784ef618b2 remove the sx8 block driver
commit d13bc4d84a8e91060d3797fc95c1a0202bfd1499 upstream.

This driver is for fairly obscure hardware, and has only seen random
drive-by changes after the maintainer stopped working on it in 2005
(about a year and a half after it was introduced).  It has some
"interesting" block layer interactions, so let's just drop it unless
anyone complains.

6e37de4a14 rbd: take header_rwsem in rbd_dev_refresh() only when updating
commit 0b207d02bd9ab8dcc31b262ca9f60dbc1822500d upstream.

rbd_dev_refresh() has been holding header_rwsem across header and
parent info read-in unnecessarily for ages.  With commit 870611e4877e
("rbd: get snapshot context after exclusive lock is ensured to be
held"), the potential for deadlocks became much more real owning to
a) header_rwsem now nesting inside lock_rwsem and b) rw_semaphores
not allowing new readers after a writer is registered.

For example, assuming that I/O request 1, I/O request 2 and header
read-in request all target the same OSD:

1. I/O request 1 comes in and gets submitted
2. watch error occurs
3. rbd_watch_errcb() takes lock_rwsem for write, clears owner_cid and
   releases lock_rwsem
4. after reestablishing the watch, rbd_reregister_watch() calls
   rbd_dev_refresh() which takes header_rwsem for write and submits
   a header read-in request
5. I/O request 2 comes in: after taking lock_rwsem for read in
   __rbd_img_handle_request(), it blocks trying to take header_rwsem
   for read in rbd_img_object_requests()
6. another watch error occurs
7. rbd_watch_errcb() blocks trying to take lock_rwsem for write
8. I/O request 1 completion is received by the messenger but can't be
   processed because lock_rwsem won't be granted anymore
9. header read-in request completion can't be received, let alone
   processed, because the messenger is stranded

Change rbd_dev_refresh() to take header_rwsem only for actually
updating rbd_dev->header.  Header and parent info read-in don't need
any locking.

bc2a304401 rbd: decouple parent info read-in from updating rbd_dev

Unlike header read-in, parent info read-in is already decoupled in
get_parent_info(), but it's buried in rbd_dev_v2_parent_info() along
with the processing logic.

Separate the initial read-in and update read-in logic into
rbd_dev_setup_parent() and rbd_dev_update_parent() respectively and
have rbd_dev_v2_parent_info() just populate struct parent_image_info
(i.e. what get_parent_info() did).  Some existing QoI issues, like
flatten of a standalone clone being disregarded on refresh, remain.

2e0114edeb rbd: decouple header read-in from updating rbd_dev->header
commit 510a7330c82a7754d5df0117a8589e8a539067c7 upstream.

Make rbd_dev_header_info() populate a passed struct rbd_image_header
instead of rbd_dev->header and introduce rbd_dev_update_header() for
updating mutable fields in rbd_dev->header upon refresh.  The initial
read-in of both mutable and immutable fields in rbd_dev_image_probe()
passes in rbd_dev->header so no update step is required there.

rbd_init_layout() is now called directly from rbd_dev_image_probe()
instead of individually in format 1 and format 2 implementations.

be4d2b4562 loop: Select I/O scheduler 'none' from inside add_disk()

We noticed that the user interface of Android devices becomes very slow
under memory pressure. This is because Android uses the zram driver on top
of the loop driver for swapping, because under memory pressure the swap
code alternates reads and writes quickly, because mq-deadline is the
default scheduler for loop devices and because mq-deadline delays writes by
five seconds for such a workload with default settings. Fix this by making
the kernel select I/O scheduler 'none' from inside add_disk() for loop
devices. This default can be overridden at any time from user space,
e.g. via a udev rule. This approach has an advantage compared to changing
the I/O scheduler from userspace from 'mq-deadline' into 'none', namely
that synchronize_rcu() does not get called.

This patch changes the default I/O scheduler for loop devices from
'mq-deadline' into 'none'.

Additionally, this patch reduces the Android boot time on my test setup
with 0.5 seconds compared to configuring the loop I/O scheduler from user
space.

the device number will overflow, resulting in failure to create a block
device.

