aedbaf4f6a KVM: x86: Extract kvm_update_cpuid_runtime() from kvm_update_cpuid()
Beside called in kvm_vcpu_ioctl_set_cpuid*(), kvm_update_cpuid() is also
called 5 places else in x86.c and 1 place else in lapic.c. All those 6
places only need the part of updating guest CPUIDs (OSXSAVE, OSPKE, APIC,
KVM_FEATURE_PV_UNHALT, ...) based on the runtime vcpu state, so extract
them as a separate kvm_update_cpuid_runtime().

To fix it, record the beginning of an update of the APIC map in
apic_map_dirty.  If another APIC map change switches apic_map_dirty
back to DIRTY during the update, kvm_recalculate_apic_map should not
make it CLEAN, and the other caller will go through the slow path.

af28dfacbe kvm: lapic: fix broken vcpu hotplug
Guest fails to online hotplugged CPU with error
  smpboot: do_boot_cpu failed(-1) to wakeup CPU#4

It's caused by the fact that kvm_apic_set_state(), which used to call
recalculate_apic_map() unconditionally and pulled hotplugged CPU into
apic map, is updating map conditionally on state changes.  In this case
the APIC map is not considered dirty and the is not updated.

Fix the issue by forcing unconditional update from kvm_apic_set_state(),
like it used to be.

ae95f566b3 KVM: X86: TSCDEADLINE MSR emulation fastpath
This patch implements a fast path for emulation of writes to the TSCDEADLINE
MSR.  Besides shortcutting various housekeeping tasks in the vCPU loop,
the fast path can also deliver the timer interrupt directly without going
through KVM_REQ_PENDING_TIMER because it runs in vCPU context.

da4ad88cab kvm: Replace vcpu->swait with rcuwait
The use of any sort of waitqueue (simple or regular) for
wait/waking vcpus has always been an overkill and semantically
wrong. Because this is per-vcpu (which is blocked) there is
only ever a single waiting vcpu, thus no need for any sort of
queue.

As such, make use of the rcuwait primitive, with the following
considerations:

  - rcuwait already provides the proper barriers that serialize
  concurrent waiter and waker.

  - Task wakeup is done in rcu read critical region, with a
  stable task pointer.

  - Because there is no concurrency among waiters, we need
  not worry about rcuwait_wait_event() calls corrupting
  the wait->task. As a consequence, this saves the locking
  done in swait when modifying the queue. This also applies
  to per-vcore wait for powerpc kvm-hv.

The x86 tscdeadline_latency test mentioned in 8577370fb0
("KVM: Use simple waitqueue for vcpu->wq") shows that, on avg,
latency is reduced by around 15-20% with this change.

24647e0a39 KVM: x86: Return updated timer current count register from KVM_GET_LAPIC
kvm_vcpu_ioctl_get_lapic (implements KVM_GET_LAPIC ioctl) does a bulk copy
of the LAPIC registers but must take into account that the one-shot and
periodic timer current count register is computed upon reads and is not
present in register state. When restoring LAPIC state (e.g. after
migration), restart timers from their their current count values at time of
save.

Note: When a one-shot timer expires, the code in arch/x86/kvm/lapic.c does
not zero the value of the LAPIC initial count register (emulating HW
behavior). If no other timer is run and pending prior to a subsequent
KVM_GET_LAPIC call, the returned register set will include the expired
one-shot initial count. On a subsequent KVM_SET_LAPIC call the code will
see a non-zero initial count and start a new one-shot timer using the
expired timer's count. This is a prior existing bug and will be addressed

afaf0b2f9b KVM: x86: Copy kvm_x86_ops by value to eliminate layer of indirection
Replace the kvm_x86_ops pointer in common x86 with an instance of the
struct to save one pointer dereference when invoking functions.  Copy the
struct by value to set the ops during kvm_init().

Arbitrarily use kvm_x86_ops.hardware_enable to track whether or not the
ops have been initialized, i.e. a vendor KVM module has been loaded.

This patch optimizes the virtual IPI fastpath emulation sequence:

We can observe ~0.67% performance improvement for IPI microbenchmark

94be4b85d8 KVM: LAPIC: Also cancel preemption timer when disarm LAPIC timer
The timer is disarmed when switching between TSC deadline and other modes,
we should set everything to disarmed state, however, LAPIC timer can be
emulated by preemption timer, it still works if vmx->hv_deadline_timer is
not -1. This patch also cancels preemption timer when disarm LAPIC timer.

4abaffce4d KVM: LAPIC: Recalculate apic map in batch
In the vCPU reset and set APIC_BASE MSR path, the apic map will be recalculated
several times, each time it will consume 10+ us observed by ftrace in my
non-overcommit environment since the expensive memory allocate/mutex/rcu etc
operations. This patch optimizes it by recaluating apic map in batch, I hope
this can benefit the serverless scenario which can frequently create/destroy
VMs.

Before patch:

kvm_lapic_reset  ~27us

After patch:

kvm_lapic_reset  ~14us

Observed by ftrace, improve ~48%.

We have both APIC_SHORT_MASK and KVM_APIC_SHORT_MASK defined for the
shorthand mask.  Similarly, we have both APIC_DEST_MASK and
KVM_APIC_DEST_MASK defined for the destination mode mask.

Drop the KVM_APIC_* macros and replace the only user of them to use
the APIC_DEST_* macros instead.  At the meantime, move APIC_SHORT_MASK
and APIC_DEST_MASK from lapic.c to lapic.h.

7ee30bc132 KVM: x86: deliver KVM IOAPIC scan request to target vCPUs
In IOAPIC fixed delivery mode instead of flushing the scan
requests to all vCPUs, we should only send the requests to
vCPUs specified within the destination field.

