77be3250d9 dev-cache: move global variables into cmd struct
The cmd struct is now required in many more functions, and
it's added as a function arg for most direct dev-cache function
calls.  The cmd struct is added to struct device (dev->cmd) so
that it can be accessed in many other cases where dev-cache
functions are being called from places where getting the cmd
struct is too difficult.

57bb46c5e7 filter: use bcache for filter reads
Filters are still applied before any device reading or
the label scan, but any filter checks that want to read
the device are skipped and the device is flagged.

After bcache is populated, but before lvm looks for
devices (i.e. before label scan), the filters are
reapplied to the devices that were flagged above.
The filters will then find the data they need in
bcache.

5011cac9cf filters: add cmd->full_filter - composite of cmd->filter and cmd->lvmetad_filter
There are actually three filter chains if lvmetad is used:
  - cmd->lvmetad_filter used when when scanning devices for lvmetad
  - cmd->filter used when processing lvmetad responses
  - cmd->full_fiilter (which is just cmd->lvmetad_filter + cmd->filter chained together) used
    for remaining situations

This patch adds the third one - "cmd->full_filter" - currently this is
used if device processing does not fall into any of the groups before,
for example, devices which does not have the PV label yet and we're just
creating a new one or we're processing the devices where the list of the
devices (PVs) is not returned by lvmetad initially.

Currently, the cmd->full_filter is used exactly in these functions:
  - lvmcache_label_scan
  - _pvcreate_check
  - pvcreate_vol
  - lvmdiskscan
  - pvscan
  - _process_each_label

If lvmetad is used, then simply cmd->full_filter == cmd->filter because
cmd->lvmetad_filter is NULL in this case.

254944d890 Set to a defined value vars used after error path
Static analyzer noticed this vars are used even when error is reported
back thus their state is undefined - set to 0 for this case.

c08c564e21 Use new dev_open_readonly fn to prevent opening devices for read-write when not necessary.
Before, we used vg_write_lock_held call to determnine the way a device is
opened. Unfortunately, this opened many devices in RW mode when it was not
really necessary. With the OPTIONS+="watch" rule used in the udev rules,
this could fire numerous events while closing such devices (and it caused
useless scans from within udev rules in return).

A common bug we hit with this was with the lvremove command which was unable
to remove the LV since it was being opened from within the udev rules. This
patch should minimize such situations (at least with respect to LVM handling
of devices).

Though there's still a possibility someone will open a device 'outside' in
parallel and fire the event based on the watch rule when closing a device
once opened for RW.

8ef2b021ed Default stripesize 64k & config file setting for it;
Clear many compiler warnings (i386) & associated bugs - hopefully without
introducing too many new bugs:-)  (Same exercise required for other archs.)
Default compilation has optimisation - or else use ./configure --enable-debug

25b733809a Merge with text format branch.
Lots of changes/very little testing so far => there'll be bugs!

Use 'vgcreate -M text' to create a volume group with its metadata stored
in text files.  Text format metadata changes should be reasonably atomic,
with a (basic) automatic recovery mechanism if the system crashes while a
change is in progress.

Add a metadata section to lvm.conf to specify multiple directories if
you want (recommended) to keep multiple copies of the metadata (eg on
different filesystems).

e.g. metadata {
        dirs = ["/etc/lvm/metadata1","/usr/local/lvm/metadata2"]
}

Plenty of refinements still in the pipeline.
