Typically when an out of resource CQE status is detected, the
lpfc_ramp_down_queue_handler() logic is called to help reduce I/O load by
reducing an sdev's queue_depth.
However, the current lpfc_rampdown_queue_depth() logic does not help reduce
queue_depth. num_cmd_success is never updated and is always zero, which
means new_queue_depth will always be set to sdev->queue_depth. So,
new_queue_depth = sdev->queue_depth - new_queue_depth always sets
new_queue_depth to zero. And, scsi_change_queue_depth(sdev, 0) is
essentially a no-op.
Change the lpfc_ramp_down_queue_handler() logic to set new_queue_depth
equal to sdev->queue_depth subtracted from number of times num_rsrc_err was
incremented. If num_rsrc_err is >= sdev->queue_depth, then set
new_queue_depth equal to 1. Eventually, the frequency of Good_Status
frames will signal SCSI upper layer to auto increase the queue_depth back
to the driver default of 64 via scsi_handle_queue_ramp_up().

The call to lpfc_sli4_resume_rpi() in lpfc_rcv_padisc() may return an
unsuccessful status. In such cases, the elsiocb is not issued, the
completion is not called, and thus the elsiocb resource is leaked.
Check return value after calling lpfc_sli4_resume_rpi() and conditionally
release the elsiocb resource.

The myrb and myrs drivers use an odd way of implementing their sysfs files,
calling snprintf() with a fixed length of 32 bytes to print into a page
sized buffer. One of the strings is actually longer than 32 bytes, which
clang can warn about:

These could all be plain sprintf() without a length as the buffer is always
long enough. On the other hand, sysfs files should not be overly long
either, so just double the length to make sure the longest strings don't
get truncated here.

Commit fc663711b944 ("scsi: core: Remove the /proc/scsi/${proc_name}
directory earlier") fixed a bug related to modules loading/unloading, by
adding a call to scsi_proc_hostdir_rm() on scsi_remove_host(). But that led
to a potential duplicate call to the hostdir_rm() routine, since it's also
called from scsi_host_dev_release(). That triggered a regression report,
which was then fixed by commit be03df3d4bfe ("scsi: core: Fix a procfs host
directory removal regression"). The fix just dropped the hostdir_rm() call
from dev_release().
But it happens that this proc directory is created on scsi_host_alloc(),
and that function "pairs" with scsi_host_dev_release(), while
scsi_remove_host() pairs with scsi_add_host(). In other words, it seems the
reason for removing the proc directory on dev_release() was meant to cover
cases in which a SCSI host structure was allocated, but the call to
scsi_add_host() didn't happen. And that pattern happens to exist in some
error paths, for example.
Syzkaller causes that by using USB raw gadget device, error'ing on
usb-storage driver, at usb_stor_probe2(). By checking that path, we can see
that the BadDevice label leads to a scsi_host_put() after a SCSI host
allocation, but there's no call to scsi_add_host() in such path. That leads
to messages like this in dmesg (and a leak of the SCSI host proc
structure):

The proper fix seems to still call scsi_proc_hostdir_rm() on dev_release(),
but guard that with the state check for SHOST_CREATED; there is even a
comment in scsi_host_dev_release() detailing that: such conditional is
meant for cases where the SCSI host was allocated but there was no calls to
{add,remove}_host(), like the usb-storage case.
This is what we propose here and with that, the error path of usb-storage
does not trigger the warning anymore.

Some callback functions used here take a boolean argument, others take a
status argument. This breaks KCFI type checking, so clang now warns about
the function pointer cast:

If the driver detects that the controller is not ready before sending the
first IOC facts command, it will wait for a maximum of 10 seconds for it to
become ready. However, even if the controller becomes ready within 10
seconds, the driver will still issue a diagnostic reset.
Modify the driver to avoid sending a diag reset if the controller becomes
ready within the 10-second wait time.

JAZZ_ESP is a bool kconfig symbol that selects SCSI_SPI_ATTRS. When
CONFIG_SCSI=m, this results in SCSI_SPI_ATTRS=m while JAZZ_ESP=y, which
causes many undefined symbol linker errors.
Fix this by only offering to build this driver when CONFIG_SCSI=y.

LUNs going into "failed ready running" state observed on >1T and on even
numbers of size (2T, 4T, 6T, 8T and 10T). The issue occurs when DIF is
enabled at the host.
The kernel logs:
Cannot setup S/G List for HBAIO segs 1/1 SGL 512 SCSI 256: 3 0
The host lpfc driver is failing to setup scatter/gather list (protection
data) for the I/Os.
The return type lpfc_bg_setup_sgl()/lpfc_bg_setup_sgl_prot() causes the
compiler to remove the most significant bit. Use an unsigned type instead.