This patch introduces kvm_get_dest_vcpus_mask() API which
retrieves an array of target vCPUs by using
kvm_apic_map_get_dest_lapic() and then based on the
vcpus_idx, it sets the bit in a bitmap. However, if the above
fails kvm_get_dest_vcpus_mask() finds the target vCPUs by
traversing all available vCPUs. Followed by setting the
bits in the bitmap.

If we had different vCPUs in the previous request for the
same redirection table entry then bits corresponding to
these vCPUs are also set. This to done to keep
ioapic_handled_vectors synchronized.

This bitmap is then eventually passed on to
kvm_make_vcpus_request_mask() to generate a masked request
only for the target vCPUs.

This would enable us to reduce the latency overhead on isolated
vCPUs caused by the IPI to process due to KVM_REQ_IOAPIC_SCAN.

fixed KVM to also latch pending LAPIC INIT event when vCPU is in VMX
operation.

However, current API of KVM_SET_MP_STATE allows userspace to put vCPU
into KVM_MP_STATE_SIPI_RECEIVED or KVM_MP_STATE_INIT_RECEIVED even when
vCPU is in VMX operation.

Fix this by introducing a util method to check if vCPU state latch INIT
signals and use it in KVM_SET_MP_STATE handler.

4b9852f4f3 KVM: x86: Fix INIT signal handling in various CPU states
Commit cd7764fe9f ("KVM: x86: latch INITs while in system management mode")
changed code to latch INIT while vCPU is in SMM and process latched INIT
when leaving SMM. It left a subtle remark in commit message that similar
treatment should also be done while vCPU is in VMX non-root-mode.

However, INIT signals should actually be latched in various vCPU states:
(*) For both Intel and AMD, INIT signals should be latched while vCPU
is in SMM.
(*) For Intel, INIT should also be latched while vCPU is in VMX
operation and later processed when vCPU leaves VMX operation by
executing VMXOFF.
(*) For AMD, INIT should also be latched while vCPU runs with GIF=0
or in guest-mode with intercept defined on INIT signal.

To fix this:
1) Add kvm_x86_ops->apic_init_signal_blocked() such that each CPU vendor
can define the various CPU states in which INIT signals should be
blocked and modify kvm_apic_accept_events() to use it.
2) Modify vmx_check_nested_events() to check for pending INIT signal
while vCPU in guest-mode. If so, emualte vmexit on
EXIT_REASON_INIT_SIGNAL. Note that nSVM should have similar behaviour
but is currently left as a TODO comment to implement in the future
because nSVM don't yet implement svm_check_nested_events().

Note: Currently KVM nVMX implementation don't support VMX wait-for-SIPI
activity state as specified in MSR_IA32_VMX_MISC bits 6:8 exposed to
guest (See nested_vmx_setup_ctls_msrs()).
If and when support for this activity state will be implemented,
kvm_check_nested_events() would need to avoid emulating vmexit on
INIT signal in case activity-state is wait-for-SIPI. In addition,
kvm_apic_accept_events() would need to be modified to avoid discarding
SIPI in case VMX activity-state is wait-for-SIPI but instead delay
SIPI processing to vmx_check_nested_events() that would clear
pending APIC events and emulate vmexit on SIPI.

b14c876b99 kvm: x86: skip populating logical dest map if apic is not sw enabled
recalculate_apic_map does not santize ldr and it's possible that
multiple bits are set. In that case, a previous valid entry
can potentially be overwritten by an invalid one.

This condition is hit when booting a 32 bit, >8 CPU, RHEL6 guest and then
triggering a crash to boot a kdump kernel. This is the sequence of
events:
1. Linux boots in bigsmp mode and enables PhysFlat, however, it still
writes to the LDR which probably will never be used.
2. However, when booting into kdump, the stale LDR values remain as
they are not cleared by the guest and there isn't a apic reset.
3. kdump boots with 1 cpu, and uses Logical Destination Mode but the
logical map has been overwritten and points to an inactive vcpu.

On PREEMPT_RT enabled kernels unmarked hrtimers are moved into soft
interrupt expiry mode by default.

While that's not a functional requirement for the KVM local APIC timer
emulation, it's a latency issue which can be avoided by marking the timer
so hard interrupt context expiry is enforced.

0c5f81dad4 KVM: LAPIC: Inject timer interrupt via posted interrupt
Dedicated instances are currently disturbed by unnecessary jitter due
to the emulated lapic timers firing on the same pCPUs where the
vCPUs reside.  There is no hardware virtual timer on Intel for guest
like ARM, so both programming timer in guest and the emulated timer fires
incur vmexits.  This patch tries to avoid vmexit when the emulated timer
fires, at least in dedicated instance scenario when nohz_full is enabled.

In that case, the emulated timers can be offload to the nearest busy
housekeeping cpus since APICv has been found for several years in server
processors. The guest timer interrupt can then be injected via posted interrupts,
which are delivered by the housekeeping cpu once the emulated timer fires.

The host should tuned so that vCPUs are placed on isolated physical
processors, and with several pCPUs surplus for busy housekeeping.
If disabled mwait/hlt/pause vmexits keep the vCPUs in non-root mode,
~3% redis performance benefit can be observed on Skylake server, and the
number of external interrupt vmexits drops substantially.  Without patch

  masking or handling by the CPU.