Fix it by imiting the size of the max allocation.

63afd76621 xen/blkfront: Only check REQ_FUA for writes

The existing code silently converts read operations with the
REQ_FUA bit set into write-barrier operations. This results in data
loss as the backend scribbles zeroes over the data instead of returning
it.

While the REQ_FUA bit doesn't make sense on a read operation, at least
one well-known out-of-tree kernel module does set it and since it
results in data loss, let's be safe here and only look at REQ_FUA for
writes.

a35d89d360 rbd: get snapshot context after exclusive lock is ensured to be held

Move capturing the snapshot context into the image request state
machine, after exclusive lock is ensured to be held for the duration of
dealing with the image request.  This is needed to ensure correctness
of fast-diff states (OBJECT_EXISTS vs OBJECT_EXISTS_CLEAN) and object
deltas computed based off of them.  Otherwise the object map that is
forked for the snapshot isn't guaranteed to accurately reflect the
contents of the snapshot when the snapshot is taken under I/O.  This
breaks differential backup and snapshot-based mirroring use cases with
fast-diff enabled: since some object deltas may be incomplete, the
destination image may get corrupted.

0638dcc7e7 treewide: Remove uninitialized_var() usage

Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.

In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

dde34ef0bc drbd: correctly submit flush bio on barrier

When we receive a flush command (or "barrier" in DRBD), we currently use
a REQ_OP_FLUSH with the REQ_PREFLUSH flag set.

The correct way to submit a flush bio is by using a REQ_OP_WRITE without
any data, and set the REQ_PREFLUSH flag.

Since commit b4a6bb3a67aa ("block: add a sanity check for non-write
flush/fua bios"), this triggers a warning in the block layer, but this
has been broken for quite some time before that.

So use the correct set of flags to actually make the flush happen.

9787b328c4 rbd: avoid use-after-free in do_rbd_add() when rbd_dev_create() fails

If getting an ID or setting up a work queue in rbd_dev_create() fails,
use-after-free on rbd_dev->rbd_client, rbd_dev->spec and rbd_dev->opts
is triggered in do_rbd_add().  The root cause is that the ownership of
these structures is transfered to rbd_dev prematurely and they all end
up getting freed when rbd_dev_create() calls rbd_dev_free() prior to
returning to do_rbd_add().

Found by Linux Verification Center (linuxtesting.org) with SVACE, an
incomplete patch submitted by Natalia Petrova <n.petrova@fintech.ru>.

version of the reproducer:

atomic_read(&config->recv_threads) == 0, the task can hung because it
waits the completion of the inflight IOs.

This patch fixes the issue by clearing queue, not just shutdown, when
signal interrupt nbd_start_device_ioctl().

13b2856037 loop: Check for overflow while configuring loop

The userspace can configure a loop using an ioctl call, wherein
a configuration of type loop_config is passed (see lo_ioctl()'s
case on line 1550 of drivers/block/loop.c). This proceeds to call
loop_configure() which in turn calls loop_set_status_from_info()
(see line 1050 of loop.c), passing &config->info which is of type
loop_info64*. This function then sets the appropriate values, like
the offset.

loop_device has lo_offset of type loff_t (see line 52 of loop.c),
which is typdef-chained to long long, whereas loop_info64 has
lo_offset of type __u64 (see line 56 of include/uapi/linux/loop.h).

The function directly copies offset from info to the device as
follows (See line 980 of loop.c):
	lo->lo_offset = info->lo_offset;

This results in an overflow, which triggers a warning in iomap_iter()
due to a call to iomap_iter_done() which has:
	WARN_ON_ONCE(iter->iomap.offset > iter->pos);

Thus, check for negative value during loop_set_status_from_info().

60ac50daad xen/blkfront: force data bouncing when backend is untrusted

Split the current bounce buffering logic used with persistent grants
into it's own option, and allow enabling it independently of
persistent grants.  This allows to reuse the same code paths to
perform the bounce buffering required to avoid leaking contiguous data
in shared pages not part of the request fragments.

