b0cab8b517 RDMA/cm: Fix memory leak in ib_cm_insert_listen
commit 2990f223ffa7bb25422956b9f79f9176a5b38346 upstream.

cm_alloc_id_priv() allocates resource for the cm_id_priv. When
cm_init_listen() fails it doesn't free it, leading to memory leak.

Add the missing error unwind.

bb55ca1612 RDMA/rxe: Generate a completion for unsupported/invalid opcode
commit 2f917af777011c88e977b9b9a5d00b280d3a59ce upstream.

Current rxe_requester() doesn't generate a completion when processing an
unsupported/invalid opcode. If rxe driver doesn't support a new opcode
(e.g. RDMA Atomic Write) and RDMA library supports it, an application
using the new opcode can reproduce this issue. Fix the issue by calling

31dca00d0c RDMA/hfi1: Fix potential integer multiplication overflow errors
commit f93e91a0372c922c20d5bee260b0f43b4b8a1bee upstream.

When multiplying of different types, an overflow is possible even when
storing the result in a larger type. This is because the conversion is
done after the multiplication. So arithmetic overflow and thus in
incorrect value is possible.

Correct an instance of this in the inter packet delay calculation. Fix by
ensuring one of the operands is u64 which will promote the other to u64 as
well ensuring no overflow.

fc0750e659 RDMA/hfi1: Prevent use of lock before it is initialized
[ Upstream commit 05c03dfd09c069c4ffd783b47b2da5dcc9421f2c ]

If there is a failure during probe of hfi1 before the sdma_map_lock is
initialized, the call to hfi1_free_devdata() will attempt to use a lock
that has not been initialized. If the locking correctness validator is on
then an INFO message and stack trace resembling the following may be seen:

  INFO: trying to register non-static key.
  The code is fine but needs lockdep annotation, or maybe
  you didn't initialize this object before use?
  turning off the locking correctness validator.

The use of sdma_map_lock in sdma_clean() is for freeing the sdma_map
memory, and sdma_map is not allocated/initialized until after
sdma_map_lock has been initialized. This code only needs to be run if
sdma_map is not NULL, and so checking for that condition will avoid trying
to use the lock before it is initialized.

cc80d3c37c RDMA/hfi1: Prevent panic when SDMA is disabled
[ Upstream commit 629e052d0c98e46dde9f0824f0aa437f678d9b8f ]

If the hfi1 module is loaded with HFI1_CAP_SDMA off, a call to
hfi1_write_iter() will dereference a NULL pointer and panic. A typical
stack frame is:

  sdma_select_user_engine [hfi1]
  hfi1_user_sdma_process_request [hfi1]
  hfi1_write_iter [hfi1]
  do_iter_readv_writev
  do_iter_write
  vfs_writev
  do_writev
  do_syscall_64

The fix is to test for SDMA in hfi1_write_iter() and fail the I/O with
EINVAL.

968a668376 IB/rdmavt: add missing locks in rvt_ruc_loopback
[ Upstream commit 22cbc6c2681a0a4fe76150270426e763d52353a4 ]

The documentation of the function rvt_error_qp says both r_lock and
s_lock need to be held when calling that function.
It also asserts using lockdep that both of those locks are held.
rvt_error_qp is called form rvt_send_cq, which is called from
rvt_qp_complete_swqe, which is called from rvt_send_complete, which is
called from rvt_ruc_loopback in two places. Both of these places do not
hold r_lock. Fix this by acquiring a spin_lock of r_lock in both of
these places.
The r_lock acquiring cannot be added in rvt_qp_complete_swqe because
some of its other callers already have r_lock acquired.

17d14456f6 scsi: iscsi: Stop queueing during ep_disconnect
[ Upstream commit 891e2639deae721dc43764a44fa255890dc34313 ]

During ep_disconnect we have been doing iscsi_suspend_tx/queue to block new
I/O but every driver except cxgbi and iscsi_tcp can still get I/O from
__iscsi_conn_send_pdu() if we haven't called iscsi_conn_failure() before
ep_disconnect. This could happen if we were terminating the session, and
the logout timed out before it was even sent to libiscsi.

Fix the issue by adding a helper which reverses the bind_conn call that
allows new I/O to be queued. Drivers implementing ep_disconnect can use this
to make sure new I/O is not queued to them when handling the disconnect.

5f54364ff6 RDMA/hfi1: Fix use-after-free bug for mm struct
commit 2bbac98d0930e8161b1957dc0ec99de39ade1b3c upstream.

Under certain conditions, such as MPI_Abort, the hfi1 cleanup code may
represent the last reference held on the task mm.
hfi1_mmu_rb_unregister() then drops the last reference and the mm is freed
before the final use in hfi1_release_user_pages(). A new task may
allocate the mm structure while it is still being used, resulting in
problems. One manifestation is corruption of the mmap_sem counter leading
to a hang in down_write(). Another is corruption of an mm struct that is
in use by another task.

