007a23d38a s390/cio: fix tracepoint subchannel type field
[ Upstream commit 8692a24d0fae19f674d51726d179ad04ba95d958 ]

The subchannel-type field "st" of s390_cio_stsch and s390_cio_msch
tracepoints is incorrectly filled with the subchannel-enabled SCHIB
value "ena". Fix this by assigning the correct value.

10cb803aff s390/qeth: Fix kernel panic after setting hsuid
[ Upstream commit 8a2e4d37afb8500b276e5ee903dee06f50ab0494 ]

Symptom:
When the hsuid attribute is set for the first time on an IQD Layer3
device while the corresponding network interface is already UP,
the kernel will try to execute a napi function pointer that is NULL.

[ 2057.572846]
[ 2057.572847] Kernel panic - not syncing: Fatal exception in interrupt
-------------------------------------------------------------------------------------------

Analysis:
There is one napi structure per out_q: card->qdio.out_qs[i].napi
The napi.poll functions are set during qeth_open().

Since
commit 1cfef80d4c2b ("s390/qeth: Don't call dev_close/dev_open (DOWN/UP)")
qeth_set_offline()/qeth_set_online() no longer call dev_close()/
dev_open(). So if qeth_free_qdio_queues() cleared
card->qdio.out_qs[i].napi.poll while the network interface was UP and the
card was offline, they are not set again.

Reproduction:
chzdev -e $devno layer2=0
ip link set dev $network_interface up
echo 0 > /sys/bus/ccwgroup/devices/0.0.$devno/online
echo foo > /sys/bus/ccwgroup/devices/0.0.$devno/hsuid
echo 1 > /sys/bus/ccwgroup/devices/0.0.$devno/online
-> Crash (can be enforced e.g. by af_iucv connect(), ip link down/up, ...)

Note that a Completion Queue (CQ) is only enabled or disabled, when hsuid
is set for the first time or when it is removed.

Workarounds:
- Set hsuid before setting the device online for the first time
or
- Use chzdev -d $devno; chzdev $devno hsuid=xxx; chzdev -e $devno;
to set hsuid on an existing device. (this will remove and recreate the
network interface)

Fix:
There is no need to free the output queues when a completion queue is
added or removed.
card->qdio.state now indicates whether the inbound buffer pool and the
outbound queues are allocated.
card->qdio.c_q indicates whether a CQ is allocated.

10452edd17 s390/cio: Ensure the copied buf is NUL terminated
[ Upstream commit da7c622cddd4fe36be69ca61e8c42e43cde94784 ]

Currently, we allocate a lbuf-sized kernel buffer and copy lbuf from
userspace to that buffer. Later, we use scanf on this buffer but we don't
ensure that the string is terminated inside the buffer, this can lead to
OOB read when using scanf. Fix this issue by using memdup_user_nul instead.

559f3a6333 s390/cio: fix race condition during online processing
[ Upstream commit 2d8527f2f911fab84aec04df4788c0c23af3df48 ]

A race condition exists in ccw_device_set_online() that can cause the
online process to fail, leaving the affected device in an inconsistent
state. As a result, subsequent attempts to set that device online fail
with return code ENODEV.

The problem occurs when a path verification request arrives after
a wait for final device state completed, but before the result state
is evaluated.

Fix this by ensuring that the CCW-device lock is held between
determining final state and checking result state.

Note that since:

commit 2297791c92d0 ("s390/cio: dont unregister subchannel from child-drivers")

path verification requests are much more likely to occur during boot,
resulting in an increased chance of this race condition occurring.

beb3ff19a5 s390/qdio: handle deferred cc1
[ Upstream commit 607638faf2ff1cede37458111496e7cc6c977f6f ]

A deferred condition code 1 response indicates that I/O was not started
and should be retried. The current QDIO implementation handles a cc1
response as I/O error, resulting in a failed QDIO setup. This can happen
for example when a path verification request arrives at the same time
as QDIO setup I/O is started.

Fix this by retrying the QDIO setup I/O when a cc1 response is received.

Note that since

16307e7bc1 s390/qeth: handle deferred cc1
[ Upstream commit afb373ff3f54c9d909efc7f810dc80a9742807b2 ]

The IO subsystem expects a driver to retry a ccw_device_start, when the
subsequent interrupt response block (irb) contains a deferred
condition code 1.

Symptoms before this commit:
On the read channel we always trigger the next read anyhow, so no
different behaviour here.
On the write channel we may experience timeout errors, because the
expected reply will never be received without the retry.
Other callers of qeth_send_control_data() may wrongly assume that the ccw
was successful, which may cause problems later.

