On KVM GSI routing table updates, specially those where they have vIOMMUs
with interrupt remapping enabled (to boot >255vcpus setups without relying
on KVM_FEATURE_MSI_EXT_DEST_ID), a VMM may update the backing VF MSIs
with a new VCPU affinity.
On AMD with AVIC enabled, the new vcpu affinity info is updated via:
avic_pi_update_irte()
irq_set_vcpu_affinity()
amd_ir_set_vcpu_affinity()
amd_iommu_{de}activate_guest_mode()
Where the IRTE[GATag] is updated with the new vcpu affinity. The GATag
contains VM ID and VCPU ID, and is used by IOMMU hardware to signal KVM
(via GALog) when interrupt cannot be delivered due to vCPU is in
blocking state.
The issue is that amd_iommu_activate_guest_mode() will essentially
only change IRTE fields on transitions from non-guest-mode to guest-mode
and otherwise returns *with no changes to IRTE* on already configured
guest-mode interrupts. To the guest this means that the VF interrupts
remain affined to the first vCPU they were first configured, and guest
will be unable to issue VF interrupts and receive messages like this
from spurious interrupts (e.g. from waking the wrong vCPU in GALog):

Given the fact that amd_ir_set_vcpu_affinity() uses
amd_iommu_activate_guest_mode() underneath it essentially means that VCPU
affinity changes of IRTEs are nops. Fix it by dropping the check for
guest-mode at amd_iommu_activate_guest_mode(). Same thing is applicable to
amd_iommu_deactivate_guest_mode() although, even if the IRTE doesn't change
underlying DestID on the host, the VFIO IRQ handler will still be able to
poke at the right guest-vCPU.

Smatch complains that
drivers/iommu/rockchip-iommu.c:1306 rk_iommu_probe() warn: missing unwind goto?
The rk_iommu_probe function, after obtaining the irq value through
platform_get_irq, directly returns an error if the returned value
is negative, without releasing any resources.
Fix this by adding a new error handling label "err_pm_disable" and
use a goto statement to redirect to the error handling process. In
order to preserve the original semantics, set err to the value of irq.

When an overflow occurs in the PRI queue, the SMMU toggles the overflow
flag in the PROD register. To exit the overflow condition, the PRI thread
is supposed to acknowledge it by toggling this flag in the CONS register.
Unacknowledged overflow causes the queue to stop adding anything new.
Currently, the priq thread always writes the CONS register back to the
SMMU after clearing the queue.
The writeback is not necessary if the OVFLG in the PROD register has not
been changed, no overflow has occured.
This commit checks the difference of the overflow flag between CONS and
PROD register. If it's different, toggles the OVACKFLG flag in the CONS
register and write it to the SMMU.
The situation is similar for the event queue.
The acknowledge register is also toggled after clearing the event
queue but never propagated to the hardware. This would only be done the
next time when executing evtq thread.
Unacknowledged event queue overflow doesn't affect the event
queue, because the SMMU still adds elements to that queue when the
overflow condition is active.
But it feel nicer to keep SMMU in sync when possible, so use the same
way here as well.

Some platforms support more than 128 stream matching groups than what is
defined by the ARM SMMU architecture specification. But due to some unknown
reasons, those additional groups don't exhibit the same behavior as the
architecture supported ones.
For instance, the additional groups will not detect the quirky behavior of
some firmware versions intercepting writes to S2CR register, thus skipping
the quirk implemented in the driver and causing boot crash.
So let's limit the groups to 128 for now until the issue with those groups
are fixed and issue a notice to users in that case.

commit b9c6ff94e43a ("iommu/amd: Re-factor guest virtual APIC
(de-)activation code") while refactoring guest virtual APIC
activation/de-activation code, stored information for activate/de-activate
in "struct amd_ir_data". It used 32-bit integer data type for storing the
"Guest Virtual APIC Table Root Pointer" (ga_root_ptr), though the
"ga_root_ptr" is actually a 40-bit field in IRTE (Interrupt Remapping
Table Entry).
This causes interrupts from PCIe devices to not reach the guest in the case
of PCIe passthrough with SME (Secure Memory Encryption) enabled as _SME_
bit in the "ga_root_ptr" is lost before writing it to the IRTE.
Fix it by using 64-bit data type for storing the "ga_root_ptr". While at
that also change the data type of "ga_tag" to u32 in order to match
the IOMMU spec.

