ctdb_pkt_recv_recv() and ctdb_read_packet() give us a non-null hdr on
success, so drop the error path check in favour of an assert.
This fixes a regression in 3913b9a4088b83d6ed7f94d136a26ecfa7e16b35,
where tevent_req_error() may be skipped in the ctdbd_parse_done()
ctdb_pkt_recv_recv() error path.

Seeing:
ctdb_read_packet failed: Cannot allocate memory
[..., 0] ../source3/lib/ctdbd_conn.c:121(cluster_fatal)
cluster fatal event: ctdbd died
The error is due to a memory allocation failure rather than ctdbd
dying. However, the error message makes people wonder why ctdbd died.
Another alternative would be to wrap cluster_fatal() and have the
wrapper interpret the return value from ctdb_read_packet() to choose
from a set of more precise messages to pass to cluster_fatal(). For a
memory allocation it isn't strictly necessary to call cluster_fatal(),
but all is probably lost and it is still probably better to try to
exit cleanly as soon as possible instead of crashing somewhere.

ctdb_private is much broader. Right now we implement the protocol
ourselves. In the future, we might switch to the native ctdb
client implementation defined in ctdb_client.h, but that's a
different project :-)

Implement the ctdb packet layer for async parse send/recv with tevent.
ctdbd_setup_fde() will is used to create an fde from the
connection fd and will be called from dbwrap_ctdb.
ctdbd_parse_send() and ctdbd_parse_recv() will be used by dbwrap_ctdb
for async packet sending and receiving.

Split ctdbd_init_connection() into an internal function that does the
connection setup and only keep the conn object allocation in
ctdbd_init_connection().
This is in preperation of adding ctdbd_reinit_connection() which will
use the new internal function as well.

CTDB can send CTDB_REPLY_ERROR in case it encounters an error condition.
This is treated as successful migration as "ret" is not set.

If the node has flag NODE_FLAGS_PERMANENTLY_DISABLED, then it is
still working. Only avoid connections if node has any of the flags
in NODE_FLAGS_INACTIVE.