Note that since
commit 2297791c92d0 ("s390/cio: dont unregister subchannel from child-drivers")
and
commit 5ef1dc40ffa6 ("s390/cio: fix invalid -EBUSY on ccw_device_start")
deferred CC1s are much more likely to occur. See the commit message of the
latter for more background information.

handling of the zcrypt card object which could lead to a free

The fix is simple: Before use of the queue not only the queue object

not only the queue but also the card object's reference count is
decreased with zcrypt_card_put().

ad999aa181 s390/dasd: fix double module refcount decrement

977bb962a1 s390/dasd: Use dev_*() for device log messages
[ Upstream commit 79ae56fc475869d636071f66d9e4ef2a3819eee6 ]

All log messages in dasd.c use the printk variants of pr_*(). They all
add the name of the affected device manually to the log message.
This can be simplified by using the dev_*() variants of printk, which
include the device information and make a separate call to dev_name()
unnecessary.

The KMSG_COMPONENT and the pr_fmt() definition can be dropped. Note that
this removes the "dasd: " prefix from the one pr_info() call in
dasd_init(). However, the log message already provides all relevant
information.

Quiesce and resume are functions that tell Linux to stop/resume
issuing I/Os to a specific DASD.
The DASD driver allows a manual quiesce/resume via ioctl.

Autoquiesce will define an amount of triggers that will lead to
an automatic quiesce if a certain event occurs.
There is no automatic resume.

All events will be reported via DASD Extended Error Reporting (EER)

65c5a1ba2c s390/cio: fix invalid -EBUSY on ccw_device_start
commit 5ef1dc40ffa6a6cb968b0fdc43c3a61727a9e950 upstream.

The s390 common I/O layer (CIO) returns an unexpected -EBUSY return code
when drivers try to start I/O while a path-verification (PV) process is
pending. This can lead to failed device initialization attempts with
symptoms like broken network connectivity after boot.

Fix this by replacing the -EBUSY return code with a deferred condition
code 1 reply to make path-verification handling consistent from a
driver's point of view.

The problem can be reproduced semi-regularly using the following process,
while repeating steps 2-3 as necessary (example assumes an OSA device
with bus-IDs 0.0.a000-0.0.a002 on CHPID 0.02):

1. echo 0.0.a000,0.0.a001,0.0.a002 >/sys/bus/ccwgroup/drivers/qeth/group
2. echo 0 > /sys/bus/ccwgroup/devices/0.0.a000/online
3. echo 1 > /sys/bus/ccwgroup/devices/0.0.a000/online ; \
   echo on > /sys/devices/css0/chp0.02/status

Background information:

The common I/O layer starts path-verification I/Os when it receives
indications about changes in a device path's availability. This occurs
for example when hardware events indicate a change in channel-path
status, or when a manual operation such as a CHPID vary or configure
operation is performed.

If a driver attempts to start I/O while a PV is running, CIO reports a
successful I/O start (ccw_device_start() return code 0). Then, after
completion of PV, CIO synthesizes an interrupt response that indicates
an asynchronous status condition that prevented the start of the I/O
(deferred condition code 1).

If a PV indication arrives while a device is busy with driver-owned I/O,
PV is delayed until after I/O completion was reported to the driver's
interrupt handler. To ensure that PV can be started eventually, CIO
reports a device busy condition (ccw_device_start() return code -EBUSY)
if a driver tries to start another I/O while PV is pending.

In some cases this -EBUSY return code causes device drivers to consider
a device not operational, resulting in failed device initialization.

Note: The code that introduced the problem was added in 2003. Symptoms
started appearing with the following CIO commit that causes a PV
indication when a device is removed from the cio_ignore list after the
associated parent subchannel device was probed, but before online
processing of the CCW device has started:

2297791c92d0 ("s390/cio: dont unregister subchannel from child-drivers")

During boot, the cio_ignore list is modified by the cio_ignore dracut
module [1] as well as Linux vendor-specific systemd service scripts[2].
When combined, this commit and boot scripts cause a frequent occurrence
of the problem during boot.

5140c4d5f4 s390/qeth: Fix potential loss of L3-IP@ in case of network issues
commit 2fe8a236436fe40d8d26a1af8d150fc80f04ee1a upstream.