On platforms that do not support IOMMU Extended capability bit 0
Page-walk Coherency, CPU caches are not snooped when IOMMU is accessing
any translation structures. IOMMU access goes only directly to
memory. Intel IOMMU code was missing a flush for the PASID table
directory that resulted in the unrecoverable fault as shown below.
This patch adds clflush calls whenever allocating and updating
a PASID table directory to ensure cache coherency.
On the reverse direction, there's no need to clflush the PASID directory
pointer when we deactivate a context entry in that IOMMU hardware will
not see the old PASID directory pointer after we clear the context entry.
PASID directory entries are also never freed once allocated.

The second (UID) strcmp in acpi_dev_hid_uid_match considers
"0" and "00" different, which can prevent device registration.
Have the AMD IOMMU driver's ivrs_acpihid parsing code remove
any leading zeroes to make the UID strcmp succeed. Now users
can safely specify "AMDxxxxx:00" or "AMDxxxxx:0" and expect
the same behaviour.

This driver treats IOMMU_DOMAIN_IDENTITY the same as UNMANAGED, which
cannot possibly be correct.
UNMANAGED domains are required to start out blocking all DMAs. This seems
to be what this driver does as it allocates a first level 'dt' for the IO
page table that is 0 filled.
Thus UNMANAGED looks like a working IO page table, and so IDENTITY must be
a mistake. Remove it.

As comment of pci_get_domain_bus_and_slot() says, it returns
a pci device with refcount increment, when finish using it,
the caller must decrement the reference count by calling
pci_dev_put(). So call it before returning from ppr_notifier()
to avoid refcount leak.

Because driver has enum type permissions and iommu subsystem has bitmap
type, we have to be careful how check for combined read and write
permissions is done. In such case, we have to mask both permissions and
check that both are set at the same time.
Current code just masks both flags but doesn't check that both are set.
In short, it always sets R/W permission, regardles if requested
permissions were RO, WO or RW. Fix that.

We have to reset masters for all faults - permissions, L1 fault or L2
fault. Currently it's done only for permissions. If other type of fault
happens, master is in locked up state. Fix that by really considering
all fault sources.

Reset signal is asserted by writing 0 to the corresponding locations of
masters we want to reset. So in order to deassert all reset signals, we
should write 1's to all locations.
Current code writes 1's to locations of masters which were just reset
which is good. However, at the same time it also writes 0's to other
locations and thus asserts reset signals of remaining masters. Fix code
by writing all 1's when we want to deassert all reset signals.
This bug was discovered when working with Cedrus (video decoder). When
it faulted, display went blank due to reset signal assertion.

If we break for_each_pci_dev() loop with pdev not NULL, we need to call

pci_dev_put() for the error path to avoid reference count leak.

requests seeking supervisor privilege or not. SRE bit in scalable-mode
PASID table entry is treated as Reserved(0) for implementation not
supporting SRS cap.
Checking SRS cap before setting SRE bit can avoid the non-recoverable
fault of "Non-zero reserved field set in PASID Table Entry" caused by
setting SRE bit while there is no SRS cap support. The fault messages
look like below:

A splat from kmem_cache_destroy() was seen with a kernel prior to
commit ee2653bbe89d ("iommu/vt-d: Remove domain and devinfo mempool")
when there was a failure in init_dmars(), because the iommu_domain
cache still had objects. While the mempool code is now gone, there
still is a leak of the si_domain memory if init_dmars() fails. So
clean up si_domain in the init_dmars() error path.

There are two issues here:
1) The "len" variable needs to be checked before the very first write.
Otherwise if omap2_iommu_dump_ctx() with "bytes" less than 32 it is a
buffer overflow.
2) The snprintf() function returns the number of bytes that *would* have
been copied if there were enough space. But we want to know the
number of bytes which were *actually* copied so use scnprintf()
instead.

The Intel IOMMU driver possibly selects between the first-level and the
second-level translation tables for DMA address translation. However,
the levels of page-table walks for the 4KB base page size are calculated
from the SAGAW field of the capability register, which is only valid for
the second-level page table. This causes the IOMMU driver to stop working
if the hardware (or the emulated IOMMU) advertises only first-level
translation capability and reports the SAGAW field as 0.
This solves the above problem by considering both the first level and the
second level when calculating the supported page table levels.

