7640e1eb8c scsi: pm80xx: Make mpi_build_cmd locking consistent
Driver submits all internal requests (like abort_task, event acknowledgment
etc.) through inbound queue 0. While submitting those, driver does not
acquire any lock and this may lead to a race when there is an I/O request
coming in on CPU0 and submitted through inbound queue 0.  To avoid this,
lock acquisition has been moved to pm8001_mpi_build_cmd().  All command
submission will go through this path.

5a141315ed scsi: pm80xx: Increase the number of outstanding I/O supported to 1024
The pm80xx driver currently sets the controller queue depth to
256. Hoewver, the controller supports outstanding I/Os up 1024.

Increase the number of outstanding I/Os from 256 to 1024.  CCBs and tags
are allocated according to outstanding I/Os. Also update the can_queue
value (max_out_io - PM8001_RESERVE_SLOT) used by the SCSI midlayer.

27bc43bd7c scsi: pm80xx: Remove DMA memory allocation for ccb and device structures
Remove DMA memory allocation for Devices and CCB structure. Instead
allocate memory outside of DMA memory.  DMA memory is a limited system
resource and it is better to allocate memory outside of DMA memory when
possible.

05c6c029a4 scsi: pm80xx: Increase number of supported queues
Current driver uses fixed number of Inbound and Outbound queues and all of
the I/O, TMF and internal requests are submitted through those. A global
spin lock is used to control the shared access. This can create a lock
contention and it is real bottleneck in the I/O path.

To avoid this, the number of supported Inbound and Outbound queues is
increased to 64, and the number of queues used is decided based on number
of CPU cores online and number of MSI-X vectors allocated. Also add locks
per queue instead of using the global lock.

3f649ab728 treewide: Remove uninitialized_var() usage
Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.

In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

for x86_64, i386, arm64, arm, powerpc, powerpc64le, s390x, mips, sparc64,
alpha, and m68k.

dba2cc03b9 scsi: pm80xx: sysfs attribute for non fatal dump
Added the sysfs attribute for non fatal log so that management utility can
get the non fatal dump from driver. The non-fatal error is an error
condition or abnormal behavior detected by the host, or detected and
reported by the controller to the host.The non-fatal error does not stop
the controller firmware and enables it to still respond to host requests.
A typical example of a non-fatal error is an I/O timeout or an unusual
error notification from the controller. Since the firmware is operational,
the error dump information is pushed to host memory (by firmware) upon
request from the host.

9d9c7c20fb scsi: pm80xx: Free the tag when mpi_set_phy_profile_resp is received
In pm80xx driver, the command mpi_set_phy_profile_req is sent by host
during boot to configure the phy profile such as analog setting page, rate
control page. However, the tag is not freed when its response is
received. As a result, 16 tags are missing for each HBA after boot.  When
NCQ is enabled with queue depth 16, it needs at least, 15 * 16 = 240 tags
for each HBA to achieve the best performance. In current pm80xx driver with
setting CCB_MAX = 256, the total number of tags in each HBA is 255 for data
IO. Hence, without returning those tags to the pool after boot, some device
will finally be forced to non-ncq mode by ATA layer due to excessive errors
(i.e. LLDD cannot allocate tag for queued task).

A kexec reboot causes the controller fw to assert. This assertion shows up
in two ways, the controller doesn't show up as ready and an interrupt is
waiting as soon as the handler is registered. To resolve this added below
fix:

 - Split the interrupt handling setup into two parts, setup and request.

 - If the controller ready register indicates not-ready, but that the not
   readiness is only on the IOC units we can still try a reset to bring the
   system back to the pre-reboot state.

58bf14c17e scsi: pm80xx: Increase request sg length
Increasing the per-request size maximum (max_sectors_kb) runs into the
per-device DMA scatter gather list limit (max_segments) for users of the io
vector system calls (eg, readv and writev). This is because the kernel
combines io vectors into DMA segments when possible, but it doesn't work
for our user because the vectors in the buffer cache get scrambled.  This
change bumps the advertised max scatter gather length to 528 to cover 2M w/
x86's 4k pages and some extra for the user checksum.  It trims the size of
some of the tables we don't care about and exposes all of the command slots
upstream to the SCSI layer.  Also reduced the PM8001_MAX_CCB to 256 as
pm8001 driver has memory limit depend on machine capability. If we increase
the sg length, we need to trade-off it by decreasing PM8001_MAX_CCB.
PM8001_MAX_CCB = 256 does not have any influence on normal use

75c0b0e118 compat_ioctl: scsi: handle HDIO commands from drivers
The ata_sas_scsi_ioctl() function implements a number of HDIO_* commands
for SCSI devices, it is used by all libata drivers as well as a few
drivers that support SAS attached SATA drives.