Symptom:
In case of a bad cable connection (e.g. dirty optics) a fast sequence of
network DOWN-UP-DOWN-UP could happen. UP triggers recovery of the qeth
interface. In case of a second DOWN while recovery is still ongoing, it
can happen that the IP@ of a Layer3 qeth interface is lost and will not
be recovered by the second UP.

Problem:
When registration of IP addresses with Layer 3 qeth devices fails, (e.g.
because of bad address format) the respective IP address is deleted from
its hash-table in the driver. If registration fails because of a ENETDOWN
condition, the address should stay in the hashtable, so a subsequent
recovery can restore it.

3caa4af834df ("qeth: keep ip-address after LAN_OFFLINE failure")
fixes this for registration failures during normal operation, but not
during recovery.

Solution:
Keep L3-IP address in case of ENETDOWN in qeth_l3_recover_ip(). For
consistency with qeth_l3_add_ip() we also keep it in case of EADDRINUSE,
i.e. for some reason the card already/still has this address registered.

08f65c9067 s390/vfio-ap: fix sysfs status attribute for AP queue devices
[ Upstream commit a0d8f4eeb7c4ffaee21702bcc91a09b3988c5b7a ]

The 'status' attribute for AP queue devices bound to the vfio_ap device
driver displays incorrect status when the mediated device is attached to a
guest, but the queue device is not passed through. In the current
implementation, the status displayed is 'in_use' which is not correct; it
should be 'assigned'. This can happen if one of the queue devices
associated with a given adapter is not bound to the vfio_ap device driver.
For example:

Queues listed in /sys/bus/ap/drivers/vfio_ap:
14.0005
14.0006
14.000d
16.0006
16.000d

Queues listed in /sys/devices/vfio_ap/matrix/$UUID/matrix
14.0005
14.0006
14.000d
16.0005
16.0006
16.000d

Queues listed in /sys/devices/vfio_ap/matrix/$UUID/guest_matrix
14.0005
14.0006
14.000d

The reason no queues for adapter 0x16 are listed in the guest_matrix is
because queue 16.0005 is not bound to the vfio_ap device driver, so no
queue associated with the adapter is passed through to the guest;
therefore, each queue device for adapter 0x16 should display 'assigned'
instead of 'in_use', because those queues are not in use by a guest, but
only assigned to the mediated device.

Let's check the AP configuration for the guest to determine whether a
queue device is passed through before displaying a status of 'in_use'.

6690a0acbb s390/vfio-ap: let on_scan_complete() callback filter matrix and update guest's APCB
commit 774d10196e648e2c0b78da817f631edfb3dfa557 upstream.

When adapters and/or domains are added to the host's AP configuration, this
may result in multiple queue devices getting created and probed by the
vfio_ap device driver. For each queue device probed, the matrix of adapters
and domains assigned to a matrix mdev will be filtered to update the
guest's APCB. If any adapters or domains get added to or removed from the
APCB, the guest's AP configuration will be dynamically updated (i.e., hot
plug/unplug). To dynamically update the guest's configuration, its VCPUs
must be taken out of SIE for the period of time it takes to make the
update. This is disruptive to the guest's operation and if there are many
queues probed due to a change in the host's AP configuration, this could be
troublesome. The problem is exacerbated by the fact that the
'on_scan_complete' callback also filters the mdev's matrix and updates
the guest's AP configuration.

In order to reduce the potential amount of disruption to the guest that may
result from a change to the host's AP configuration, let's bypass the
filtering of the matrix and updating of the guest's AP configuration in the
probe callback - if due to a host config change - and defer it until the
'on_scan_complete' callback is invoked after the AP bus finishes its device
scan operation. This way the filtering and updating will be performed only
once regardless of the number of queues added.

baf3fcb268 s390/vfio-ap: loop over the shadow APCB when filtering guest's AP configuration
commit 16fb78cbf56e42b8efb2682a4444ab59e32e7959 upstream.

While filtering the mdev matrix, it doesn't make sense - and will have
unexpected results - to filter an APID from the matrix if the APID or one
of the associated APQIs is not in the host's AP configuration. There are
two reasons for this:

1. An adapter or domain that is not in the host's AP configuration can be
assigned to the matrix; this is known as over-provisioning. Queue
devices, however, are only created for adapters and domains in the
host's AP configuration, so there will be no queues associated with an
over-provisioned adapter or domain to filter.

2. The adapter or domain may have been externally removed from the host's
configuration via an SE or HMC attached to a DPM enabled LPAR. In this
case, the vfio_ap device driver would have been notified by the AP bus
via the on_config_changed callback and the adapter or domain would
   have already been filtered.

