In all places where we access or modify a file or it's associated metadata, we
use fsp_get_io_fd() to fetch the low-level fd from the fsp. This ensures we
don't accidentally use a pathref fsp where the fd would be opened as root on
systems lacking O_PATH.

Since the introduction of the vfs_aio_fork:erratic_testing_mode this
crashed reliably, as we had two different structs behind
SMB_VFS_HANDLE_SET_DATA. I had always believed that due to the fact that
we have specific aio_fork tests in our autobuild, this would have been
tested. But it was not, because the share definition missed the the "aio
read/write size = 1" to actually use the async code in vfs_aio_fork.

Not currently used - no logic changes inside.
This will make it possible to pass down a long-lived talloc
context from the loading function for modules to use instead
of having them internally all use talloc_autofree_context()
which is a hidden global.
Updated all known module interface numbers, and added a
WHATSNEW.

Removes accessor functions as now this parameter is set
under user control in smb.conf. Default is 100.
Note that this doesn't limit the number of outstanding
aio requests, it just causes them to go onto the
pthreadpool queue.
Now we need to prioritize pthreadpool pipe replies
ahead of incoming SMB2 requests, but that's a patch
for another day.

Code needs fixing when HAVE_STRUCT_MSGHDR_MSG_CONTROL is
not defined. Also Solaris doesn't have msg_flags field
(which we set to zero anyway, so if we initialize
the entire struct to zero, we never need to refer to it).

Compile the basic aio engine always, it works via libasys/pthreadpool_sync
in a sync fashion even if no pthreads are around. Everything else (linux
aio, posix aio, aio fork) is now compiled as modules based on specific
system capabilities
