When a node is starting, CTDB reports remote nodes as unhealthy by
default. This can be misleading.
To hide this, report an "UNKNOWN" pseudo state when a remote node is
not disconnected and the runstate is less than or equal to
"FIRST_RECOVERY".

the case of the original. A couple of minor cleanups were made in the
documentation (such as "LVSMASTER" -> "LVS leader").

This happens when dummy_client has not registered the SRVID (for its
10th connection) before the 2nd simple_test.
Change the initial wait to ensure that the SRVID is registered.

Now they are clearly all part of CTDB.
TEST_SOCKET_WRAPPER_SO_PATH gets too long in
integration_local_daemons.bash, so change it to
CTDB_TEST_SWRAP_SO_PATH instead of just prefixing.

There is no good reason why the code needs to be this way. The
intervening code was removed years ago leaving a more complex version
of something very simple.
