commit d2ac7bef95c9ead307801ccb6cb6dfbeb14247bf upstream.
Generic PHYs must be powered-off before they can be tore down.
Similarly, suspending legacy PHYs after having powered them off makes no
sense.
Fix the dwc3_core_exit() (e.g. called during suspend) and open-coded
dwc3_probe() error-path sequences that got this wrong.
Note that this makes dwc3_core_exit() match the dwc3_core_init() error
path with respect to powering off the PHYs.

commit 7441b273388b9a59d8387a03ffbbca9d5af6348c upstream.
The DWC3_EVENT_PENDING flag is used to protect against invalid call to
top-half interrupt handler, which can occur when there's a delay in
software detection of the interrupt line deassertion.
However, the clearing of this flag was done prior to unmasking the
interrupt line, creating opportunity where the top-half handler can
come. This breaks the serialization and creates a race between the
top-half and bottom-half handler, resulting in losing synchronization
between the controller and the driver when processing events.
To fix this, make sure the clearing of the DWC3_EVENT_PENDING is done at
the end of the bottom-half handler.

commit 84918a89d6efaff075de570b55642b6f4ceeac6d upstream.
The interrupt service routine registered for the gadget is a primary
handler which mask the interrupt source and a threaded handler which
handles the source of the interrupt. Since the threaded handler is
voluntary threaded, the IRQ-core does not disable bottom halves before
invoke the handler like it does for the forced-threaded handler.
Due to changes in networking it became visible that a network gadget's
completions handler may schedule a softirq which remains unprocessed.
The gadget's completion handler is usually invoked either in hard-IRQ or
soft-IRQ context. In this context it is enough to just raise the softirq
because the softirq itself will be handled once that context is left.
In the case of the voluntary threaded handler, there is nothing that
will process pending softirqs. Which means it remain queued until
another random interrupt (on this CPU) fires and handles it on its exit
path or another thread locks and unlocks a lock with the bh suffix.
Worst case is that the CPU goes idle and the NOHZ complains about
unhandled softirqs.
Disable bottom halves before acquiring the lock (and disabling
interrupts) and enable them after dropping the lock. This ensures that
any pending softirqs will handled right away.

commit 117b4e96c7f362eb6459543883fc07f77662472c upstream.
With CPU re-ordering on write instructions, there might
be a chance that the HWO is set before the TRB is updated
with the new mapped buffer address.
And in the case where core is processing a list of TRBs
it is possible that it fetched the TRBs when the HWO is set
but before the buffer address is updated.
Prevent this by adding a memory barrier before the HWO
is updated to ensure that the core always process the
updated TRBs.

commit 51f1954ad853d01ba4dc2b35dee14d8490ee05a1 upstream.
We can't depend on the TRB's HWO bit to determine if the TRB ring is
"full". A TRB is only available when the driver had processed it, not
when the controller consumed and relinquished the TRB's ownership to the
driver. Otherwise, the driver may overwrite unprocessed TRBs. This can
happen when many transfer events accumulate and the system is slow to
process them and/or when there are too many small requests.
If a request is in the started_list, that means there is one or more
unprocessed TRBs remained. Check this instead of the TRB's HWO bit
whether the TRB ring is full.

commit d00889080ab60051627dab1d85831cd9db750e2a upstream.
There is no validation of the index from dwc3_wIndex_to_dep() and we might
be referring a non-existing ep and trigger a NULL pointer exception. In
certain configurations we might use fewer eps and the index might wrongly
indicate a larger ep index than existing.
By adding this validation from the patch we can actually report a wrong
index back to the caller.
In our usecase we are using a composite device on an older kernel, but
upstream might use this fix also. Unfortunately, I cannot describe the
hardware for others to reproduce the issue as it is a proprietary
implementation.

coming from a disconnected scenario, i.e. after receiving the disconnect

connected parameter will still be set to true, allowing for EP queuing
to continue while we are disabling the functions. To avoid this, set the
connected flag to false until the stop active transfers is complete.

