iommu/amd: remove unused variable flush_addr
Variable flush_addr is being assigned but is never read; it is redundant and can be removed. These guides can be rendered in a number of formats, like HTML and PDF. In order to build the documentation, use ``make htmldocs`` or ``make pdfdocs``.
