Thomas Gleixner 48227e21f7 x86/entry: Convert SIMD coprocessor error exception to IDTENTRY
Convert #XF to IDTENTRY_ERRORCODE:
  - Implement the C entry point with DEFINE_IDTENTRY
  - Emit the ASM stub with DECLARE_IDTENTRY
  - Handle INVD_BUG in C
  - Remove the ASM idtentry in 64bit
  - Remove the open coded ASM entry code in 32bit
  - Fixup the XEN/PV code
  - Remove the old prototypes
  - Remove the RCU warning as the new entry macro ensures correctness

No functional change.

In order to build the documentation, use ``make htmldocs`` or
``make pdfdocs``.  The formatted documentation can also be read online at:

There are various text files in the Documentation/ subdirectory,
several of them using the Restructured Text markup notation.

Please read the Documentation/process/changes.rst file, as it contains the
requirements for building and running the kernel, and information about
the problems which may result by upgrading your kernel.