43c2d7890e IB/rdmavt: add lock to call to rvt_error_qp to prevent a race condition
[ Upstream commit 4d809f69695d4e7d1378b3a072fa9aef23123018 ]

The documentation of the function rvt_error_qp says both r_lock and s_lock
need to be held when calling that function. It also asserts using lockdep
that both of those locks are held. However, the commit I referenced in
Fixes accidentally makes the call to rvt_error_qp in rvt_ruc_loopback no
longer covered by r_lock. This results in the lockdep assertion failing

5b1cef5798 RDMA/cma: Do not change route.addr.src_addr outside state checks
commit 22e9f71072fa605cbf033158db58e0790101928d upstream.

If the state is not idle then resolve_prepare_src() should immediately
fail and no change to global state should happen. However, it
unconditionally overwrites the src_addr trying to build a temporary any

For instance if the state is already RDMA_CM_LISTEN then this will corrupt
the src_addr and would cause the test in cma_cancel_operation():

           if (cma_any_addr(cma_src_addr(id_priv)) && !id_priv->cma_dev)

Which would manifest as this trace from syzkaller:

This is indicating that an rdma_id_private was destroyed without doing
cma_cancel_listens().

Instead of trying to re-use the src_addr memory to indirectly create an

bind to that as any other normal flow would do. rdma_bind_addr() will copy
it over the src_addr once it knows the state is valid.

The panic happens in hfi1_ipoib_txreq_deinit() because there is a NULL
deref when hfi1_ipoib_netdev_dtor() is called in this error case.

hfi1_ipoib_txreq_init() and hfi1_ipoib_rxq_init() are self unwinding so
fix by adjusting the error paths accordingly.

Other changes:
- hfi1_ipoib_free_rdma_netdev() is deleted including the free_netdev()
  since the netdev core code deletes calls free_netdev()
- The switch to the accelerated entrances is moved to the success path.

04a032ea24 RDMA/bnxt_re: Scan the whole bitmap when checking if "disabling RCFW with pending cmd-bit"
[ Upstream commit a917dfb66c0a1fa1caacf3d71edcafcab48e6ff0 ]

The 'cmdq->cmdq_bitmap' bitmap is 'rcfw->cmdq_depth' bits long. The size
stored in 'cmdq->bmap_size' is the size of the bitmap in bytes.

Remove this erroneous 'bmap_size' and use 'rcfw->cmdq_depth' directly in
'bnxt_qplib_disable_rcfw_channel()'. Otherwise some error messages may be
missing.

Other uses of 'cmdq_bitmap' already take into account 'rcfw->cmdq_depth'
directly.

4d12546cf9 RDMA/hns: Do not destroy QP resources in the hw resetting phase
commit b0969f83890bf8b47f5c8bd42539599b2b52fdeb upstream.

When hns_roce_v2_destroy_qp() is called, the brief calling process of the
driver is as follows:

 ......
 hns_roce_v2_destroy_qp
 hns_roce_v2_qp_modify
	   hns_roce_cmd_mbox
 hns_roce_qp_destroy

If hns_roce_cmd_mbox() detects that the hardware is being reset during the
execution of the hns_roce_cmd_mbox(), the driver will not be able to get
the return value from the hardware (the firmware cannot respond to the
driver's mailbox during the hardware reset phase).

The driver needs to wait for the hardware reset to complete before
continuing to execute hns_roce_qp_destroy(), otherwise it may happen that
the driver releases the resources but the hardware is still accessing. In
order to fix this problem, HNS RoCE needs to add a piece of code to wait
for the hardware reset to complete.

The original interface get_hw_reset_stat() is the instantaneous state of
the hardware reset, which cannot accurately reflect whether the hardware
reset is completed, so it needs to be replaced with the ae_dev_reset_cnt
interface.

The sign that the hardware reset is complete is that the return value of
the ae_dev_reset_cnt interface is greater than the original value
reset_cnt recorded by the driver.

commit 52414e27d6b568120b087d1fbafbb4482b0ccaab upstream.

is_reset is used to indicate whether the hardware starts to reset. When

of resource destroy actions will be intercepted by driver. When the driver
cleans up resources, but the hardware is still accessed, the following
errors will appear:

2c08271f4e IB/hfi1: Fix leak of rcvhdrtail_dummy_kvaddr
commit 60a8b5a1611b4a26de4839ab9c1fc2a9cf3e17c1 upstream.

This buffer is currently allocated in hfi1_init():

	if (reinit)
		ret = init_after_reset(dd);
	else
		ret = loadtime_init(dd);
	if (ret)
		goto done;

	/* allocate dummy tail memory for all receive contexts */
	dd->rcvhdrtail_dummy_kvaddr = dma_alloc_coherent(&dd->pcidev->dev,
							 sizeof(u64),
							 &dd->rcvhdrtail_dummy_dma,
							 GFP_KERNEL);

	if (!dd->rcvhdrtail_dummy_kvaddr) {
		dd_dev_err(dd, "cannot allocate dummy tail memory\n");
		ret = -ENOMEM;
		goto done;
	}

The reinit triggered path will overwrite the old allocation and leak it.

Fix by moving the allocation to hfi1_alloc_devdata() and the deallocation
to hfi1_free_devdata().
