
handle those places with simple trivial patches.
Out of that work I discovered we have generic interfaces for storing
quota by projid. I had never heard of the project identifiers before.
Adding full user namespace support for project identifiers accounts
for most of the code size growth in my git tree.
Ultimately there will be work to relax privlige checks from
"capable(FOO)" to "ns_capable(user_ns, FOO)" where it is safe allowing
root in a user names to do those things that today we only forbid to
non-root users because it will confuse suid root applications.
While I was pushing kuid_t and kgid_t changes deep into the audit code
I made a few other cleanups. I capitalized on the fact we process
netlink messages in the context of the message sender. I removed
usage of NETLINK_CRED, and started directly using current->tty.
Some of these patches have also made it into maintainer trees, with no
problems from identical code from different trees showing up in
linux-next.
After reading through all of this code I feel like I might be able to
win a game of kernel trivial pursuit."

- Pass the user namespace the uid and gid values in the xattr are stored
in into posix_acl_from_xattr.
- Pass the user namespace kuid and kgid values should be converted into
when storing uid and gid values in an xattr in posix_acl_to_xattr.
- Modify all callers of posix_acl_from_xattr and posix_acl_to_xattr to
pass in &init_user_ns.
In the short term this change is not strictly needed but it makes the
code clearer. In the longer term this change is necessary to be able to
mount filesystems outside of the initial user namespace that natively
store posix acls in the linux xattr format.

The only missing piece to make freezing work reliably with ext2 is to
stop iput() of unlinked inode from deleting the inode on frozen filesystem.
So add a necessary protection to ext2_evict_inode().
We also provide appropriate ->freeze_fs and ->unfreeze_fs functions.

Convert ext2_count_free() to use memweight() instead of table lookup
based counting clear bits implementation. This change only affects the
code segments enabled by EXT2FS_DEBUG.
Note that this memweight() call can't be replaced with a single
bitmap_weight() call, although the pointer to the memory area is aligned
to long-word boundary. Because the size of the memory area may not be a
multiple of BITS_PER_LONG, then it returns wrong value on big-endian
architecture.
This also includes the following changes.
- Remove unnecessary map == NULL check in ext2_count_free() which
always takes non-null pointer as the memory area.
- Fix printk format warning that only reveals with EXT2FS_DEBUG.

Since the moment writes to quota files are using block device page cache and
space for quota structures is reserved at the moment they are first accessed we
have no reason to sync quota before inode writeback. In fact this order is now
only harmful since quota information can easily change during inode writeback
(either because conversion of delayed-allocated extents or simply because of
allocation of new blocks for simple filesystems not using page_mkwrite).
So move syncing of quota information after writeback of inodes into ->sync_fs
method. This way we do not have to use ->quota_sync callback which is primarily
intended for use by quotactl syscall anyway and we get rid of calling
->sync_fs() twice unnecessarily. We skip quota syncing for OCFS2 since it does
proper quota journalling in all cases (unlike ext3, ext4, and reiserfs which
also support legacy non-journalled quotas) and thus there are no dirty quota
structures.

Pull user namespace enhancements from Eric Biederman:
"This is a course correction for the user namespace, so that we can
reach an inexpensive, maintainable, and reasonably complete
implementation.
Highlights:
- Config guards make it impossible to enable the user namespace and
code that has not been converted to be user namespace safe.
- Use of the new kuid_t type ensures the if you somehow get past the
config guards the kernel will encounter type errors if you enable
user namespaces and attempt to compile in code whose permission
checks have not been updated to be user namespace safe.
- All uids from child user namespaces are mapped into the initial
user namespace before they are processed. Removing the need to add
an additional check to see if the user namespace of the compared
uids remains the same.
- With the user namespaces compiled out the performance is as good or
better than it is today.
- For most operations absolutely nothing changes performance or
operationally with the user namespace enabled.
- The worst case performance I could come up with was timing 1
billion cache cold stat operations with the user namespace code
enabled. This went from 156s to 164s on my laptop (or 156ns to
164ns per stat operation).
- (uid_t)-1 and (gid_t)-1 are reserved as an internal error value.
Most uid/gid setting system calls treat these value specially
anyway so attempting to use -1 as a uid would likely cause
entertaining failures in userspace.
- If setuid is called with a uid that can not be mapped setuid fails.
I have looked at sendmail, login, ssh and every other program I
could think of that would call setuid and they all check for and
handle the case where setuid fails.
- If stat or a similar system call is called from a context in which
we can not map a uid we lie and return overflowuid. The LFS
experience suggests not lying and returning an error code might be
better, but the historical precedent with uids is different and I
can not think of anything that would break by lying about a uid we
can't map.
- Capabilities are localized to the current user namespace making it
safe to give the initial user in a user namespace all capabilities.
My git tree covers all of the modifications needed to convert the core
kernel and enough changes to make a system bootable to runlevel 1."

The comment is outdated and isn't particularly informative anyway - NULL
meaning the default behavior is very common in kernel. And we really set about
half of entries. So remove the whole comment for ext2_export_ops.

We don't need i_mutex in ext2_quota_write() because writes to quota file
are serialized by dqio_mutex anyway. Changes to quota files outside of quota
code are forbidded and enforced by NOATIME and IMMUTABLE bits.

This allows comparing hash and len in one operation on 64-bit
architectures. Right now only __d_lookup_rcu() takes advantage of this,
since that is the case we care most about.
The use of anonymous struct/unions hides the alternate 64-bit approach
from most users, the exception being a few cases where we initialize a
'struct qstr' with a static initializer. This makes the problematic
cases use a new QSTR_INIT() helper function for that (but initializing
just the name pointer with a "{ .name = xyzzy }" initializer remains
valid, as does just copying another qstr structure).

After we moved inode_sync_wait() from end_writeback() it doesn't make sense
to call the function end_writeback() anymore. Rename it to clear_inode()
which well says what the function really does - set I_CLEAR flag.
