To fix the issue, driver needs to clear the MBX_INTR_WAIT flag on purging
the mailbox. When the stale mailbox completion does arrive, it will be

A direct attach tape device, when gets swapped with another, was not
discovered. Fix this by looking at loop map and reinitialize link if there
are devices present.

Aborting commands that have already been sent to the firmware can
cause BUG in qlt_free_cmd(): BUG_ON(cmd->sg_mapped)
For instance:
- Command passes rdx_to_xfer state, maps sgl, sends to the firmware
- Reset occurs, qla2xxx performs ISP error recovery, aborts the command
- Target stack calls qlt_abort_cmd() and then qlt_free_cmd()
- BUG_ON(cmd->sg_mapped) in qlt_free_cmd() occurs because sgl was not
unmapped
Thus, unmap sgl in qlt_abort_cmd() for commands with the aborted flag set.

While a session is in the middle of a relogin, a late RSCN can be delivered
from switch. RSCN trigger fabric scan where the scan logic can trigger
another session login while a login is in progress. Reduce the extra
trigger to prevent multiple logins to the same session.

For N2N topology, ELS Passthrough is used to send PLOGI. On failure of ELS
pass through PLOGI, driver flipped over to using LLIOCB PLOGI for N2N. This
is not consistent. Delete the session to restart the connection where ELS
pass through PLOGI would be used consistently.

User experienced device lost. The log shows Get port data base command was
queued up, failed, and requeued again. Every time it is requeued, it set
the FCF_ASYNC_ACTIVE. This prevents any recovery code from occurring
because driver thinks a recovery is in progress for this session. In
essence, this session is hung. The reason it gets into this place is the
session deletion got in front of this call due to link perturbation.
Break the requeue cycle and exit. The session deletion code will trigger a
session relogin.

User experienced no task management error while target device is responding
with error. The RSP_CODE field in the status IOCB is in little endian.
Driver assumes it's big endian and it picked up erroneous data.
Convert the data back to big endian as is on the wire.

User experienced some of the LUN failed to get rediscovered after long
cable pull test. The issue is triggered by a race condition between driver
setting session online state vs starting the LUN scan process at the same
time. Current code set the online state after notifying the session is
available. In this case, trigger to start the LUN scan process happened
before driver could set the session in online state. LUN scan ends up with
failure due to the session online check was failing.
Set the online state before reporting of the availability of the session.

A device logout in loop topology initiates a device connection teardown
which loses the FW device handle. In loop topo, the device handle is not
regrabbed leading to device login failures and eventually to loss of the
device. Fix this by taking the main login path that does it.

Fix stuck sessions in get port database. When a thread is in the process of
re-establishing a session, a flag is set to prevent multiple threads /
triggers from doing the same task. This flag was left on, where any attempt
to relogin was locked out. Clear this flag, if the attempt has failed.

When user uses issue_lip to do link bounce, driver sends additional target
reset to remote device before resetting the link. The target reset would
affect other paths with active I/Os. This patch will remove the unnecessary
target reset.

The sgl may be left unmapped in error cases of response sending. For
instance, qlt_rdy_to_xfer() maps sgl and exits when session is being
deleted keeping the sgl mapped.
This patch removes use-after-free of the sgl and ensures that the sgl is
unmapped for any command that was not sent to firmware.

Target de-configuration panics at high CPU load because TPGT and WWPN can
be removed on separate threads.
TPGT removal requests a reset HBA on a separate thread and waits for reset
complete (phase1). Due to high CPU load that HBA reset can be delayed for
some time.
WWPN removal does qlt_stop_phase2(). There it is believed that phase1 has
already completed and thus tgt.tgt_ops is subsequently cleared. However,
tgt.tgt_ops is needed to process incoming traffic and therefore this will
cause one of the following panics:

On timeout, SRB pointer was cleared from outstanding command array and
dropped. It was not allowed to go through the done process and cleanup.
This patch will abort the SRB where FW will return it with an error status
and resume the normal cleanup.

Session is stuck if driver sees FW has received a PRLI. Driver allows FW to
finish with processing of PRLI by checking back with FW at a later time to
see if the PRLI has finished. Instead, driver failed to push forward after
re-checking PRLI completion.

Current code will send PRLI with FC-NVMe bit set for the targets which
support only FCP. This may result into issue with targets which do not
understand NVMe and will go into a strange state. This patch would restart
the login process by going back to PLOGI state. The PLOGI state will force
the target to respond to correct PRLI request.

Some storage arrays advertise FCP LUNs and NVMe namespaces behind the same
WWN. The driver now offers a user option by way of NVRAM parameter to
allow users to choose, on a per port basis, the kind of FC-4 type they
would like to prioritize for login.

Calling vha->hw->tgt.tgt_ops->free_cmd() from qlt_xmit_response() is wrong
since the command for which a response is sent must remain valid until the
SCSI target core calls .release_cmd(). It has been observed that the
following scenario triggers a kernel crash:
- qlt_xmit_response() calls qlt_check_reserve_free_req()
- qlt_check_reserve_free_req() returns -EAGAIN
- qlt_xmit_response() calls vha->hw->tgt.tgt_ops->free_cmd(cmd)
- transport_handle_queue_full() tries to retransmit the response
Fix this crash by reverting the patch that introduced it.

Emit a warning when ->done or ->free are called on an already freed
srb. There is a hidden use-after-free bug in the driver which corrupts
the srb memory pool which originates from the cleanup callbacks.
An extensive search didn't bring any lights on the real problem. The
initial fix was to set both pointers to NULL and try to catch invalid
accesses. But instead the memory corruption was gone and the driver
didn't crash. Since not all calling places check for NULL pointer, add
explicitly default handlers. With this we workaround the memory
corruption and add a debug help.

In the case of a failed retry, a positive value EIO is returned here. I
think this is a typo error. It is necessary to return an error value.

Current code will send PRLI with FC-NVMe bit set for the targets which
support only FCP. This may result into issue with targets which do not
understand NVMe and will go into a strange state. This patch would restart
the login process by going back to PLOGI state. The PLOGI state will force
the target to respond to correct PRLI request.

Fix race condition between GNL completion processing and GNL request. Late
submission of GNL request was not seen by the GNL completion thread. This
patch will re-submit the GNL request for late submission fcport.
