Make recursive directory path creation reusable via
dir_create_recursive.
While we already have dm_create_dir() - it's not taking mode arg,
so let's make lvm's internal file helper function.
As we start refactoring the code to break dependencies (see doc/refactoring.txt),
I want us to use full paths in the includes (eg, #include "base/data-struct/list.h").
This makes it more obvious when we're breaking abstraction boundaries, eg, including a file in
metadata/ from base/
When available use nanosecond stat info.
If commands are running closely enough after config update,
the .cache file from persistent filter could have been ignored.
This happens sometimes during i.e. synthetic test suite run.
There's a possibility someone will use the '/' in the hostname. Since we
generate a temporary file name (path) including the hostname, any '/' would
be ambiguous.
We can always set such hostname using 'sethostname' from unistd.h. But the
'hostname' command already includes the check and removes the '/' char.
However, some old versions still allow that.

Since this is only a temporary name and the possibility of this error is
quite negligible, we don't need any complex escape sequence here, just a
simple char replace.

and diagnose unlikely write failure.

* lib/format_text/format-text.c (_vg_write_file): Likewise.
* lib/log/log.c (fin_log): Similar, but use dm_fclose directly.
Include "\n" at end of each fprintf format string.
Fix some memory leaks in error paths found by coverity.
Use C99 struct initialisers.
Move DEFS into configure.h.
Clean-ups to remove miscellaneous compiler warnings.