We started using a 64 bit completion value. Unfortunately, we only
stored the low 32-bits, so a very large completion value would never
be matched in iommu_completion_wait().

of arch_test_bit() from `long` to `unsigned long`[0].
Under certain conditions (for example, when ACPI NUMA is disabled
via command line), pxm_to_node() can return %NUMA_NO_NODE (-1).
It is valid 'magic' number of NUMA node, but not valid bit number
to use in bitops.
node_online() eventually descends to test_bit() without checking
for the input, assuming it's on caller side (which might be good
for perf-critical tasks). There, -1 becomes %ULONG_MAX which leads
to an insane array index when calculating bit position in memory.
For now, add an explicit check for @node being not %NUMA_NO_NODE
before calling test_bit(). The actual logics didn't change here
at all.

Notifier calling chain uses priority to determine the execution
order of the notifiers or listeners registered to the chain.
PCI bus device hot add utilizes the notification mechanism.
The current code sets low priority (INT_MIN) to Intel
dmar_pci_bus_notifier and postpones DMAR decoding after adding
new device into IOMMU. The result is that struct device pointer
cannot be found in DRHD search for the new device's DMAR/IOMMU.
Subsequently, the device is put under the "catch-all" IOMMU
instead of the correct one. This could cause system hang when
device TLB invalidation is sent to the wrong IOMMU. Invalidation
timeout error and hard lockup have been observed and data
inconsistency/crush may occur as well.
This patch fixes the issue by setting a positive priority(1) for
dmar_pci_bus_notifier while the priority of IOMMU bus notifier
uses the default value(0), therefore DMAR decoding will be in
advance of DRHD search for a new device to find the correct IOMMU.
Following is a 2-step example that triggers the bug by simulating
PCI device hot add behavior in Intel Sapphire Rapids server.

will check it to avoid null-ptr-deref.

The bug is here:
if (!iommu || iommu->dev->of_node != spec->np) {
The list iterator value 'iommu' will *always* be set and non-NULL by
list_for_each_entry(), so it is incorrect to assume that the iterator
value will be NULL if the list is empty or no element is found (in fact,
it will point to a invalid structure object containing HEAD).
To fix the bug, use a new value 'iter' as the list iterator, while use
the old value 'iommu' as a dedicated variable to point to the found one,
and remove the unneeded check for 'iommu->dev->of_node != spec->np'
outside the loop.

On some systems it can take a long time for the hardware to enable the
GA log of the AMD IOMMU. The current wait time is only 0.1ms, but
testing showed that it can take up to 14ms for the GA log to enter
running state after it has been enabled.
Sometimes the long delay happens when booting the system, sometimes
only on resume. Adjust the timeout accordingly to not print a warning
when hardware takes a longer than usual.
There has already been an attempt to fix this with commit
9b45a7738eec ("iommu/amd: Fix loop timeout issue in iommu_ga_log_enable()")
But that commit was based on some wrong math and did not fix the issue
in all cases.

Lack the list_del in the mtk_iommu_remove, and remove
bus_set_iommu(*, NULL) since there may be several iommu HWs.
we can not bus_set_iommu null when one iommu driver unbind.
This could be a fix for mt2712 which support 2 M4U HW and list them.

The VT-d spec requires (10.4.4 Global Command Register, TE
field) that:
Hardware implementations supporting DMA draining must drain
any in-flight DMA read/write requests queued within the
Root-Complex before completing the translation enable
command and reflecting the status of the command through
the TES field in the Global Status register.
Unfortunately, some integrated graphic devices fail to do
so after some kind of power state transition. As the
result, the system might stuck in iommu_disable_translati
on(), waiting for the completion of TE transition.
This adds RPLS to a quirk list for those devices and skips
TE disabling if the qurik hits.