The only command that is not safe for compat ioctls here is
HDIO_GET_32BIT. Change the implementation to check for in_compat_syscall()
in order to do both cases correctly, and change all callers to use it
as both native and compat callback pointers, including the indirect
callers through sas_ioctl and ata_scsi_ioctl.

The condition (reg_val != 2) || (reg_val != 3) will always be true because
reg_val cannot be equal to two different values at the same time. Fix this

is not a 2 and not a 3 as was originally intended.

51c1c5f6ed scsi: pm80xx: Cleanup command when a reset times out
Added the fix so the if driver properly sent the abort it tries to remove
it from the firmware's list of outstanding commands regardless of the abort
status. This means that the task gets freed 'now' rather than possibly
getting freed later when the scsi layer thinks it's leaked but still valid.

91a43fa61f scsi: pm80xx: Fix command issue sizing
The commands to the controller are sent in fixed sized chunks which are set
per-chip-generation and stashed in iomb_size. The driver fills in structs
matching the register layout and memcpy this to memory shared with the
controller. However, there are two problem cases:

	1) Things like phy_start_req are too large because they share the
	sas_identify_frame definition with libsas, and it includes the crc
	word. This means that it's overwriting the start of the next
	command block, that's ok except if it happens at the end of the
	shared memory area.

	2) Things like set_nvm_data_req which are shared between the HAL
	layers. This means that it's sending 'random' data for things that
	are in the reserved area. So far we haven't found a case where the
	controller FW cares, but sending possible gibberish (for most of
	the structures this is in the reserved area so previously zeroed)
	is not recommended.

a88d9db94c scsi: pm80xx: Fix dereferencing dangling pointer
sas_task structure should not be used after task_done is called.  If the
device is gone or not attached, we call task_done on t and continue to use
in the sas_task in rest of the function. task_done is pointing to
sas_ata_task_done, may free the memory associated with the task before
returning.

The default logging doesn't include the device name, so it's difficult to
determine which controller is being logged about in error scenarios. The
logging level was only settable via sysfs, which made it inconvenient for
actual debugging. This changes the default to only cover error handling.

4daf1ef3c6 scsi: pm80xx: Convert 'long' mdelay to msleep
For delays longer than 20ms [um]delay isn't recommended.

pm80xx_chip_soft_rst starts off with a 500ms delay before it even gets
around to checking for the results of the reset. As long as it's at least
500ms it doesn't matter what the scheduler is doing.  The delay in the
pm8001_exec_internal_task_abort does nothing, and theory is this is a delay
to avoid a double-free.

Code that iterates over all standard PCI BARs typically uses
PCI_STD_RESOURCE_END.  However, that requires the unusual test
"i <= PCI_STD_RESOURCE_END" rather than something the typical
"i < PCI_STD_NUM_BARS".

Add a definition for PCI_STD_NUM_BARS and change loops to use the more
idiomatic C style to help avoid fencepost errors.

Disabling the SATA drive interface cause kernel panic. When the drive
Interface is disabled, device should be deregistered after aborting all

2a3d4eb8e2 scsi: flip the default on use_clustering
Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page.  Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

72349b62a5 scsi: pm80xx: Fixed system hang issue during kexec boot
When the firmware is not responding, execution of kexec boot causes a system
hang. When firmware assertion happened, driver get notified with interrupt
vector updated in MPI configuration table. Then, the driver will read
scratchpad register and set controller_fatal_error flag to true.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

     hardware properties, i.e. don't inflict the write at the 0.5
     seconds boundary which originates from the PC CMOS RTC to all RTCs.
     No functional change as drivers need to be updated separately.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

 - when the two scanners agreed on the detected license(s), that became

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

when there's an error in 'ncq mode' the host has to read the ncq error
log (10h) to clear the error state. however, the ccb that is setup for
doing this doesn't setup the ccb so that the previous state is
cleared. if the ccb was previously used for an IO n_elems is set and
pm8001_ccb_task_free() treats this as the signal to go free a
scatter-gather list (that's already been freed).

