user_info->mapped_state has nothing to do with enum auth_password_state,
user_info->password_state is the one that holds the auth_password_state value.
Luckily user_info->password_state was never referenced in the
encrypt_user_info() callers.

DsCrackNameOneName on a member does not really have a big user database. We
should delegate as much responsibility as possible to our DC.

DsCrackNameOneName directly fails for DRSUAPI_DS_NAME_FORMAT_NT4_ACCOUNT
if the name passed in does not contain a \. The only caller of
map_user_info_cracknames (auth_check_password_send) passes in
lpcfg_workgroup(), which does not contain a \. Add in the \ also for
the default_domain case.

to properly support multi-domain forests we need to determine if an
incoming username is part of a known forest domain or not. To do this
for all possible SPN forms, we need to use CrackNames.
This changes map_user_info() to use CrackNames if a SAM context is
available, and asks the CrackNames services to parse the incoming
username and domain into a NT4 form, which can then be used in the
SAM.

This also changes the calling convention slightly - we should always
allocate this with talloc_zero() to allow some elements to be
optional. Some elements may only make sense in Samba3, which I hope
will use this common structure.
