a5fe5a7cdd Fix vgexport/vgimport to set/reset PV exported flag so pv_attr is correct. 
Add vgid to struct physical_volume and pass with vg_name to some functions.

8211a13ce0 Always insert an intermediate layer for mirrors. 
Suppress hidden LVs from reports unless --all is given.
Use square brackets for hidden LVs in reports.
Centralise restrictions on LV names.