In Thomas's testing, hardware cpu will not respect soft disable LAPIC
when IRR has already been set or APICv posted-interrupt is in flight,
so we can skip soft disable APIC checking when clearing IRR and set ISR,
continue to respect soft disable APIC when attempting to set IRR.

a251fb90ab KVM: x86: Fix apic dangling pointer in vcpu
The function kvm_create_lapic() attempts to allocate the apic structure
and sets a pointer to it in the virtual processor structure. However, if
get_zeroed_page() failed, the function frees the apic chunk, but forgets
to set the pointer in the vcpu to NULL. It's not a security issue since
there isn't a use of that pointer if kvm_create_lapic() returns error,
but it's more accurate that way.

b6c4bc659c KVM: LAPIC: Optimize timer latency further
Advance lapic timer tries to hidden the hypervisor overhead between the
host emulated timer fires and the guest awares the timer is fired. However,
it just hidden the time between apic_timer_fn/handle_preemption_timer ->
wait_lapic_expire, instead of the real position of vmentry which is
mentioned in the orignial commit d0659d946b ("KVM: x86: add option to
advance tscdeadline hrtimer expiration"). There is 700+ cpu cycles between
the end of wait_lapic_expire and before world switch on my haswell desktop.

This patch tries to narrow the last gap(wait_lapic_expire -> world switch),
it takes the real overhead time between apic_timer_fn/handle_preemption_timer
and before world switch into consideration when adaptively tuning timer
advancement. The patch can reduce 40% latency (~1600+ cycles to ~1000+ cycles
on a haswell desktop) for kvm-unit-tests/tscdeadline_latency when testing
busy waits.

ec0671d568 KVM: LAPIC: Delay trace_kvm_wait_lapic_expire tracepoint to after vmexit
wait_lapic_expire() call was moved above guest_enter_irqoff() because of
its tracepoint, which violated the RCU extended quiescent state invoked
by guest_enter_irqoff()[1][2]. This patch simply moves the tracepoint
below guest_exit_irqoff() in vcpu_enter_guest(). Snapshot the delta before
VM-Enter, but trace it after VM-Exit. This can help us to move
wait_lapic_expire() just before vmentry in the later patch.

Checking for a pending non-periodic interrupt in start_hv_timer() leads
to restart_apic_timer() making an unnecessary call to start_sw_timer()
due to start_hv_timer() returning false.

Alternatively, start_hv_timer() could return %true when there is a
pending non-periodic interrupt, but that approach is less intuitive,
i.e. would require a beefy comment to explain an otherwise simple check.

Explicitly call cancel_hv_timer() instead of returning %false to coerce
restart_apic_timer() into canceling it by way of start_sw_timer().

Functionally, the existing code is correct in the sense that it doesn't
doing anything visibily wrong, e.g. generate spurious interrupts or miss
an interrupt.  But it's extremely confusing and inefficient, e.g. there
are multiple extraneous calls to apic_timer_expired() that effectively
get dropped due to @timer_pending being %true.

b6aa57c69c KVM: lapic: Convert guest TSC to host time domain if necessary
To minimize the latency of timer interrupts as observed by the guest,
KVM adjusts the values it programs into the host timers to account for
the host's overhead of programming and handling the timer event.  In
the event that the adjustments are too aggressive, i.e. the timer fires
earlier than the guest expects, KVM busy waits immediately prior to
entering the guest.

Currently, KVM manually converts the delay from nanoseconds to clock
cycles.  But, the conversion is done in the guest's time domain, while
the delay occurs in the host's time domain.  This is perfectly ok when
the guest and host are using the same TSC ratio, but if the guest is
using a different ratio then the delay may not be accurate and could
wait too little or too long.

When the guest is not using the host's ratio, convert the delay from
guest clock cycles to host nanoseconds and use ndelay() instead of
__delay() to provide more accurate timing.  Because converting to
nanoseconds is relatively expensive, e.g. requires division and more
multiplication ops, continue using __delay() directly when guest and
host TSCs are running at the same ratio.

c3941d9e0c KVM: lapic: Allow user to disable adaptive tuning of timer advancement
The introduction of adaptive tuning of lapic timer advancement did not
allow for the scenario where userspace would want to disable adaptive
tuning but still employ timer advancement, e.g. for testing purposes or
to handle a use case where adaptive tuning is unable to settle on a
suitable time.  This is epecially pertinent now that KVM places a hard
threshold on the maximum advancment time.

Rework the timer semantics to accept signed values, with a value of '-1'
being interpreted as "use adaptive tuning with KVM's internal default",
and any other value being used as an explicit advancement time, e.g. a
time of '0' effectively disables advancement.

Note, this does not completely restore the original behavior of
lapic_timer_advance_ns.  Prior to tracking the advancement per vCPU,
which is necessary to support autotuning, userspace could adjust
lapic_timer_advance_ns for *running* vCPU.  With per-vCPU tracking, the
module params are snapshotted at vCPU creation, i.e. applying a new
advancement effectively requires restarting a VM.

Dynamically updating a running vCPU is possible, e.g. a helper could be
added to retrieve the desired delay, choosing between the global module
param and the per-VCPU value depending on whether or not auto-tuning is
(globally) enabled, but introduces a great deal of complexity.  The
wrapper itself is not complex, but understanding and documenting the
effects of dynamically toggling auto-tuning and/or adjusting the timer
advancement is nigh impossible since the behavior would be dependent on
KVM's implementation as well as compiler optimizations.  In other words,
providing stable behavior would require extremely careful consideration
now and in the future.

Given that the expected use of a manually-tuned timer advancement is to
"tune once, run many", use the vastly simpler approach of recognizing
changes to the module params only when creating a new vCPU.

39497d7660 KVM: lapic: Track lapic timer advance per vCPU
Automatically adjusting the globally-shared timer advancement could
corrupt the timer, e.g. if multiple vCPUs are concurrently adjusting
the advancement value.  That could be partially fixed by using a local
variable for the arithmetic, but it would still be susceptible to a
race when setting timer_advance_adjust_done.

And because virtual_tsc_khz and tsc_scaling_ratio are per-vCPU, the
correct calibration for a given vCPU may not apply to all vCPUs.

Furthermore, lapic_timer_advance_ns is marked __read_mostly, which is
effectively violated when finding a stable advancement takes an extended
amount of timer.