In function pm8001_pci_probe(), on errors that the control flow jumps to
label err_out_ha_free, function pm8001_free() is called. In pm8001_free(),
scsi_host_put() is called to release shost, which keeps the return value
of scsi_host_alloc(). After pm8001_free() returns, kfree() is called to
free shost again, resulting in a double free bug. This patch removes
scsi_host_put() from pm8001_free() and explicitly calls scsi_host_put()
to release Scsi_Host in need.

e83596b41c pm80xx: Remove bogus address masking in pm8001_ioremap()
It is unclear what the original intent of the masking was, but it is
clearly incorrect to truncate a physical address before calling
ioremap().  On systems where there are valid physical address bits above
bit-31 (arm64 for example) the result is an eventual OOPs when
initializing the driver.

Remove the bogus code to fix it.

This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with

2a188cb42b pm80xx: remove the SCSI host before detaching from SAS transport
Previously, when this module was unloaded via 'rmmod' with at least one
drive attached, the SCSI error handler thread would become stuck in an
infinite recovery loop and lockup the system, necessitating a reboot.

Once the SAS layer is detached, the driver will fail any subsequent
commands since the target devices are removed.  However, removing the
SCSI host generates a SYNCHRONIZE CACHE (10) command, which was failed
and left the error handler no method of recovery.

This patch simply removes the SCSI host first so that no more commands
can come down, prior to cleaning up the SAS layer.  Note that the stack
is built up with the SCSI host first, and then the SAS layer.  Perhaps
it should be reversed for symmetry, so that commands cannot be sent to
the pm80xx driver prior to attaching the SAS layer?

What was really strange about this bug was that it was introduced at
commit cff549e486 ("[SCSI]: proper state checking and module refcount
handling in scsi_device_get").  This commit appears to tinker with how
the reference counting is performed for SCSI device objects.  My theory
is that prior to this commit, the refcount for a device object was
blindly incremented at some point during the teardown process which
coincidentially made the device stick around during the procedure, which
also coincidentially made any commands sent to the driver not fail
(since the device was technically still "there").  After this commit was
applied, my theory is the refcount for the device object is not being
incremented at a specific point anymore, which makes the device go away,
and thus made the pm80xx driver fail any subsequent commands.

c913df3f3d pm80xx: avoid a panic if MSI(X) interrupts are disabled
If MSI(X) interrupts are disabled via the kernel command line
(pci=nomsi), the pm8001 driver will kernel panic because it does not
detect that MSI interrupts are disabled and will soldier on and attempt to
configure MSI interrupts anyways.  This leads to a kernel panic, most
likely because a required data structure is not available down the
line.  Using the pci_msi_enabled() function in order to detect if MSI
interrupts are enabled before configuring them resolves this issue and
avoids a kernel panic when the module is loaded.  Additionally, the
irq_vector structure must be initialized when legacy interrupts are
being used otherwise legacy interrupts will simply not function and
result in another panic.

b650a8806e pm80xx: wait a minimum of 500ms before issuing commands to SPCv
The documentation for the 8070 and 8072 SPCv chip explicitly states that
a minimum of 500ms must elapse before issuing commands, otherwise the
SPCv may not process them and the firmware may get into an unrecoverable
state requiring a reboot.  While the Linux guys will probably think this
is 'racy', it is called out in the chip documentation and inserting this
delay makes power management function properly.

faf321b0b7 pm80xx: do not examine registers for iButton feature if ATTO adapter
ATTO adapters do not support this feature.  If the firmware fails to be
ready, it should not check the examined registers in order to examine
the state of the feature in order to prevent undefined behavior.

c5614df7ff pm80xx: set PHY profiles for ATTO 12Gb SAS controllers
PHY profiles are not saved in NVRAM on ATTO 12Gb SAS controllers.
Therefore, in order for the controller to function in a wide range of
configurations, the PHY profiles must be statically set.  This patch
provides the necessary functionality to do so.

10efa460fe pm80xx: add support for ATTO devices during SAS address initiailization
ATTO SAS controllers retrieve the SAS address from the NVRAM in a location
different from non-ATTO PMC Sierra SAS controllers.  This patch makes the
necessary adjustments in order to retrieve the SAS address on these types
of adapters.