Since the matrix_mdev->shadow_apcb.apm and matrix_mdev->shadow_apcb.aqm are
copied from the mdev matrix sans the APIDs and APQIs not in the host's AP
configuration, let's loop over those bitmaps instead of those assigned to
the matrix.

d6b8d034b5 s390/vfio-ap: always filter entire AP matrix
commit 850fb7fa8c684a4c6bf0e4b6978f4ddcc5d43d11 upstream.

The vfio_ap_mdev_filter_matrix function is called whenever a new adapter or
domain is assigned to the mdev. The purpose of the function is to update
the guest's AP configuration by filtering the matrix of adapters and
domains assigned to the mdev. When an adapter or domain is assigned, only
the APQNs associated with the APID of the new adapter or APQI of the new
domain are inspected. If an APQN does not reference a queue device bound to
the vfio_ap device driver, then it's APID will be filtered from the mdev's
matrix when updating the guest's AP configuration.

Inspecting only the APID of the new adapter or APQI of the new domain will
result in passing AP queues through to a guest that are not bound to the
vfio_ap device driver under certain circumstances. Consider the following:

guest's AP configuration (all also assigned to the mdev's matrix):
14.0004
14.0005
14.0006
16.0004
16.0005
16.0006

unassign domain 4
unbind queue 16.0005
assign domain 4

When domain 4 is re-assigned, since only domain 4 will be inspected, the
APQNs that will be examined will be:
14.0004
16.0004

Since both of those APQNs reference queue devices that are bound to the
vfio_ap device driver, nothing will get filtered from the mdev's matrix
when updating the guest's AP configuration. Consequently, queue 16.0005
will get passed through despite not being bound to the driver. This
violates the linux device model requirement that a guest shall only be
given access to devices bound to the device driver facilitating their
pass-through.

To resolve this problem, every adapter and domain assigned to the mdev will
be inspected when filtering the mdev's matrix.

dc96fde8fc s390/dasd: protect device queue against concurrent access
commit db46cd1e0426f52999d50fa72cfa97fa39952885 upstream.

In dasd_profile_start() the amount of requests on the device queue are
counted. The access to the device queue is unprotected against
concurrent access. With a lot of parallel I/O, especially with alias
devices enabled, the device queue can change while dasd_profile_start()
is accessing the queue. In the worst case this leads to a kernel panic
due to incorrect pointer accesses.

Fix this by taking the device lock before accessing the queue and
counting the requests. Additionally the check for a valid profile data
pointer can be done earlier to avoid unnecessary locking in a hot path.

57e35d9229 s390/ap: fix AP bus crash on early config change callback invocation
commit e14aec23025eeb1f2159ba34dbc1458467c4c347 upstream.

Fix kernel crash in AP bus code caused by very early invocation of the
config change callback function via SCLP.

After a fresh IML of the machine the crypto cards are still offline and
will get switched online only with activation of any LPAR which has the
card in it's configuration. A crypto card coming online is reported
to the LPAR via SCLP and the AP bus offers a callback function to get
this kind of information. However, it may happen that the callback is
invoked before the AP bus init function is complete. As the callback
triggers a synchronous AP bus scan, the scan may already run but some
internal states are not initialized by the AP bus init function resulting

This patch improves the ap_bus_force_rescan() function which is
invoked by the config change callback by checking if a first
initial AP bus scan has been done. If not, the force rescan request
is simple ignored. Anyhow it does not make sense to trigger AP bus
re-scans even before the very first bus scan is complete.

6489ec0107 s390/dcssblk: fix kernel crash with list_add corruption
commit c8f40a0bccefd613748d080147469a4652d6e74c upstream.

Commit fb08a1908cb1 ("dax: simplify the dax_device <-> gendisk
association") introduced new logic for gendisk association, requiring
drivers to explicitly call dax_add_host() and dax_remove_host().

For dcssblk driver, some dax_remove_host() calls were missing, e.g. in
device remove path. The commit also broke error handling for out_dax case
in device add path, resulting in an extra put_device() w/o the previous
get_device() in that case.

This lead to stale xarray entries after device add / remove cycles. In the
case when a previously used struct gendisk pointer (xarray index) would be
used again, because blk_alloc_disk() happened to return such a pointer, the
xa_insert() in dax_add_host() would fail and go to out_dax, doing the extra
put_device() in the error path. In combination with an already flawed error
handling in dcssblk (device_register() cleanup), which needs to be
addressed in a separate patch, this resulted in a missing device_del() /
klist_del(), and eventually in the kernel crash with list_add corruption on
a subsequent device_add() / klist_add().