Opportunistically change the definition of lapic_timer_advance_ns to
a u32 so that it matches the style of struct kvm_timer.  Explicitly
pass the param to kvm_create_lapic() so that it doesn't have to be
exposed to lapic.c, thus reducing the probability of unintentionally
using the global value instead of the per-vCPU value.

57bf67e73c KVM: lapic: Disable timer advancement if adaptive tuning goes haywire
To minimize the latency of timer interrupts as observed by the guest,
KVM adjusts the values it programs into the host timers to account for
the host's overhead of programming and handling the timer event.  Now
that the timer advancement is automatically tuned during runtime, it's
effectively unbounded by default, e.g. if KVM is running as L1 the
advancement can measure in hundreds of milliseconds.

Disable timer advancement if adaptive tuning yields an advancement of
more than 5000ns, as large advancements can break reasonable assumptions
of the guest, e.g. that a timer configured to fire after 1ms won't
arrive on the next instruction.  Although KVM busy waits to mitigate the
case of a timer event arriving too early, complications can arise when
shifting the interrupt too far, e.g. kvm-unit-test's vmx.interrupt test
will fail when its "host" exits on interrupts as KVM may inject the INTR
before the guest executes STI+HLT.   Arguably the unit test is "broken"
in the sense that delaying a timer interrupt by 1ms doesn't technically
guarantee the interrupt will arrive after STI+HLT, but it's a reasonable
assumption that KVM should support.

Furthermore, an unbounded advancement also effectively unbounds the time
spent busy waiting, e.g. if the guest programs a timer with a very large
delay.

5000ns is a somewhat arbitrary threshold.  When running on bare metal,
which is the intended use case, timer advancement is expected to be in
the general vicinity of 1000ns.  5000ns is high enough that false
positives are unlikely, while not being so high as to negatively affect
the host's performance/stability.

Note, a future patch will enable userspace to disable KVM's adaptive
tuning, which will allow priveleged userspace will to specifying an
advancement value in excess of this arbitrary threshold in order to
satisfy an abnormal use case.

c09d65d9ea KVM: x86: Consider LAPIC TSC-Deadline timer expired if deadline too short
If guest sets MSR_IA32_TSCDEADLINE to value such that in host
time-domain it's shorter than lapic_timer_advance_ns, we can
reach a case that we call hrtimer_start() with expiration time set at
the past.

Because lapic_timer.timer is init with HRTIMER_MODE_ABS_PINNED, it
is not allowed to run in softirq and therefore will never expire.

To avoid such a scenario, verify that deadline expiration time is set on
host time-domain further than (now + lapic_timer_advance_ns).

A future patch can also consider adding a min_timer_deadline_ns module parameter,
similar to min_timer_period_us to avoid races that amount of ns it takes
to run logic could still call hrtimer_start() with expiration timer set
at the past.

254272ce65 kvm: x86: Add memcg accounting to KVM allocations
There are many KVM kernel memory allocations which are tied to the life of
the VM process and should be charged to the VM process's cgroup. If the
allocations aren't tied to the process, the OOM killer will not know
that killing the process will free the associated kernel memory.
Add __GFP_ACCOUNT flags to many of the allocations which are not yet being
charged to the VM process's cgroup.

Tested:
	Ran all kvm-unit-tests on a 64 bit Haswell machine, the patch
	introduced no new failures.
	Ran a kernel memory accounting test which creates a VM to touch
	memory and then checks that the kernel memory allocated for the
	process is within certain bounds.
	With this patch we account for much more of the vmalloc and slab memory
	allocated for the VM.

There remain a few allocations which should be charged to the VM's
cgroup but are not. In x86, they include:
	vcpu->arch.pio_data
There allocations are unaccounted in this patch because they are mapped
to userspace, and accounting them to a cgroup causes problems. This
should be addressed in a future patch.

a7c42bb6da x86/kvm/lapic: preserve gfn_to_hva_cache len on cache reinit
vcpu->arch.pv_eoi is accessible through both HV_X64_MSR_VP_ASSIST_PAGE and
MSR_KVM_PV_EOI_EN so on migration userspace may try to restore them in any
order. Values match, however, kvm_lapic_enable_pv_eoi() uses different
length: for Hyper-V case it's the whole struct hv_vp_assist_page, for KVM
native case it is 8. In case we restore KVM-native MSR last cache will
be reinitialized with len=8 so trying to access VP assist page beyond
8 bytes with kvm_read_guest_cached() will fail.

Check if we re-initializing cache for the same address and preserve length
in case it was greater.

If kvm_apic_map_get_dest_lapic() finds a disabled LAPIC,
it will return with bitmap==0 and (*r == -1) will be returned to
userspace.

3b8a5df6c4 KVM: LAPIC: Tune lapic_timer_advance_ns automatically
In cloud environment, lapic_timer_advance_ns is needed to be tuned for every CPU
generations, and every host kernel versions(the kvm-unit-tests/tscdeadline_latency.flat
is 5700 cycles for upstream kernel and 9600 cycles for our 3.10 product kernel,
both preemption_timer=N, Skylake server).

This patch adds the capability to automatically tune lapic_timer_advance_ns
step by step, the initial value is 1000ns as 'commit d0659d946b ("KVM: x86:
add option to advance tscdeadline hrtimer expiration")' recommended, it will be
reduced when it is too early, and increased when it is too late. The guest_tsc
and tsc_deadline are hard to equal, so we assume we are done when the delta
is within a small scope e.g. 100 cycles. This patch reduces latency
(kvm-unit-tests/tscdeadline_latency, busy waits, preemption_timer enabled)
from ~2600 cyles to ~1200 cyles on our Skylake server.

d176620277 x86/kvm/lapic: always disable MMIO interface in x2APIC mode
When VMX is used with flexpriority disabled (because of no support or
if disabled with module parameter) MMIO interface to lAPIC is still
available in x2APIC mode while it shouldn't be (kvm-unit-tests):

The issue appears because we basically do nothing while switching to
x2APIC mode when APIC access page is not used. apic_mmio_{read,write}
only check if lAPIC is disabled before proceeding to actual write.

