8211a13ce0 Always insert an intermediate layer for mirrors. 
Suppress hidden LVs from reports unless --all is given.
Use square brackets for hidden LVs in reports.
Centralise restrictions on LV names.

1832f310ac Add --type to lvcreate/resize. 
Add virtual segment types, zero and error.

A large sparse device can be constructed as a writeable snapshot of a large
zero device.