Reporting whether the backend is to be trusted can be done using a
module parameter, or from the xenstore frontend path as set by the
toolstack when adding the device.

This is CVE-2022-33742, part of XSA-403.

"qemu-ndb -d" will call ioctl 'NBD_DISCONNECT' first, however, following
message was found:

block nbd0: Send disconnect failed -32

Which indicate that something is wrong with the server. Then,
"qemu-nbd -d" will call ioctl 'NBD_CLEAR_SOCK', however ioctl can't clear
requests after commit 2516ab1543fd("nbd: only clear the queue on device
teardown"). And in the meantime, request can't complete through timeout
because nbd_xmit_timeout() will always return 'BLK_EH_RESET_TIMER', which
means such request will never be completed in this situation.

Now that the flag 'NBD_CMD_INFLIGHT' can make sure requests won't
complete multiple times, switch back to call nbd_clear_sock() in
nbd_clear_sock_ioctl(), so that inflight requests can be cleared.

When nbd module is being removing, nbd_alloc_config() may be
called concurrently by nbd_genl_connect(), although try_module_get()
will return false, but nbd_alloc_config() doesn't handle it.

The race may lead to the leak of nbd_config and its related
resources (e.g, recv_workq) and oops in nbd_read_stat() due
to the unload of nbd module as shown below:

Fixing it by checking the return value of try_module_get()
in nbd_alloc_config(). As nbd_alloc_config() may return ERR_PTR(-ENODEV),
assign nbd->config only when nbd_alloc_config() succeeds to ensure
the value of nbd->config is binary (valid or NULL).

Also adding a debug message to check the reference counter
of nbd_config during module removal.

ee3901d7c7 virtio_blk: fix the discard_granularity and discard_alignment queue limits

The discard_alignment queue limit is named a bit misleading means the
offset into the block device at which the discard granularity starts.

On the other hand the discard_sector_alignment from the virtio 1.1 looks
similar to what Linux uses as discard granularity (even if not very well
described):

  "discard_sector_alignment can be used by OS when splitting a request
   based on alignment. "

And at least qemu does set it to the discard granularity.

So stop setting the discard_alignment and use the virtio
discard_sector_alignment to set the discard granularity.

9d1764b926 nbd: Fix hung on disconnect request if socket is closed before

When userspace closes the socket before sending a disconnect
request, the following I/O requests will be blocked in
wait_for_reconnect() until dead timeout. This will cause the
following disconnect request also hung on blk_mq_quiesce_queue().
That means we have no way to disconnect a nbd device if there
are some I/O requests waiting for reconnecting until dead timeout.
It's not expected. So let's wake up the thread waiting for
reconnecting directly when a disconnect request is sent.

In preparation to limit the scope of a list iterator to the list
traversal loop, use a dedicated pointer to iterate through the list [1].

Since that variable should not be used past the loop iteration, a
separate variable is used to 'remember the current location within the
loop'.

To either continue iterating from that position or skip the iteration
(if the previous iteration was complete) list_prepare_entry() is used.

67e2b62461 floppy: use a statically allocated error counter

Interrupt handler bad_flp_intr() may cause a UAF on the recently freed
request just to increment the error count.  There's no point keeping
that one in the request anyway, and since the interrupt handler uses a
static pointer to the error which cannot be kept in sync with the
pending request, better make it use a static error counter that's reset
for each new request.  This reset now happens when entering
redo_fd_request() for a new request via set_next_request().

One initial concern about a single error counter was that errors on one
floppy drive could be reported on another one, but this problem is not
real given that the driver uses a single drive at a time, as that
PC-compatible controllers also have this limitation by using shared
signals.  As such the error count is always for the "current" drive.

7dea591300 floppy: disable FDRAWCMD by default

Minh Yuan reported a concurrency use-after-free issue in the floppy code
between raw_cmd_ioctl and seek_interrupt.

[ It turns out this has been around, and that others have reported the
  KASAN splats over the years, but Minh Yuan had a reproducer for it and
  so gets primary credit for reporting it for this fix   - Linus ]

The problem is, this driver tends to break very easily and nowadays,
nobody is expected to use FDRAWCMD anyway since it was used to
manipulate non-standard formats.  The risk of breaking the driver is
higher than the risk presented by this race, and accessing the device
requires privileges anyway.