When APIC access is virtualized we correctly manipulate with VMX controls
in vmx_set_virtual_apic_mode() and we don't get vmexits from memory writes
in x2APIC mode so there's no issue.

Disabling MMIO interface seems to be easy. The question is: what do we
do with these reads and writes? If we add apic_x2apic_mode() check to
apic_mmio_in_range() and return -EOPNOTSUPP these reads and writes will
go to userspace. When lAPIC is in kernel, Qemu uses this interface to
inject MSIs only (see kvm_apic_mem_write() in hw/i386/kvm/apic.c). This
somehow works with disabled lAPIC but when we're in xAPIC mode we will
get a real injected MSI from every write to lAPIC. Not good.

The simplest solution seems to be to just ignore writes to the region
and return ~0 for all reads when we're in x2APIC mode. This is what this
patch does. However, this approach is inconsistent with what currently
happens when flexpriority is enabled: we allocate APIC access page and
create KVM memory region so in x2APIC modes all reads and writes go to
this pre-allocated page which is, btw, the same for all vCPUs.

Dan Carpenter reported that the untrusted data returns from kvm_register_read()
results in the following static checker warning:

  error: buffer underflow 'map->phys_map' 's32min-s32max'

KVM guest can easily trigger this by executing the following assembly sequence
in Ring0:

As this will cause KVM to execute the following code-path:
vmx_handle_exit() -> handle_vmcall() -> kvm_emulate_hypercall() -> kvm_pv_send_ipi()
which will reach out-of-bounds access.

This patch fixes it by adding a check to kvm_pv_send_ipi() against map->max_apic_id,
ignoring destinations that are not present and delivering the rest. We also check
whether or not map->phys_map[min + i] is NULL since the max_apic_id is set to the
max apic id, some phys_map maybe NULL when apic id is sparse, especially kvm
unconditionally set max_apic_id to 255 to reserve enough space for any xAPIC ID.

For x86 this brings in PCID emulation and CR3 caching for shadow page
 tables, nested VMX live migration, nested VMCS shadowing, an optimized
 IPI hypercall, and some optimizations.
 
 ARM will come next week.
 
 There is a semantic conflict because tip also added an .init_platform
 callback to kvm.c.  Please keep the initializer from this branch,
 and add a call to kvmclock_init (added by tip) inside kvm_init_platform
 (added here).
 
 Also, there is a backmerge from 4.18-rc6.  This is because of a
 refactoring that conflicted with a relatively late bugfix and
 resulted in a particularly hellish conflict.  Because the conflict
 was only due to unfortunate timing of the bugfix, I backmerged and
 rebased the refactoring rather than force the resolution on you.

4180bf1b65 KVM: X86: Implement "send IPI" hypercall
Using hypercall to send IPIs by one vmexit instead of one by one for
xAPIC/x2APIC physical mode and one vmexit per-cluster for x2APIC cluster
mode. Intel guest can enter x2apic cluster mode when interrupt remmaping
is enabled in qemu, however, latest AMD EPYC still just supports xapic
mode which can get great improvement by Exit-less IPIs. This patchset
lets a guest send multicast IPIs, with at most 128 destinations per
hypercall in 64-bit mode and 64 vCPUs per hypercall in 32-bit mode.

Hardware: Xeon Skylake 2.5GHz, 2 sockets, 40 cores, 80 threads, the VM

d8f2f498d9 x86/kvm: fix LAPIC timer drift when guest uses periodic mode
Since 4.10, commit 8003c9ae20 (KVM: LAPIC: add APIC Timer
periodic/oneshot mode VMX preemption timer support), guests using
periodic LAPIC timers (such as FreeBSD 8.4) would see their timers
drift significantly over time.

Differences in the underlying clocks and numerical errors means the
periods of the two timers (hv and sw) are not the same. This
difference will accumulate with every expiry resulting in a large
error between the hv and sw timer.

This means the sw timer may be running slow when compared to the hv
timer. When the timer is switched from hv to sw, the now active sw
timer will expire late. The guest VCPU is reentered and it switches to
using the hv timer. This timer catches up, injecting multiple IRQs
into the guest (of which the guest only sees one as it does not get to
run until the hv timer has caught up) and thus the guest's timer rate
is low (and becomes increasing slower over time as the sw timer lags
further and further behind).

I believe a similar problem would occur if the hv timer is the slower
one, but I have not observed this.

Fix this by synchronizing the deadlines for both timers to the same
time source on every tick. This prevents the errors from accumulating.

Previously, we toggled between SECONDARY_EXEC_VIRTUALIZE_X2APIC_MODE
and SECONDARY_EXEC_VIRTUALIZE_APIC_ACCESSES, depending on whether or
not the EXTD bit was set in MSR_IA32_APICBASE. However, if the local
APIC is disabled, we should not set either of these APIC
virtualization control bits.

Since the commit "8003c9ae204e: add APIC Timer periodic/oneshot mode VMX
preemption timer support", a Windows 10 guest has some erratic timer
spikes.

Here the results on a 150000 times 1ms timer without any load:

Here the results on a 150000 times 1ms timer with a cpu-z stress test:

The root cause of the problem is starting hrtimer with an expiry time
already in the past can take more than 20 milliseconds to trigger the
timer function.  It can be solved by forward such past timers
immediately, rather than submitting them to hrtimer_start().
In case the timer is periodic, update the target expiration and call
hrtimer_start with it.

