Alasdair Kergon d06f64dd29 Allow multiple device names with pvscan --lvmetad.
Hold global lock in pvscan --lvmetad.  (This might need refinement.)
Add PV name to "PV gone" messages.
Adjust some log message severities.  (More changes needed.)
