device_id: avoid use_devices list lookups

- Add a 'du' pointer from each struct device when a
  dev and du structs are paired.  Allows getting du
  from dev without a search.

- Add a radix tree pointing to du's on cmd->use_devices,
  indexed by devname.  Allows quick lookup of du from a
  devname.