0bcc3fb95b KVM: lapic: stop advertising DIRECTED_EOI when in-kernel IOAPIC is in use
Devices which use level-triggered interrupts under Windows 2016 with
Hyper-V role enabled don't work: Windows disables EOI broadcast in SPIV
unconditionally. Our in-kernel IOAPIC implementation emulates an old IOAPIC
version which has no EOI register so EOI never happens.

While this is a guest OS bug (it should check that IOAPIC has the required
capabilities before disabling EOI broadcast) we can workaround it in KVM:
advertising DIRECTED_EOI with in-kernel IOAPIC makes little sense anyway.

12806ba937 KVM: lapic: Fixup LDR on load in x2apic
In x2apic mode the LDR is fixed based on the ID rather
than separately loadable like it was before x2.
When kvm_apic_set_state is called, the base is set, and if
it has the X2APIC_ENABLE flag set then the LDR is calculated;
however that value gets overwritten by the memcpy a few lines
below overwriting it with the value that came from userland.

The symptom is a lack of EOI after loading the state
(e.g. after a QEMU migration) and is due to the EOI bitmap
being wrong due to the incorrect LDR.  This was seen with
a Win2016 guest under Qemu with irqchip=split whose USB mouse
didn't work after a VM migration.

4191db26b7 KVM: x86: Update APICv on APIC reset
In kvm_apic_set_state() we update the hardware virtualized APIC after
the full APIC state has been overwritten. Do the same, when the full
APIC state has been reset in kvm_lapic_reset().

This updates some hardware state that was previously forgotten, as
far as I can tell. Also, this allows removing some APIC-related reset
code from vmx_vcpu_reset().

The description in the Intel SDM of how the divide configuration
register is used: "The APIC timer frequency will be the processor's bus
clock or core crystal clock frequency divided by the value specified in
the divide configuration register."

Observation of baremetal shown that when the TDCR is change, the TMCCT
does not change or make a big jump in value, but the rate at which it
count down change.

The patch update the emulation to APIC timer to so that a change to the
divide configuration would be reflected in the value of the counter and
when the next interrupt is triggered.

dedf9c5e21 KVM: LAPIC: Keep timer running when switching between one-shot and periodic mode
If we take TSC-deadline mode timer out of the picture, the Intel SDM
does not say that the timer is disable when the timer mode is change,
either from one-shot to periodic or vice versa.

After this patch, the timer is no longer disarmed on change of mode, so
the counter (TMCCT) keeps counting down.

So what does a write to LVTT changes ? On baremetal, the change of mode
is probably taken into account only when the counter reach 0. When this
happen, LVTT is use to figure out if the counter should restard counting
down from TMICT (so periodic mode) or stop counting (if one-shot mode).

This patch is based on observation of the behavior of the APIC timer on
baremetal as well as check that they does not go against the description
written in the Intel SDM.

72c139bacf KVM: hyperv: support HV_X64_MSR_TSC_FREQUENCY and HV_X64_MSR_APIC_FREQUENCY
It has been experimentally confirmed that supporting these two MSRs is one
of the necessary conditions for nested Hyper-V to use the TSC page. Modern
Windows guests are noticeably slower when they fall back to reading
timestamps from the HV_X64_MSR_TIME_REF_COUNT MSR instead of using the TSC
page.

The newly supported MSRs are advertised with the AccessFrequencyRegs
partition privilege flag and CPUID.40000003H:EDX[8] "Support for
determining timer frequencies is available" (both outside of the scope of
this KVM patch).

c853354429 KVM: LAPIC: Fix lapic timer injection delay
If the TSC deadline timer is programmed really close to the deadline or
even in the past, the computation in vmx_set_hv_timer will program the
absolute target tsc value to vmcs preemption timer field w/ delta == 0,
then plays a vmentry and an upcoming vmx preemption timer fire vmexit
dance, the lapic timer injection is delayed due to this duration. Actually
the lapic timer which is emulated by hrtimer can handle this correctly.

This patch fixes it by firing the lapic timer and injecting a timer interrupt
immediately during the next vmentry if the TSC deadline timer is programmed
really close to the deadline or even in the past. This saves ~300 cycles on
the tsc_deadline_timer test of apic.flat.

52b5419016 KVM: x86: Fix virtual wire mode
Intel SDM says, that at most one LAPIC should be configured with ExtINT
delivery. KVM configures all LAPICs this way. This causes pic_unlock()
to kick the first available vCPU from the internal KVM data structures.
If this vCPU is not the BSP, but some not-yet-booted AP, the BSP may
never realize that there is an interrupt.

Fix that by enabling ExtINT delivery only for the BSP.

This allows booting a Linux guest without a TSC in the above situation.
Otherwise the BSP gets stuck in calibrate_delay_converge().

Patch series "kvmalloc", v5.

There are many open coded kmalloc with vmalloc fallback instances in the
tree.  Most of them are not careful enough or simply do not care about
the underlying semantic of the kmalloc/page allocator which means that
a) some vmalloc fallbacks are basically unreachable because the kmalloc
part will keep retrying until it succeeds b) the page allocator can
invoke a really disruptive steps like the OOM killer to move forward
which doesn't sound appropriate when we consider that the vmalloc
fallback is available.

As it can be seen implementing kvmalloc requires quite an intimate
knowledge if the page allocator and the memory reclaim internals which
strongly suggests that a helper should be implemented in the memory
subsystem proper.

Most callers, I could find, have been converted to use the helper
instead.  This is patch 6.  There are some more relying on __GFP_REPEAT
in the networking stack which I have converted as well and Eric Dumazet
was not opposed [2] to convert them as well.

