Michael J. Ruhl d757c60eca IB/rdmavt: Fix concurrency panics in QP post_send and modify to error
The RC/UC code path can go through a software loopback. In this code path
the receive side QP is manipulated.

If two threads are working on the QP receive side (i.e. post_send, and
modify_qp to an error state), QP information can be corrupted.

(post_send via loopback)
  set r_sge
  loop
     update r_sge
(modify_qp)
     take r_lock
     update r_sge <---- r_sge is now incorrect
(post_send)
     update r_sge <---- crash, etc.
     ...

This can lead to one of the two following crashes:

BUG: unable to handle kernel NULL pointer dereference at (null)
  IP: hfi1_copy_sge+0xf1/0x2e0 [hfi1]

Linux kernel
============

There are several guides for kernel developers and users. These guides can
be rendered in a number of formats, like HTML and PDF. Please read
Documentation/admin-guide/README.rst first.

In order to build the documentation, use ``make htmldocs`` or
``make pdfdocs``. The formatted documentation can also be read online at:

    https://www.kernel.org/doc/html/latest/

There are various text files in the Documentation/ subdirectory,
several of them using the Restructured Text markup notation.

Please read the Documentation/process/changes.rst file, as it contains the
requirements for building and running the kernel, and information about
the problems which may result by upgrading your kernel.