Let's just add a config option to completely disable this ioctl and
leave it disabled by default.  Distros shouldn't use it, and only those
running on antique hardware might need to enable it.

b6a4055036 drbd: Fix five use after free bugs in get_initial_state

In get_initial_state, it calls notify_initial_state_done(skb,..) if
cb->args[5]==1. If genlmsg_put() failed in notify_initial_state_done(),
the skb will be freed by nlmsg_free(skb).
Then get_initial_state will goto out and the freed skb will be used by
return value skb->len, which is a uaf bug.

What's worse, the same problem goes even further: skb can also be
freed in the notify_*_state_change -> notify_*_state calls below.
Thus 4 additional uaf bugs happened.

My patch lets the problem callee functions: notify_initial_state_done
and notify_*_state_change return an error code if errors happen.
So that the error codes could be propagated and the uaf bugs can be avoid.

v2 reports a compilation warning. This v3 fixed this warning and built
successfully in my local environment with no additional warnings.

sprintf does not know the PAGE_SIZE maximum of the temporary buffer
used for outputting sysfs content and it's possible to overrun the
PAGE_SIZE buffer length.

Use a generic sysfs_emit function that knows the size of the
temporary buffer and ensures that no overrun is done for offset
attribute in
loop_attr_[offset|sizelimit|autoclear|partscan|dio]_show() callbacks.

bio chain generated by blk_queue_split().
Some split bio fails and propagates its error status to the "parent" bio.
But then the (last part of the) parent bio itself completes without error.

We would clobber the already recorded error status with BLK_STS_OK,
causing silent data corruption.

Reproducer:
-----------

How to trigger this in the real world within seconds:

DRBD on top of degraded parity raid,
small stripe_cache_size, large read_ahead setting.
Drop page cache (sysctl vm.drop_caches=1, fadvise "DONTNEED",
umount and mount again, "reboot").

Cause significant read ahead.

Large read ahead request is split by blk_queue_split().
Parts of the read ahead that are already in the stripe cache,
or find an available stripe cache to use, can be serviced.
Parts of the read ahead that would need "too much work",
would need to wait for a "stripe_head" to become available,
are rejected immediately.

For larger read ahead requests that are split in many pieces, it is very
likely that some "splits" will be serviced, but then the stripe cache is
exhausted/busy, and the remaining ones will be rejected.

a83400456f xen/blkfront: don't use gnttab_query_foreign_access() for mapped status

It isn't enough to check whether a grant is still being in use by
calling gnttab_query_foreign_access(), as a mapping could be realized
by the other side just after having called that function.

In case the call was done in preparation of revoking a grant it is
better to do so via gnttab_end_foreign_access_ref() and check the
success of that operation instead.

For the ring allocation use alloc_pages_exact() in order to avoid
high order pages in case of a multi-page ring.

If a grant wasn't unmapped by the backend without persistent grants
being used, set the device state to "error".

This is CVE-2022-23036 / part of XSA-396.

We need to check the max request size that is from user space before
allocating pages. If the request size exceeds the limit, return -EINVAL.

84e568531b floppy: Fix hang in watchdog when disk is ejected

When the watchdog detects a disk change, it calls cancel_activity(),
which in turn tries to cancel the fd_timer delayed work.

In the above scenario, fd_timer_fn is set to fd_watchdog(), meaning
it is trying to cancel its own work.
This results in a hang as cancel_delayed_work_sync() is waiting for the
watchdog (itself) to return, which never happens.

This can be reproduced relatively consistently by attempting to read a
broken floppy, and ejecting it while IO is being attempted and retried.

To resolve this, this patch calls cancel_delayed_work() instead, which
cancels the work without waiting for the watchdog to return and finish.

Before this regression was introduced, the code in this section used
del_timer(), and not del_timer_sync() to delete the watchdog timer.