Calculate the appropriate mask for non-size-aligned page selective
invalidation. Since psi uses the mask value to mask out the lower order
bits of the target address, properly flushing the iotlb requires using a
mask value such that [pfn, pfn+pages) all lie within the flushed
size-aligned region. This is not normally an issue because iova.c
always allocates iovas that are aligned to their size. However, iovas
which come from other sources (e.g. userspace via VFIO) may not be
aligned.
To properly flush the IOTLB, both the start and end pfns need to be
equal after applying the mask. That means that the most efficient mask
to use is the index of the lowest bit that is equal where all higher
bits are also equal. For example, if pfn=0x17f and pages=3, then
end_pfn=0x181, so the smallest mask we can use is 8. Any differences
above the highest bit of pages are due to carrying, so by xnor'ing pfn
and end_pfn and then masking out the lower order bits based on pages, we
get 0xffffff00, where the first set bit is the mask we want to use.

During event processing, events are read from the event queue one
by one until the queue is empty.If the master device continuously
requests address access at the same time and the SMMU generates
events, the cyclic processing of the event takes a long time and

For various reasons based on the allocator behaviour and typical
use-cases at the time, when the max32_alloc_size optimisation was
introduced it seemed reasonable to couple the reset of the tracked
size to the update of cached32_node upon freeing a relevant IOVA.
However, since subsequent optimisations focused on helping genuine
32-bit devices make best use of even more limited address spaces, it
is now a lot more likely for cached32_node to be anywhere in a "full"
32-bit address space, and as such more likely for space to become
available from IOVAs below that node being freed.
At this point, the short-cut in __cached_rbnode_delete_update() really
doesn't hold up any more, and we need to fix the logic to reliably
provide the expected behaviour. We still want cached32_node to only move
upwards, but we should reset the allocation size if *any* 32-bit space
has become available.

The AMD IOMMU logs I/O page faults and such to a ring buffer in
system memory, and this ring buffer can overflow. The AMD IOMMU
spec has the following to say about the interrupt status bit that
signals this overflow condition:
EventOverflow: Event log overflow. RW1C. Reset 0b. 1 = IOMMU
event log overflow has occurred. This bit is set when a new
event is to be written to the event log and there is no usable
entry in the event log, causing the new event information to
be discarded. An interrupt is generated when EventOverflow = 1b
and MMIO Offset 0018h[EventIntEn] = 1b. No new event log
entries are written while this bit is set. Software Note: To
resume logging, clear EventOverflow (W1C), and write a 1 to
MMIO Offset 0018h[EventLogEn].
The AMD IOMMU driver doesn't currently implement this recovery
sequence, meaning that if a ring buffer overflow occurs, logging
of EVT/PPR/GA events will cease entirely.
This patch implements the spec-mandated reset sequence, with the
minor tweak that the hardware seems to want to have a 0 written to
MMIO Offset 0018h[EventLogEn] first, before writing an 1 into this
field, or the IOMMU won't actually resume logging events.

The polling loop for the register change in iommu_ga_log_enable() needs
to have a udelay() in it. Otherwise the CPU might be faster than the
IOMMU hardware and wrongly trigger the WARN_ON() further down the code
stream. Use a 10us for udelay(), has there is some hardware where
activation of the GA log can take more than a 100ms.
A future optimization should move the activation check of the GA log
to the point where it gets used for the first time. But that is a
bigger change and not suitable for a fix.

After commit e3beca48a45b ("irqdomain/treewide: Keep firmware node
unconditionally allocated"). For tear down scenario, fn is only freed
after fail to allocate ir_domain, though it also should be freed in case
dmar_enable_qi returns error.
Besides free fn, irq_domain and ir_msi_domain need to be removed as well
if intel_setup_irq_remapping fails to enable queued invalidation.
Improve the rewinding path by add out_free_ir_domain and out_free_fwnode
lables per Baolu's suggestion.

It turns out to be possible for hotplugging out a device to reach the
stage of tearing down the device's group and default domain before the
domain's flush queue has drained naturally. At this point, it is then
possible for the timeout to expire just before the del_timer() call
in free_iova_flush_queue(), such that we then proceed to free the FQ
resources while fq_flush_timeout() is still accessing them on another
CPU. Crashes due to this have been observed in the wild while removing
NVMe devices.
Close the race window by using del_timer_sync() to safely wait for any
active timeout handler to finish before we start to free things. We
already avoid any locking in free_iova_flush_queue() since the FQ is
supposed to be inactive anyway, so the potential deadlock scenario does
not apply.
