18259d5559 Remove unused clvm variations for active LVs
Different flavors of activate_lv() and lv_is_active()
which are meaningful in a clustered VG can be eliminated
and replaced with whatever that flavor already falls back
to in a local VG.

e.g. lv_is_active_exclusive_locally() is distinct from
lv_is_active() in a clustered VG, but in a local VG they
are equivalent.  So, all instances of the variant are
replaced with the basic local equivalent.

For local VGs, the same behavior remains as before.
For shared VGs, lvmlockd was written with the explicit
requirement of local behavior from these functions
(lvmlockd requires locking_type 1), so the behavior
in shared VGs also remains the same.

7f97c7ea9a build: Don't generate symlinks in include/ dir
As we start refactoring the code to break dependencies (see doc/refactoring.txt),
I want us to use full paths in the includes (eg, #include "base/data-struct/list.h").
This makes it more obvious when we're breaking abstraction boundaries, eg, including a file in
metadata/ from base/

e86910b052 lvconvert: use excl activation for conversion
Use properly exclusive activation when reactivating origin after
snapshot merge (since origin must have been previously also exlusively
activated).

Same applies when converting volumes to thin-pool or cache.

Previously used 'only' local activation incorrectly allowed local
activation of some targets (i.e. raid) - thus 'leaking' chance to
activate same device on another node - which can be a problem
for device types like raid.

4d0793f0ec pool: rework handling of passed args
As now we can properly recognize all paramerters for pool creation,
we may drop PASS_ARG_  defines and rely on '_UNSELECTED' or 0 entries
as being those without user given args.

When setting are not given on command line - 'update' function
fill them from profiles or configuration. For this  'profile' arg
was needed to be passed around and since  'VG' itself is not needed,
it's been all replaced with 'cmd, profile, extents_size' args.

fc628e92ba metadata: also look at historical LVs when checking LV name availability
Live LVs and historical LVs are in one namespace and the name needs to
be unique in whole VG.

f545dd5952 metadata: honour 'nohistory' switch when removing thin LVs
When --nohistory switch is used with lvremove, the LV that is removed is
not recorded in metadata as 'historical LV'.

a61bc70f62 metadata: add support for interconnection of thin pool LV segment with indirect origin
Add support for making an interconnection between thin LV segment and
its indirect origin (which may be historical or live LV) - add a new
"indirect_origin" argument to attach_pool_lv function.

790b2e8748 metadata: create historical LVs when LVs are removed and interconnect with live LVs
When an LV is being removed, we create an instance of
"struct historical_logical_volume" wrapped up in
"struct generic_logical_volume".

All instances of "struct historical_logical_volume" are then recorded in
"historical_lvs" list which is part of "struct volume_group".

The "historical LV" is then interconnected with "live LVs" to
connect a history chain for the live LV.

e2b00b0a89 cleanup: use display_lvname in pmspare
Just switch to use display_lvname().
Also squeeze possibly failing strncpy into INTERNAL_ERROR
as lvname always should fit.

ff2e8b0de6 thin: simplify thin volume creation
Move code for creation of thin volume into a single place
out of lv_extend().  This allows to drop extra pool arg
for alloc_lv_segment() && lv_extend() and makes code
more easier to read and follow.

7359a9df88 pool: better error path handling
Move test for size of new LV names in front before
any creation of LV.

Properly check striped  segtype kernel presence,
since passed 'segtype' is already tested.

Keep deactivation error path local to wiping part of the function.

Create metadata with temporary flag (it's activated, zeroed
and deactivated).

9411c19b31 segments: introduce lvseg_name
Instead of segtype->ops->name()  introduce lvseg_name().

This also allows us to leave name() function 'empty' for default
return of segtype->name.

TODO: add functions for rest of ops->

d3004479cc cache: use remove_layer_from_lv
Since the usability problem were fixed, we can use this function.
Cleanup orphan LVs with TEMPORARY flags
(reduces couple blkid error reports, but couple of them
is still left...)

8a60cbcf45 thin: always activate and deactive pool when creating
When we create thin-pool we have used trick to keep
volume active, but since we now support TEMPORARY flag,
we could just localy active & deactive metadata LV,
and let the thinpool through normal activation process.

e833d84e67 cache/pool: Make the fns in pool_manip.c work with cache pools
The functions in pool_manip.c are specific to thin pools.  It's
now time to make them more generic and able to handle cache pools
as well.
