This makes it easier for people to change install media afterwards

But also this makes it more clear that if users want to use virtio-win,
they need to add an additional CDROM and not try to reuse the install
CDROM device
Now we have only one Installer class, and callers don't need to
worry about choosing a particular class type depending on their
needs, just pass cdrom vs. location to the installer init and
we figure out everything behind the scenes.
Besides simplifying the callers this makes the control flow a
lot easier to follow whether looking at InstallerTreeMedia or
Installer classes
This changes all the callers to invoke start_install directly on the
Installer object. We still stash the installer instance inside the
guest object in create.py, just for simplicity
- Remove anything for less than qemu 0.12 or libvirt 0.10, basically
rhel6 vintage stuff
- Open code some simple checks
- Remove some that are only used for unnecessary error reporting

Use consistent and minimal license header for every file
Where before this they said "
"either version 2 of the License, or (at your option) any later version."
Now they just say
"GNU GPLv2"
This fixes it to say "GNU GPLv2 or later" again.

If we try to run the testsuite on anything older than libvirt 3.1,
it immediately throws an exception before processing any tests,
due to choking on parsing drm bits from testdriver.xml. This global
failure is only due to sloppy coding though.
Turn all test cases that use testdriver.xml into skips in this case,
so we can at least get some test coverage otherwise.
This feature is available in upstream Qemu since 2.0.0 but it was also
back-ported to downstream Qemu into qemu-kvm-1.5.3 released in RHEL-7.
Add a new check and enable hv_time also in RHEL systems if qemu new
enough.

Each guest type can have its own capabilities and we should always ask
only for those capabilities.
The old approach was to get capabilities from libvirt and then for
example cycle trough all guests and return True, if any guest type
supports kvm or pae, etc.
Now we check those capabilities only for the correct guest type
according to defaults and input from user.

Make it clear which ones are used for the caps unit tests, and which
are for cli/UI testing and can be updated at will. And drop a bunch
of outdated stuff.
Previously we made all the default encoding non-permanent by XML objects
before generating the XML, making changes to the copies, and restoring
to the old state after the XML is returned to the user.
This allows us to call start_install multiple times with the same Guest
object and not alter it's original config... but that feature isn't really
useful anymore, and this behavior makes the 'customize before install'
dialog difficult to handle.
So drop most of it, and fix some of the minor fallout.
This streamlines virt-manager and virt-install implementations, requiring
installer.distro_detect to be called if we want distro detection. As
a side effect, we now get CDROM detection for free.
commit 466c2bcf9c
will generate the same index for
'hda' and 'hdaa'.
Also break test cases.
This patch will fix this.

Passing a zero to the generate_target() function's as pref_ctrl
parameter makes the 'if pref_ctrl' conditions obviously false. Also
the range created was starting from 0 and not from 1. Apart from
fixing this, also fix tests so they actually test something this time.
