* Rework the toollib interface (process_each_*) on top of new vg_read.

Fix some memory leaks in error paths found by coverity.
Use C99 struct initialisers.
Move DEFS into configure.h.
Clean-ups to remove miscellaneous compiler warnings.
Clear many compiler warnings (i386) & associated bugs - hopefully without
introducing too many new bugs:-) (Same exercise required for other archs.)
Default compilation has optimisation - or else use ./configure --enable-debug
allocation policy. This can currently take one of three values:

Notice that 'SIMPLE' has turned into the slightly more meaningful NEXT_FREE.