Fix this by adding the missing dax_remove_host() calls, and also move the
put_device() in the error path to restore the previous logic.

attributes use PKEY_TYPE_EP11_AES (instead of PKEY_TYPE_EP11)

There should be no negative impacts to userspace because the internal

d28f76be79 s390/dasd: fix hanging device after request requeue
[ Upstream commit 8a2278ce9c25048d999fe1a3561def75d963f471 ]

The DASD device driver has a function to requeue requests to the
blocklayer.
This function is used in various cases when basic settings for the device
have to be changed like High Performance Ficon related parameters or copy
pair settings.

The functions iterates over the device->ccw_queue and also removes the
requests from the block->ccw_queue.
In case the device is started on an alias device instead of the base
device it might be removed from the block->ccw_queue without having it
canceled properly before. This might lead to a hanging device since the
request is no longer on a queue and can not be handled properly.

Fix by iterating over the block->ccw_queue instead of the
device->ccw_queue. This will take care of all blocklayer related requests
and handle them on all associated DASD devices.

d4f5dcf68c s390/zcrypt: fix reply buffer calculations for CCA replies
[ Upstream commit 4cfca532ddc3474b3fc42592d0e4237544344b1a ]

The length information for available buffer space for CCA
replies is covered with two fields in the T6 header prepended
on each CCA reply: fromcardlen1 and fromcardlen2. The sum of
these both values must not exceed the AP bus limit for this
card (24KB for CEX8, 12KB CEX7 and older) minus the always
present headers.

The current code adjusted the fromcardlen2 value in case
of exceeding the AP bus limit when there was a non-zero
value given from userspace. Some tests now showed that this
was the wrong assumption. Instead the userspace value given for
this field should always be trusted and if the sum of the
two fields exceeds the AP bus limit for this card the first
field fromcardlen1 should be adjusted instead.

So now the calculation is done with this new insight in mind.
Also some additional checks for overflow have been introduced
and some comments to provide some documentation for future
maintainers of this complicated calculation code.

Furthermore the 128 bytes of fix overhead which is used
in the current code is not correct. Investigations showed
that for a reply always the same two header structs are
prepended before a possible payload. So this is also fixed
with this patch.

645603ab5f scsi: zfcp: Defer fc_rport blocking until after ADISC response
commit e65851989001c0c9ba9177564b13b38201c0854c upstream.

Storage devices are free to send RSCNs, e.g. for internal state changes. If
this happens on all connected paths, zfcp risks temporarily losing all
paths at the same time. This has strong requirements on multipath
configuration such as "no_path_retry queue".

Avoid such situations by deferring fc_rport blocking until after the ADISC
response, when any actual state change of the remote port became clear.
The already existing port recovery triggers explicitly block the fc_rport.
The triggers are: on ADISC reject or timeout (typical cable pull case), and
on ADISC indicating that the remote port has changed its WWPN or
the port is meanwhile no longer open.

As a side effect, this also removes a confusing direct function call to
another work item function zfcp_scsi_rport_work() instead of scheduling
that other work item. It was probably done that way to have the rport block
side effect immediate and synchronous to the caller.

[ Upstream commit 1cfef80d4c2b2c599189f36f36320b205d9447d9 ]

dev_close() and dev_open() are issued to change the interface state to DOWN
or UP (dev->flags IFF_UP). When the netdev is set DOWN it loses e.g its
Ipv6 addresses and routes. We don't want this in cases of device recovery
(triggered by hardware or software) or when the qeth device is set
offline.

Setting a qeth device offline or online and device recovery actions call
netif_device_detach() and/or netif_device_attach(). That will reset or
set the LOWER_UP indication i.e. change the dev->state Bit
__LINK_STATE_PRESENT. That is enough to e.g. cause bond failovers, and
still preserves the interface settings that are handled by the network

6deb8727f2 s390/dasd: print copy pair message only for the correct error
commit 856d8e3c633b183df23549ce760ae84478a7098d upstream.

The DASD driver has certain types of requests that might be rejected by
the storage server or z/VM because they are not supported. Since the
missing support of the command is not a real issue there is no user
visible kernel error message for this.

For copy pair setups there is a specific error that IO is not allowed on
secondary devices. This error case is explicitly handled and an error
message is printed.

The code checking for the error did use a bitwise 'and' that is used to
check for specific bits. But in this case the whole sense byte has to
match.