3456a07614 xen/blkfront: don't trust the backend response data blindly
commit b94e4b147fd1992ad450e1fea1fdaa3738753373 upstream.

Today blkfront will trust the backend to send only sane response data.
In order to avoid privilege escalations or crashes in case of malicious
backends verify the data to be within expected limits. Especially make
sure that the response always references an outstanding request.

Introduce a new state of the ring BLKIF_STATE_ERROR which will be
switched to in case an inconsistency is being detected. Recovering from
this state is possible only via removing and adding the virtual device
again (e.g. via a suspend/resume cycle).

Make all warning messages issued due to valid error responses rate
limited in order to avoid message floods being triggered by a malicious
backend.

In order to avoid a malicious backend being able to influence the local
copy of a request build the request locally first and then copy it to
the ring page instead of doing it the other way round as today.

23def86bbf zram: off by one in read_block_state()

snprintf() returns the number of bytes it would have printed if there
were space.  But it does not count the NUL terminator.  So that means
that if "count == copied" then this has already overflowed by one
character.

This bug likely isn't super harmful in real life.

3446e5ecdf block: ataflop: fix breakage introduced at blk-mq refactoring

Refactoring of the Atari floppy driver when converting to blk-mq
has broken the state machine in not-so-subtle ways:

finish_fdc() must be called when operations on the floppy device
have completed. This is crucial in order to relase the ST-DMA
lock, which protects against concurrent access to the ST-DMA
controller by other drivers (some DMA related, most just related
to device register access - broken beyond compare, I know).

When rewriting the driver's old do_request() function, the fact
that finish_fdc() was called only when all queued requests had
completed appears to have been overlooked. Instead, the new
request function calls finish_fdc() immediately after the last
request has been queued. finish_fdc() executes a dummy seek after
most requests, and this overwrites the state machine's interrupt
hander that was set up to wait for completion of the read/write
request just prior. To make matters worse, finish_fdc() is called
before device interrupts are re-enabled, making certain that the
read/write interupt is missed.

Shifting the finish_fdc() call into the read/write request
completion handler ensures the driver waits for the request to
actually complete. With a queue depth of 2, we won't see long
request sequences, so calling finish_fdc() unconditionally just
adds a little overhead for the dummy seeks, and keeps the code
simple.

While we're at it, kill ataflop_commit_rqs() which does nothing
but run finish_fdc() unconditionally, again likely wiping out an
in-flight request.

b3fa499d72 block: nbd: add sanity check for first_minor

Syzbot hit WARNING in internal_create_group(). The problem was in
too big disk->first_minor.

disk->first_minor is initialized by value, which comes from userspace
and there wasn't any sanity checks about value correctness. It can cause
duplicate creation of sysfs files/links, because disk->first_minor will
be passed to MKDEV() which causes truncation to byte. Since maximum
minor value is 0xff, let's check if first_minor is correct minor number.

NOTE: the root case of the reported warning was in wrong error handling
in register_disk(), but we can avoid passing knowingly wrong values to
sysfs API, because sysfs error messages can confuse users. For example:
user passed 1048576 as index, but sysfs complains about duplicate
creation of /dev/block/43:0. It's not obvious how 1048576 becomes 0.

ebad44b643 cryptoloop: add a deprecation warning

Support for cryptoloop has been officially marked broken and deprecated
in favor of dm-crypt (which supports the same broken algorithms if
needed) in Linux 2.6.4 (released in March 2004), and support for it has
been entirely removed from losetup in util-linux 2.23 (released in April
2013).  Add a warning and a deprecation schedule.

67871ada3a Revert "floppy: reintroduce O_NDELAY fix"

The patch breaks userspace implementations (e.g. fdutils) and introduces
regressions in behaviour. Previously, it was possible to O_NDELAY open a
floppy device with no media inserted or with write protected media without
an error. Some userspace tools use this particular behavior for probing.

It's not the first time when we revert this patch. Previous revert is in
commit f2791e7eadf4 (Revert "floppy: refactor open() flags handling").

This reverts commit 8a0c014cd20516ade9654fc13b51345ec58e7be8.

nbd_clear_que() and completing requests in recv_work(),