The current FC error recovery is sending up to three REC (recovery) frames
in 10 second intervals, and as a final step sending an ABTS after 30
seconds for the command itself. Unfortunately sending an ABTS is also the
action for the SCSI abort handler, and the default timeout for SCSI
commands is also 30 seconds. This causes two ABTS to be scheduled, with the
libfc one slightly earlier. The ABTS scheduled by SCSI EH then sees the
command to be already aborted, and will always return with a 'GOOD' status
irrespective on the actual result from the first ABTS. This causes the
SCSI EH abort handler to always succeed, and SCSI EH never to be engaged.
Fix this by not issuing an ABTS when a SCSI command is present for the
exchange, but rather wait for the abort scheduled from SCSI EH. And warn
if an abort is already scheduled to avoid similar errors in the future.

Because file_name and phba->ModelName are both declared a size 80 bytes,
the extra ".grp" file extension could cause an overflow into file_name.
Define a ELX_FW_NAME_SIZE macro with value 84. 84 incorporates the 4 extra
characters from ".grp". file_name is changed to be declared as a char and
initialized to zeros i.e. null chars.

In fnic_init_module() exists redundant check for return value from

skb_share_check() already drops the reference to the skb when returning
NULL. Using kfree_skb() in the error handling path leads to an skb double
free.
Fix this by removing the variable tmp_skb, and return directly when
skb_share_check() returns NULL.

Since the SCSI core does not reuse the tag of the SCSI command that is
being aborted by .eh_abort() before .eh_abort() has finished it is not
necessary to check from inside that callback whether or not the SCSI
command has already completed. Instead, rely on the firmware to return an
error code when attempting to abort a command that has already
completed. Additionally, rely on the firmware to return an error code when
attempting to abort an already aborted command.
In qla2x00_abort_srb(), use blk_mq_request_started() instead of
sp->completed and sp->aborted.

In BMC environments with concurrent access to multiple registers, certain
registers occasionally yield a value of 0 even after 3 retries due to
hardware errata. As a fix, we have extended the retry count from 3 to 30.
The same errata applies to the mpt3sas driver, and a similar patch has

fc_lport_ptp_setup() did not check the return value of fc_rport_create()
which can return NULL and would cause a NULL pointer dereference. Address
this issue by checking return value of fc_rport_create() and log error
message on fc_rport_create() failed.

utf16s_to_utf8s does not NULL terminate the output string. For us to be
able to add a NULL character when utf16s_to_utf8s returns, we need to make
sure that there is space for such NULL character at the end of the output
buffer. We can achieve this by passing an output buffer size to
utf16s_to_utf8s that is one character less than what we allocated.
Other call sites of utf16s_to_utf8s appear to be using the same technique
where they artificially reduce the buffer size by one to leave space for a
NULL character or line feed character.

System crash when using debug kernel due to link list corruption. The cause
of the link list corruption is due to session deletion was allowed to queue
up twice. Here's the internal trace that show the same port was allowed to
double queue for deletion on different cpu.

The lock is used to synchronize different sysfs operations, it doesn't
protect any resource that will be touched by an interrupt. Consequently
it's not required to disable IRQs. Replace the spinlock with a mutex to fix
the deadlock.

User accidently passed module parameter ql2xenabledif=1 which is
unsupported. However, driver still initialized which lead to guard tag
errors during device discovery.
Remove unsupported ql2xenabledif=1 option and validate the user input.

Link up failure occurred where driver failed to see certain events from FW
indicating link up (AEN 8011) and fabric login completion (AEN 8014).
Without these 2 events, driver would not proceed forward to scan the
fabric. The cause of this is due to delay in the receive of interrupt for
Mailbox 60 that causes qla to set the fw_started flag late. The late
setting of this flag causes other interrupts to be dropped. These dropped
interrupts happen to be the link up (AEN 8011) and fabric login completion
(AEN 8014).
Set fw_started flag early to prevent interrupts being dropped.

Different behavior were experienced of session being torn down vs not when
TMF is timed out. When FW detects the time out, the session is torn down.
When driver detects the time out, the session is not torn down.
Allow TMF error to return to upper layer without session tear down.