This patch (of 9):

Using kmalloc with the vmalloc fallback for larger allocations is a
common pattern in the kernel code.  Yet we do not have any common helper
for that and so users have invented their own helpers.  Some of them are
really creative when doing so.  Let's just add kv[mz]alloc and make sure
it is implemented properly.  This implementation makes sure to not make
a large memory pressure for > PAGE_SZE requests (__GFP_NORETRY) and also
to not warn about allocation failures.  This also rules out the OOM
killer as the vmalloc is a more approapriate fallback than a disruptive
user visible action.

This patch also changes some existing users and removes helpers which
are specific for them.  In some cases this is not possible (e.g.
ext4_kvmalloc, libcfs_kvzalloc) because those seems to be broken and
require GFP_NO{FS,IO} context which is not vmalloc compatible in general
(note that the page table allocation is GFP_KERNEL).  Those need to be
fixed separately.

While we are at it, document that __vmalloc{_node} about unsupported gfp
mask because there seems to be a lot of confusion out there.
kvmalloc_node will warn about GFP_KERNEL incompatible (which are not
superset) flags to catch new abusers.  Existing ones would have to die
slowly.

This reverts commit bbd6411513.

I've been sitting on this revert for too long and it unfortunately
missed 4.11.  It's also the reason why I haven't merged ring-based
dirty tracking for 4.12.

Using kvm_vcpu_memslots in kvm_gfn_to_hva_cache_init and
kvm_vcpu_write_guest_offset_cached means that the MSR value can
now be used to access SMRAM, simply by making it point to an SMRAM
physical address.  This is problematic because it lets the guest
OS overwrite memory that it shouldn't be able to touch.

Provide versions of struct gfn_to_hva_cache functions that
take vcpu as a parameter instead of struct kvm.  The existing functions
are not needed anymore, so delete them.  This allows dirty pages to
be logged in the vcpu dirty ring, instead of the global dirty ring,
for ring-based dirty memory tracking.

b95234c840 kvm: x86: do not use KVM_REQ_EVENT for APICv interrupt injection
Since bf9f6ac8d7 ("KVM: Update Posted-Interrupts Descriptor when vCPU
is blocked", 2015-09-18) the posted interrupt descriptor is checked
unconditionally for PIR.ON.  Therefore we don't need KVM_REQ_EVENT to
trigger the scan and, if NMIs or SMIs are not involved, we can avoid
the complicated event injection path.

Calling kvm_vcpu_kick if PIR.ON=1 is also useless, though it has been
there since APICv was introduced.

However, without the KVM_REQ_EVENT safety net KVM needs to be much
more careful about races between vmx_deliver_posted_interrupt and
vcpu_enter_guest.  First, the IPI for posted interrupts may be issued
between setting vcpu->mode = IN_GUEST_MODE and disabling interrupts.
If that happens, kvm_trigger_posted_interrupt returns true, but
smp_kvm_posted_intr_ipi doesn't do anything about it.  The guest is
entered with PIR.ON, but the posted interrupt IPI has not been sent
and the interrupt is only delivered to the guest on the next vmentry
(if any).  To fix this, disable interrupts before setting vcpu->mode.
This ensures that the IPI is delayed until the guest enters non-root mode;
it is then trapped by the processor causing the interrupt to be injected.

Second, the IPI may be issued between kvm_x86_ops->sync_pir_to_irr(vcpu)
and vcpu->mode = IN_GUEST_MODE.  In this case, kvm_vcpu_kick is called
but it (correctly) doesn't do anything because it sees vcpu->mode ==
OUTSIDE_GUEST_MODE.  Again, the guest is entered with PIR.ON but no
posted interrupt IPI is pending; this time, the fix for this is to move
the RVI update after IN_GUEST_MODE.

Both issues were mostly masked by the liberal usage of KVM_REQ_EVENT,
though the second could actually happen with VT-d posted interrupts.
In both race scenarios KVM_REQ_EVENT would cancel guest entry, resulting
in another vmentry which would inject the interrupt.

This saves about 300 cycles on the self_ipi_* tests of vmexit.flat.

Calls to apic_find_highest_irr are scanning IRR twice, once
in vmx_sync_pir_from_irr and once in apic_search_irr.  Change
sync_pir_from_irr to get the new maximum IRR from kvm_apic_update_irr;
now that it does the computation, it can also do the RVI write.

In order to avoid complications in svm.c, make the callback optional.

cef84c302f KVM: x86: flush pending lapic jump label updates on module unload
KVM's lapic emulation uses static_key_deferred (apic_{hw,sw}_disabled).
These are implemented with delayed_work structs which can still be
pending when the KVM module is unloaded. We've seen this cause kernel
panics when the kvm_intel module is quickly reloaded.

Use the new static_key_deferred_flush() API to flush pending updates on
module unload.

5bd5db385b KVM: x86: allow hotplug of VCPU with APIC ID over 0xff
LAPIC after reset is in xAPIC mode, which poses a problem for hotplug of
VCPUs with high APIC ID, because reset VCPU is waiting for INIT/SIPI,
but there is no way to uniquely address it using xAPIC.

From many possible options, we chose the one that also works on real
hardware: accepting interrupts addressed to LAPIC's x2APIC ID even in
xAPIC mode.

KVM intentionally differs from real hardware, because real hardware
(Knights Landing) does just "x2apic_id & 0xff" to decide whether to
accept the interrupt in xAPIC mode and it can deliver one interrupt to
more than one physical destination, e.g. 0x123 to 0x123 and 0x23.

ad3610919e kvm: x86: avoid atomic operations on APICv vmentry
On some benchmarks (e.g. netperf with ioeventfd disabled), APICv
posted interrupts turn out to be slower than interrupt injection via
KVM_REQ_EVENT.