This leads to the problem that the copy pair related error message is
erroneously printed for other error cases that are usually not reported.
This might heavily confuse users and lead to follow on actions that might
disrupt application processing.

Fix by checking the sense byte for the exact value and not single bits.

requests again. This does unfortunately not cover internal requests which
are used for path verification for example.

This could lead to a hanging device when a path event or anything else
that triggers internal requests occurs on a quiesced device.

Fix by also calling dasd_schedule_device_bh() which triggers handling of
internal requests on resume.

25cb64ecc3 s390/zcrypt: do not retry administrative requests
commit af40322e90d4e0093569eceb7d3a28ab635f3e75 upstream.

All kind of administrative requests should not been retried. Some card
firmware detects this and assumes a replay attack. This patch checks
on failure if the low level functions indicate a retry (EAGAIN) and
checks for the ADMIN flag set on the request message. If this both
are true, the response code for this message is changed to EIO to make
sure the zcrypt API layer does not attempt to retry the request. As of
now the ADMIN flag is set for a request message when
- for EP11 the field 'flags' of the EP11 CPRB struct has the leftmost
  bit set.
- for CCA when the CPRB minor version is 'T3', 'T5', 'T6' or 'T7'.

Please note that the do-not-retry only applies to a request
which has been sent to the card (= has been successfully enqueued) but
the reply indicates some kind of failure and by default it would be
replied. It is totally fine to retry a request if a previous attempt
to enqueue the msg into the firmware queue had some kind of failure
and thus the card has never seen this request.

1e1e2ee0cf s390/dasd: Use correct lock while counting channel queue length
commit ccc45cb4e7271c74dbb27776ae8f73d84557f5c6 upstream.

The lock around counting the channel queue length in the BIODASDINFO
ioctl was incorrectly changed to the dasd_block->queue_lock with commit
583d6535cb9d ("dasd: remove dead code"). This can lead to endless list
iterations and a subsequent crash.

The queue_lock is supposed to be used only for queue lists belonging to
dasd_block. For dasd_device related queue lists the ccwdev lock must be
used.

Fix the mentioned issues by correctly using the ccwdev lock instead of
the queue lock.

02cf4a336e s390/qdio: fix do_sqbs() inline assembly constraint
commit 2862a2fdfae875888e3c1c3634e3422e01d98147 upstream.

Use "a" constraint instead of "d" constraint to pass the state parameter to
the do_sqbs() inline assembly. This prevents that general purpose register
zero is used for the state parameter.

If the compiler would select general purpose register zero this would be
problematic for the used instruction in rsy format: the register used for
the state parameter is a base register. If the base register is general
purpose register zero the contents of the register are unexpectedly ignored
when the instruction is executed.

This only applies to z/VM guests using QIOASSIST with dedicated (pass through)
QDIO-based devices such as FCP [zfcp driver] as well as real OSA or
HiperSockets [qeth driver].

A possible symptom for this case using zfcp is the following repeating kernel
message pattern:

zfcp <devbusid>: A QDIO problem occurred
zfcp <devbusid>: A QDIO problem occurred
zfcp <devbusid>: qdio: ZFCP on SC using AI:1 QEBSM:1 PRI:1 TDD:1 SIGA: W
zfcp <devbusid>: A QDIO problem occurred
zfcp <devbusid>: A QDIO problem occurred

Each of the qdio problem message can be accompanied by the following entries
for the affected subchannel in
/sys/kernel/debug/s390dbf/qdio_error/hex_ascii for zfcp or qeth:

 ccq: 69....
 SQBS ERROR.

6a40fda14b s390/vfio-ap: fix memory leak in vfio_ap device driver
[ Upstream commit 8f8cf767589f2131ae5d40f3758429095c701c84 ]

The device release callback function invoked to release the matrix device
uses the dev_get_drvdata(device *dev) function to retrieve the
pointer to the vfio_matrix_dev object in order to free its storage. The
problem is, this object is not stored as drvdata with the device; since the
kfree function will accept a NULL pointer, the memory for the
vfio_matrix_dev object is never freed.

Since the device being released is contained within the vfio_matrix_dev
object, the container_of macro will be used to retrieve its pointer.

Commit ada1da31ce34 ("s390/sclp: sort out physical vs

address for sclp_early_sccb pointer. However, it did
not take into account that kasan_early_init() can also
output messages and sclp_early_sccb should be adjusted
by the time kasan_early_init() is called.

Currently it is not a problem, since virtual and physical
addresses on s390 are the same. Nevertheless, should they
ever differ, this would cause an invalid pointer access.