commit fca3f138105727c3a22edda32d02f91ce1bf11c9 upstream
Originally the procedure of the ULPI transaction finish detection has been
developed as a simple busy-loop with just decrementing counter and no
delays. It's wrong since on different systems the loop will take a
different time to complete. So if the system bus and CPU are fast enough
to overtake the ULPI bus and the companion PHY reaction, then we'll get to
take a false timeout error. Fix this by converting the busy-loop procedure
to take the standard bus speed, address value and the registers access
mode into account for the busy-loop delay calculation.
Here is the way the fix works. It's known that the ULPI bus is clocked
with 60MHz signal. In accordance with [1] the ULPI bus protocol is created
so to spend 5 and 6 clock periods for immediate register write and read
operations respectively, and 6 and 7 clock periods - for the extended
register writes and reads. Based on that we can easily pre-calculate the
time which will be needed for the controller to perform a requested IO
operation. Note we'll still preserve the attempts counter in case if the
DWC USB3 controller has got some internals delays.

with IRQ handler when deferred probing happens due to extcon,
therefore IRQ_NOAUTOEN needs to be set so that irq is not enabled until
extcon is registered.
Remove setting of IRQ_NOAUTOEN and move the registration of
shared irq to a point after dwc3_omap_extcon_register() and
of_platform_populate(). This avoids possibility of probe deferring and
above said race condition.

commit 6180026341e852a250e1f97ebdcf71684a3c81b9 upstream.
There are 2 control endpoint structures for DWC3. However, the driver
only updates the OUT direction control endpoint structure during
ConnectDone event. DWC3 driver needs to update the endpoint max packet
size for control IN endpoint as well. If the max packet size is not
properly set, then the driver will incorrectly calculate the data
transfer size and fail to send ZLP for HS/FS 3-stage control read
transfer.
The fix is simply to update the max packet size for the ep0 IN direction
during ConnectDone event.

commit 40d829fb2ec636b6b4b0cc95e2546ab9aca04cc9 upstream.
The PIDs for Isochronous data transfers are incorrect
for high bandwidth IN endpoints when the request length
is less than EP wMaxPacketSize.
As per spec correct PIDs for ISOC data transfers are:
1) For request length <= maxpacket
- DATA0,
2) For maxpacket < length <= (2 * maxpacket)
- DATA1, DATA0
3) For (2 * maxpacket) < length <= (3 * maxpacket)
- DATA2, DATA1, DATA0.
But driver always sets PCM fields based on wMaxPacketSize
due to which DATA2 happens even for small requests.
Fix this by setting the PCM field of trb->size depending
on request length rather than fixing it to the value
depending on wMaxPacketSize.
Ideally it shouldn't give any issues as dwc3 will send
0-length packet for next IN token if host sends (even
after receiving a short packet). Windows seems to ignore
this but with MacOS frame loss observed when using f_uvc.

Now races can happen between interrupt handler execution and PM runtime in
error handling code path in probe and in dwc3_omap_remove() which will lead
to system crash:
in probe:
...
err1:
pm_runtime_put_sync(dev);
^^ PM runtime can race with IRQ handler when deferred probing happening
due to extcon
pm_runtime_disable(dev);
return ret;
in dwc3_omap_remove:
...
dwc3_omap_disable_irqs(omap);
^^ IRQs are disabled in HW, but handler may still run
of_platform_depopulate(omap->dev);
pm_runtime_put_sync(&pdev->dev);
^^ PM runtime can race with IRQ handler
pm_runtime_disable(&pdev->dev);
return 0;
So, OMAP DWC3 IRQ need to be disabled before calling
pm_runtime_put() in probe and in dwc3_omap_remove().

commit d325a1de49d61ee11aca58a529571c91ecea7879 upstream.
The dwc3 driver can overwite its previous events if its top-half IRQ
handler (TH) gets invoked again before processing the events in the
cache. We see this as a hang in the file transfer and the host will
attempt to reset the device. TH gets the event count and deasserts the
interrupt line by writing DWC3_GEVNTSIZ_INTMASK to DWC3_GEVNTSIZ. If
there's a new event coming between reading the event count and interrupt
deassertion, dwc3 will lose previous pending events. More generally, we
will see 0 event count, which should not affect anything.
This shouldn't be possible in the current dwc3 implementation. However,
through testing and reading the PCIe trace, the TH occasionally still
gets invoked one more time after HW interrupt deassertion. (With PCIe
legacy interrupts, TH is called repeatedly as long as the interrupt line
is asserted). We suspect that there is a small detection delay in the
SW.
To avoid this issue, Check DWC3_EVENT_PENDING flag to determine if the
events are processed in the bottom-half IRQ handler. If not, return
IRQ_HANDLED and don't process new event.