There is a long call chain that &fip->ctlr_lock is acquired by isr
fnic_isr_msix_wq_copy() under hard IRQ context. Thus other process context
code acquiring the lock should disable IRQ, otherwise deadlock could happen
if the IRQ preempts the execution while the lock is held in process context

spin_lock_bh() is not enough since fnic_isr_msix_wq_copy() is a
hardirq.
These flaws were found by an experimental static analysis tool I am
developing for irq-related deadlock.
The patch fix the potential deadlocks by spin_lock_irqsave() to disable
hard irq.

The qedf_dbg_debug_cmd_read() function invokes sprintf() directly on a
__user pointer, which may crash the kernel.
Avoid doing that by using a small on-stack buffer for scnprintf() and then
calling simple_read_from_buffer() which does a proper copy_to_user() call.

There are three places that qla4xxx parses nlattrs:
- qla4xxx_set_chap_entry()
- qla4xxx_iface_set_param()
- qla4xxx_sysfs_ddb_set_param()
and each of them directly converts the nlattr to specific pointer of
structure without length checking. This could be dangerous as those
attributes are not validated and a malformed nlattr (e.g., length 0) could
result in an OOB read that leaks heap dirty data.
Add the nla_len check before accessing the nlattr data and return EINVAL if
the length check fails.

beiscsi_iface_set_param() parses nlattr with nla_for_each_attr and assumes
every attributes can be viewed as struct iscsi_iface_param_info.
This is not true because there is no any nla_policy to validate the
attributes passed from the upper function iscsi_set_iface_params().
Add the nla_len check before accessing the nlattr data and return EINVAL if
the length check fails.

The functions iscsi_if_set_param() and iscsi_if_set_host_param() convert an
nlattr payload to type char* and then call C string handling functions like
sscanf and kstrdup:

sscanf(data, "%d", &value);
However, since the nlattr is provided by the user-space program and the
nlmsg skb is allocated with GFP_KERNEL instead of GFP_ZERO flag (see
netlink_alloc_large_skb() in netlink_sendmsg()), dirty data on the heap can
lead to an OOB access for those string handling functions.
By investigating how the bug is introduced, we find it is really
interesting as the old version parsing code starting from commit
fd7255f51a13 ("[SCSI] iscsi: add sysfs attrs for uspace sync up") treated
the nlattr as integer bytes instead of string and had length check in
iscsi_copy_param():

But, since the commit a54a52caad4b ("[SCSI] iscsi: fixup set/get param
functions"), the code treated the nlattr as C string while forgetting to
add any strlen checks(), opening the possibility of an OOB access.
Fix the potential OOB by adding the strlen() check before accessing the
buf. If the data passes this check, all low-level set_param handlers can
safely treat this buf as legal C string.

Hyper-V synthetic SCSI devices do not support the MAINTENANCE_IN SCSI
command, so scsi_report_opcode() always fails, resulting in messages like
this:

The recently added support for command duration limits calls
scsi_report_opcode() four times as each device comes online, which
significantly increases the number of messages logged in a system with many
disks.
Fix the problem by always marking Hyper-V synthetic SCSI devices as not
supporting scsi_report_opcode(). With this setting, the MAINTENANCE_IN SCSI
command is not issued and no messages are logged.

As &qedi_percpu->p_work_lock is acquired by hard IRQ qedi_msix_handler(),
other acquisitions of the same lock under process context should disable
IRQ, otherwise deadlock could happen if the IRQ preempts the execution
while the lock is held in process context on the same CPU.
qedi_cpu_offline() is one such function which acquires the lock in process
context.

This flaw was found by an experimental static analysis tool I am developing
for IRQ-related deadlocks.
The tentative patch fix the potential deadlock by spin_lock_irqsave()

The raid_component_add() function was added to the kernel tree via patch
"[SCSI] embryonic RAID class" (2005). Remove this function since it never
has had any callers in the Linux kernel. And also raid_component_release()
is only used in raid_component_add(), so it is also removed.

Commit 41320b18a0e0 ("scsi: snic: Fix possible memory leak if device_add()
fails") fixed the memory leak caused by dev_set_name() when device_add()
failed. However, it did not consider that 'tgt' has already been released
when put_device(&tgt->dev) is called. Remove kfree(tgt) in the error path
to avoid double free of 'tgt' and move put_device(&tgt->dev) after the
removed kfree(tgt) to avoid a use-after-free.

If device_add() returns error, the name allocated by dev_set_name() needs
be freed. As the comment of device_add() says, put_device() should be used
to decrease the reference count in the error path. So fix this by calling
put_device(), then the name can be freed in kobject_cleanp().
