This effectively reverts commit 17eba16b. It looks like these flags have
been introduced as part of the onefs support which has been removed
again. As there is no other use for the flags, remove them.

This was only implemented by onefs in this way. If we get around to use
for example fanotify or something similar, we can either re-add them or
do it in a different way. For now, simplify the code.

Due to inconsistent use of lp_kernel_oplocks() we could miss kernel
oplocks being on/off in some of our oplock handling code, and thus
use the wrong logic.
Ensure all logic around koplocks and lp_kernel_oplocks() is consistent.

We need to expect any amount of "stat" opens on the file without
triggering an assert.
This is the correct fix for bug #11844. I guess we haven't seens this
very often before bug #12766 got fixed, because most clients were using
LEASES instead of OPLOCKS.

Update an optimisation in update_num_read_oplocks() that checks for
exclusive oplocks to the lease area.
The idea of the optimisation is to avoid expensive db queries in
brlock.tdb if we *know* we're the only open.

A client can:
- open a handle (h1)
- write some data to h1.
- open a 2nd handle h2 (downgrades both handles to level II)
- try to read the data on h2 (this gets old data)

The name of this function has changed, but the DEBUG statements have
not been adapted. This is the case in a lot of our code. With __func__
this problem goes away: __func__ is C99, and we also use it already.

Now that we transmit the level we want to break to via the msg.op_type
we can unify MSG_SMB_BREAK_REQUEST and MSG_SMB_ASYNC_LEVEL2_BREAK and
thus simplify the code a bit.

The level we have to break to depend on the breakers create_disposition:
If we overwrite, we have to break to none.
This patch overloads the "op_type" field in the break message we send
across to the smbd holding the oplock with the oplock level we want to
break to. Because it depends on the create_disposition in the breaking
open, only the breaker can make that decision. We might want to use
a different mechanism for this in the future, but for now using the
op_type field seems acceptable to me.

one caller: process_oplock_async_level2_break_message. The further
goal is to merge process_oplock_async_level2_break_message with
process_oplock_break_message.

FAKE_LEVEL_II_OPLOCK was an indicator to break level2 oplock holders
on write. This information is now being held in brlock.tdb, which makes
the FAKE_LEVEL_II_OPLOCK type unnecessary.