This patch optimizes a bit the IRR update, avoiding expensive atomic
operations in the common case where PI.ON=0 at vmentry or the PIR vector
is mostly zero.  This saves at least 20 cycles (1%) per vmexit, as
measured by kvm-unit-tests' inl_from_qemu test (20 runs):

8003c9ae20 KVM: LAPIC: add APIC Timer periodic/oneshot mode VMX preemption timer support
Most windows guests still utilize APIC Timer periodic/oneshot mode
instead of tsc-deadline mode, and the APIC Timer periodic/oneshot
mode are still emulated by high overhead hrtimer on host. This patch
converts the expected expire time of the periodic/oneshot mode to
guest deadline tsc in order to leverage VMX preemption timer logic
for APIC Timer tsc-deadline mode. After each preemption timer vmexit
preemption timer is restarted to emulate LVTT current-count register
is automatically reloaded from the initial-count register when the
count reaches 0. This patch reduces ~5600 cycles for each APIC Timer
periodic mode operation virtualization.

 The ugly bit is the conflicts.  A couple of them are simple conflicts due
 to 4.7 fixes, but most of them are with other trees. There was definitely
 too much reliance on Acked-by here.  Some conflicts are for KVM patches
 where _I_ gave my Acked-by, but the worst are for this pull request's
 patches that touch files outside arch/*/kvm.  KVM submaintainers should
 probably learn to synchronize better with arch maintainers, with the
 latter providing topic branches whenever possible instead of Acked-by.
 This is what we do with arch/x86.  And I should learn to refuse pull
 requests when linux-next sends scary signals, even if that means that
 submaintainers have to rebase their branches.
 
 Anyhow, here's the list:
 
 - arch/x86/kvm/vmx.c: handle_pcommit and EXIT_REASON_PCOMMIT was removed
 by the nvdimm tree.  This tree adds handle_preemption_timer and
 EXIT_REASON_PREEMPTION_TIMER at the same place.  In general all mentions
 of pcommit have to go.
 
 There is also a conflict between a stable fix and this patch, where the
 stable fix removed the vmx_create_pml_buffer function and its call.
 
 - virt/kvm/kvm_main.c: kvm_cpu_notifier was removed by the hotplug tree.
 This tree adds kvm_io_bus_get_dev at the same place.
 
 - virt/kvm/arm/vgic.c: a few final bugfixes went into 4.7 before the
 file was completely removed for 4.8.
 
 - include/linux/irqchip/arm-gic-v3.h: this one is entirely our fault;
 this is a change that should have gone in through the irqchip tree and
 pulled by kvm-arm.  I think I would have rejected this kvm-arm pull
 request.  The KVM version is the right one, except that it lacks
 GITS_BASER_PAGES_SHIFT.
 
 - arch/powerpc: what a mess.  For the idle_book3s.S conflict, the KVM
 tree is the right one; everything else is trivial.  In this case I am
 not quite sure what went wrong.  The commit that is causing the mess
 (fd7bacbca4, "KVM: PPC: Book3S HV: Fix TB corruption in guest exit
 path on HMI interrupt", 2016-05-15) touches both arch/powerpc/kernel/
 and arch/powerpc/kvm/.  It's large, but at 396 insertions/5 deletions
 I guessed that it wasn't really possible to split it and that the 5
 deletions wouldn't conflict.  That wasn't the case.
 
 - arch/s390: also messy.  First is hypfs_diag.c where the KVM tree
 moved some code and the s390 tree patched it.  You have to reapply the
 relevant part of commits 6c22c98637, plus all of e030c1125e, to
 arch/s390/kernel/diag.c.  Or pick the linux-next conflict
 resolution from http://marc.info/?l=kvm&m=146717549531603&w=2.
 Second, there is a conflict in gmap.c between a stable fix and 4.8.
 The KVM version here is the correct one.

1767e931e3 x86/kvm: Audit and remove any unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends.  That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig.  In the case of
kvm where it is modular, we can extend that to also include files
that are building basic support functionality but not related
to loading or registering the final module; such files also have
no need whatsoever for module.h

The advantage in removing such instances is that module.h itself
sources about 15 other headers; adding significantly to what we feed
cpp, and it can obscure what headers we are effectively using.

Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each instance for the
presence of either and replace as needed.

Several instances got replaced with moduleparam.h since that was
really all that was required for those particular files.

af1bae5497 KVM: x86: bump KVM_MAX_VCPU_ID to 1023
kzalloc was replaced with kvm_kvzalloc to allow non-contiguous areas and
rcu had to be modified to cope with it.

The practical limit for KVM_MAX_VCPU_ID right now is INT_MAX, but lower
value was chosen in case there were bugs.  1023 is sufficient maximum
APIC ID for 288 VCPUs.

c519265f2a KVM: x86: add a flag to disable KVM x2apic broadcast quirk
Add KVM_X2APIC_API_DISABLE_BROADCAST_QUIRK as a feature flag to
KVM_CAP_X2APIC_API.

The quirk made KVM interpret 0xff as a broadcast even in x2APIC mode.
The enableable capability is needed in order to support standard x2APIC and
remain backward compatible.

3713131345 KVM: x86: add KVM_CAP_X2APIC_API
KVM_CAP_X2APIC_API is a capability for features related to x2APIC
enablement.  KVM_X2APIC_API_32BIT_FORMAT feature can be enabled to
extend APIC ID in get/set ioctl and MSI addresses to 32 bits.
Both are needed to support x2APIC.

The feature has to be enableable and disabled by default, because
get/set ioctl shifted and truncated APIC ID to 8 bits by using a
non-standard protocol inspired by xAPIC and the change is not
backward-compatible.

Changes to MSI addresses follow the format used by interrupt remapping
unit.  The upper address word, that used to be 0, contains upper 24 bits
of the LAPIC address in its upper 24 bits.  Lower 8 bits are reserved as
0.  Using the upper address word is not backward-compatible either as we
didn't check that userspace zeroed the word.  Reserved bits are still
not explicitly checked, but non-zero data will affect LAPIC addresses,
which will cause a bug.
